If she is acute pyelonephritis only not recurrent, you are right tx:
blood culture and iv antibiotics .
But this case:Her history” she has vesicoureteral reflux and a history of
recurrent urinary tract infections”, do we need to take a CT scan and
Ultrasound first?

那个婴儿的病是“Exanthem subitum” 吗， 不太记得了， 在 KAPLAN NOTES

Routine imagine is not required. However, if pt does not improve in 2-3days,
complications are suspected, ultrasound or CT scan can be done.
