policy.
My insurance: in-network insurance pays 70% after you meet your deductible.
hospital based doctors such as radiologist, anethesiologist ( they may be
out of network) agree to charge 70% to insurance and 30 % to patients
because you have no choice for these doctors.
If your insurance insists you need to pay out of network fees, file a claim
to explain that you have no choice. I had a car accident and used ambulance.
Insurance said the ambulance I used was out of network. I said I had no
choice and I had to use any ambulance that 911 dispatched. Then insurance
covered the whole bill of ambulance.
