I think you should take the offer.
First and most important, you secure the position (H1 visa) and have time to
finish your PhD. And big money after graduation no matter where you
graduate.
Second, no fellowship in house is not a big problem because of your research
background and if you keep doing clinical research. Certain programs has in
house fellowship are good too, but the resident will try to kill each other
for the position. Let alone we are minority compared to other ethnic groups
.
Third, all the people who work hard to try to enter the door of US doctor

我本人倒是真的认识一个学姐，resident去的很一般的community医院，但她毕业将近
20年，所以可能和你没有可比性。但fellow去的很好的大学program,因为她research
background 很强。
我的point是：如果community医院住院医的资历并不影响今后fellow的话，还是可以去

好像以前Jimmyzhang有个帖子是说这个的，觉得很有道理----是说把你要去iv的地方给
排个顺序先，如果这个给prematch的program排在第N位之前，就从了，否则相反。这个
N是几我忘了，不敢乱说。至于怎么排，你综合自己的情况考虑了，career plan，家庭
因素。
我觉得，无论去了哪里，都要先说服自己才好。因为将来多少都会有不顺利的时候，有
的人多有的人少而已。怕就怕将来遇到事儿后悔当初为什么没选别的地方，难免抱怨环
境而不是从自己身上想办法提高，那干起来心情也不会好，影响今后的performance就
得不偿失了。
所以，楼主想清楚了就做决定。一旦做出决定了（无论接不接这个offer），就相信这
是我能做出的最佳选择，高高兴兴地去面对将来的住院生活。

好吧，我觉得楼主确实很有压力，因为人有选择的时候比没有选择的时候更痛苦。 我
坚决赞成楼主拿下这个pre-match. 我朋友的一个例子特别有参考价值。 他从协和毕业
，一直是班上的第一名 （不要人肉了）， 毕业出国在一所人人皆知的学校读PhD， 同
时考版，都是99， 口语也非常好。 4年以前第一次match也是拿了十几个IV，（他只申
请了50个左右，牛人啊）， 结果有3个给了prematch，呵呵，据他说就是因为有了
prematch， 信心暴增，非要得到他最想去的前5个program， 结果居然没有match上，
更奇怪的是第二年没有任何prematch， 最后match到一个program， 其实跟他锯掉的那
些差不多。他浪费了一年，而且，那一年压力暴大， 毕业，身份。他非常后悔， 他说
如果他早接受prematch， 就不会有后面的问题。不过牛人还是牛人，他在那个so so的
program做的非常好，PD给的推荐信很好，马上要到这边来做fellow了。
当然，如果没有毕业啊，身份问题啊，等待也是值得的。

我也听说有pre-match据掉，后来没有match上的个案。不过我认识一个朋友，背景绝对
比lz弱，去年老早拿到一个prematch，他坚决的据了，伊一心进univeristy program，
后来花了整整1－2个月到处去面试，后来如愿以偿的去了university program。
就看lz有多么的想进univerit program了～～～

如果你以后想做fellow，手头还有十几个IV包括大学医院的，那就等等看，虽说老中有
research申请fellow有优势，但在社区医院比大学医院还是差远了，主要是比较难拿到
牛人推荐信，尤其是你想去比较热的专业的话。
如果只是想顺顺利利熬过这三年就出来practice，如果这个program你各方面都感觉不
错，resident也显得比较happy的话，那可以考虑拿了，会节省很多精力和时间。

我今天也得到了Prematch。也在犹豫。我只面了两个地方，还有17个排在后面。其中有
几个非常好的大学program。但我的条件和盘片不同，我已经毕业19年了，明年进
program时就20年了。能拿到这么多面试很出乎我的意料。
这个program的条件和盘片说的差不多。我犹豫的不是非得进好的program不可，是因为
这个program离家有近300迈，孩子在我目前的城市的很好的学校上学，如果接受offer
，我只能自己move过去。在家附近还有两个IV，可都排在了11月下旬。不可能等到那时
再给答复接不接受prematch。

credential：252/99， 245/99， CS passed on the first attempt.step3还没考。7
年的国内内科临床经验。现在top大学的faculty,做research,有自己的grant.有几篇很
不错的paper。
我想给prematch的program和大部分其他的给我IV的program看中的是我的科研背景和临
床经验，面试时再三问我打不打算以后做fellow。
从我自身看来，年轻当然更好，但岁数大好像不是program拒绝人的主要原因。主要是
他们看中的东西你具备不具备。再有看你的诚意如何。

情况MS情况还得起码荒废两三年做FELLOW,有这样的背景到哪儿作FELLOW都不吃亏.不

只是感觉一步一个脚印的过来了，每一步都走得很用心很努力，尽管每一步干的都还不
错但都不是十分出色，比我出色的有的是。但都算尽了心，给每个老板的印象都是脚踏
实地，勤奋努力。
所以每一步都没白干，我一共提交了四封推荐信，每封都很过硬，很过硬，没有滥竽充
数的。我的一个前老板告诉我，他从没给别人写过这么好的推荐信。估计他并不是为了
领人情。

感觉USMLE的意见比较好。
LZ学术条件不错，既然已经拿到PreMatch，说明面试也蛮好。Program的择人标准大同
小异，你再拿PreMatch的可能性还是挺大的。现在这个Program听起来是差了点， 如果
没有去名校作Elective的机会，去好的Fellowship program的机会比较小。
没有绿卡是个大问题。从Match冲名校胜算难料。

hands on USCE? I think it is almost impossible to most phd students taking
board here.Any suggestions and tricks?

拿到了prematch，很是兴奋。被很多人颂为大牛，很是羞愧难当。我这个年龄,远的不

,我都算不得成功.我只是拿的IV多了点就成了所谓牛人，确实倍感惭愧。就连我周围的
朋友,同事也没有谁认为我是牛人.得到的IV多一些,一是成绩和其他背景还可以,更重要
的就是strategy正确.只是我去interview早,所以prematch给的也早,后面肯定也会有不

去年我认识一个CMG，91级的，她当时申请IM和NEURO，每个都拿到10几个IV，光IM拿到
好几个pre-match，包括我所在的pg的pre-match。（我这个pg不算CMG friendly，我之
前的CMG是96年毕业的，后来就再也没有CMG。她最后去了另外一个pg，在community pg
算得上很好的。）她也是各个方面都很牛，paper, 毕业的学校，还要LOR。

Completely depend on your career interests. If you don't have a big heart
for research after residency, take it. For practicing physician, wherever
you are trained won't matter a lot after all. Usually, for IM and primary
care, small programs are busy but easy to go thru, and university program is
hard and possible to be kicked out. For pathology, it is opposite, you
should try your best to get into university program if you can.
If you would like to pursue research (colloborated clinical or translational
research), don't take it. It will kill your future opportunities. You have
to work very hard to back on the track.
Many many years ago, when life was not that hard, I rejected several pre-
match offers from different specialties and took the risk went thru the
match. I am happy i did it.
Life is made up choices, don't always take the easy ones, some time have to
stand up and take the risk
Another option is to ask them give you an extension. You have to very
specifically and very candidly tell them that you have to visit certain
programs and get back to them at a certain date. 90% of time, they will
grant it. Ask a blanket extension will make people unhappy, they might
withdraw the offer without telling you.
Never never sign the offer and keep interviewing, the medical society is not
that big, if you are not lucky and got caught, you medical career is pretty
much over. |
