发帖数: 249 | 1 Residency training can be demanding, stressful or even "crazy", I hope
following suggestions can be helpful for current psychiatry residents.
1. PRITE: Psychiatry residents must take national PRITE tests yearly.
Usually the program coordinator will have the previous PRITE test questions
copied on a CD，then give each resident a "PRITE CD". Please do study the
previous PRITE questions, because there are key psychiatry questions will be
tested on PRITE every year. I am quite lucky because my PhD major is
Neuroscience and there are some neurology questions included in the PRITE,
and my PRITE scores had been good。When I was a PGY3 resident, an American
PGY3 resident and I beat our residency program most PGY4 residents on PRITE,
so we were selected together with a PGY4 resident and competed in the
National Mind Game for our residency program. To my surprise, this
experience listed in my CV had helped my fellowship application a lot。
2. On call: Before I go on call, I usually will prepare some template notes
such as depression with suicidal ideation/attempt/gesture, alcohol/substance
intoxication/withdrawal, psychosis, homicidal ideation/violent outbursts/
aggression, code green/restrain note...etc, then I will store these template
notes in my smart phrases。Whenever I admitted a new patient, I just added
the new patient's information to my template note. In this way, I had saved
a lot of time. If I went to ER to see a Psychiatry consult patient, I often
found a free desk in the ER first, then I put some Psychiatry common forms
such as 72-hour hold, release of information and admission certificate in a
drawer of this desk. So when I finished my Psychiatry consult for an ER
patient, I could get all my consult paper work done at the ER.
3. Inpatient team resident: Residency training 是一个等级森严的 training
system. 如果你对你attending 对某个病人的处置方案有异议，先保留你的意见并且坚
决执行你attending的方案, 然后私下里你可以跟你的attending 讨论你的疑问。有一
次我的attending 在ER收了一个新病人，very manic with grandiosity. 说他有
millions of dollars and everybody loves him. 我发现这个病人穿着一件灰色的T
shirt 上面写着Army， 我就怀疑这个病人可能是VA 的病人。但是我的attending 已给
告诉ER physicians 我们会把这个病人admit to our team. So I admitted this
patient to our team. 但是我做完这个病人的admission paper work 以后，我给VA
hospital 打了个电话，提供了这个病人的姓名和SSN, 结果发现这个病人果然是an
established patient at the VA hospital. 所以我给我的attending 打电话说，这个
病人可以transfer to VA hospital, 我建议我的attending 在ER waiting for a
psychiatry bed 的病人里再admit 一个新病人到我们team. 虽然我多做了一个新病人
的admission work, 但是后来我的这个attending 一直对我很好并且在我intern year
结束的时侯，这个attending 建议我们Program Director给了我一个 best work ethic
award.
4. Research/publication: Our residency program requires each resident must
complete a research project or have a publication before graduation. If you
see an interesting case, you may write up a brief case report. A published
case report is counted as publication. My second case report got accepted
for publication 3 days after I submitted on line, so to publish a case
report 是一个短平快的方法 to meet the research/publication requirement.
5. 保护好自己: 现在很多residency programs 都prefer to take fresh graduates,
why? 我觉得是因为人老了，精力和体力都不如年轻人了。我们residency program 给
所有的residents meal money, 每天$10. 月底用不完meal money 就做费。开始我常常
每个月到月底的时候连$50都没花完，只好在月底用卡上剩下的meal money 在医院的
cafeteria 买一大堆瓶装水。后来我开始在自己的手机上设个alarm, 一到lunch time,
这个alarm 会提醒我自己该去吃午饭了。还有after a 30-hour overnight call, 我
发现我的记忆力基本就没了。有一次post call, 我在我们医院的停车场里转了好几圈
，都没找到我的车。气得我心里直骂这是哪个不开眼的偷车贼，放着停年场里那么多主
治医生的奔驰，宝马什么的好车不偷，非要偷我浑身掉漆的破车。等我拿出手机想call
security officer 的时侯，才终于记起来前一天来医院on call 的时候，是个星期六
，是我LG开车送我到医院上的班。所以，以后如果是我LG开车送我去 on call，我会在
手机里给自己发个reminder message :) | s*******w

universal rule to go by anywhere anyplace.

我觉得是因为人老了，精力和体力都不如年轻人了。
还有after a 30-hour overnight call, 我发现我的记忆力基本就没了。

发帖数: 249 | 11 所以一定要按时吃饭睡觉，好好锻炼身体，千万不能在做residency training 的时候
把身体累垮了。否则可能会因为 take a medical leave 而延期毕业or even lose the
resident position due to medical reasons.
