patient care responsibilities?有些虽然实际让你动手,但不合项目规定的话,推
荐信上也不敢写吧。

First, it is not easy to find hands-on USCE because of liability issues and
your malpractice coverage issues. Probably the best way is to do it while
you are in school so that there is a joint externship program that you can
do. Once you are outside the school then it may require that you pay
yourself for malpractice or to buy your externship experience.
I have no prior USCE prior to my match but that was 8 years ago. Things may
change right now so that all the applicants need to have a good USCE to
match a better hospital.
However, here is the thing. If you could not find a USCE in the end of the
day, you can always match a small prelim program and then use that to re-
match again for what you want. It is only one year. Also, if your goal is
going into fellowship, you can always match a better fellowship later on
once you work hard enough during your residency. I can tell you cases that I
know that people matched initially at a small hospital with no research
experience and USCE at all but matched top 5 fellowship program later and
then working at top 3 academic hospital (if you want top programs).
There are always roads there but you have to work hard to get what you want.
And then you will prove that saying: IMG will shine at last.
