左侧乳腺非特殊性浸润性癌（II级），部分为导管内癌伴钙化
乳头，底部及四周切缘未见癌浸润：腋下检出淋巴结（2/12）见癌转移。
免疫组化结果：ER（+>75%),PR(+>75%),HER-2(1+), Ki-67(+<25%), p63(-), CK5/6(-)
,CD34(血管+）， D2-40（淋巴管+）， alpha-catenin(-)
人在国内，问我美国有什么药可以治疗

病人是乳腺全切吗? anyway pt need radiation to the axilla, may need radiation
to the breast if lumpectomy
如果Her 2 negative, then doxorubicin /cyclophosphamide q2 weeks x 4 followed
with
paclitaxel q 2 w x4; then radiation; then anti estrogen pill daily for 5
years (postmenopause)or 10 years for premenopause
if her2 POS, mastectomy, similarly doxorubicin plus cyclophasphamide q3
weeks x 4 followed by paclitaxcel plus transtuzumab weekly for 4 weeks and
continue transtuzumab weekly for total 1 year. radiation needed as well
after paclitaxel and anti estrogen start after radiation. |
