If I am Kaiser, i want to cut the pension costs.
pension = final salary X number of working years.
Kaiser will keep final salary low for doctors; meanwhile, keeping salary low
will force senior doctors to private practices. So Kaiser will try to
employ junior doctors and residents with relatively low salaries.
For the complicated surgeries such as heart, lung and brains, Kaiser will
Contract with outside surgeons and pay contract fees. Outside surgeons do
not receive pensions anyway. In this way, Kaiser can maintain regular
employees of relatively young doctors and pay contractors' fees to outside
top surgeons. This will minimize the total pension.

it can be up or down based on productivity.
for example, a physician starts with a fixed salary of 170K, if he can
produce more bills, he can increase his salary. if he can not produce enough
bills, his salary will decrease.

My observation is male doc with stay home wife tends to choose private.
Female doc or double- doc families tends to stay in big org like US health,
PAMF, Kaiser etc for the life style and benefit.
