继christmas在ER待了一天之后，娃又住了三天院，吊水的过程惨不忍睹。
换了两茬人，千辛万苦在ER埋好了iv，当时我就觉得埋得太浅，但是那护士说没关系的
。果然，前天晚上准备再上抗生素的时候，发现iv掉出来了，肿了好大一个包，不知道
什么时候掉出来的，外面纱布包得很牢。
然后重新再上iv，住院部能上的护士全上了，最后连nicu的护士都叫来了，小手小脚所
有的血管都试了，插进去，在里面搅一阵，偶尔摸到血管，然后又滑掉了。就没一个成
功的。我实在忍不住，去跟医生说，能不能不吊了。
娃实在太太太可怜了，现在不光她有心理阴影，我也有了。

我们家小伙子在月子里的时候因为感染住了一段时间的院，ＩＶ注射抗生素，六个小时
一次循环。小伙子的手几乎每天都要被重新扎一次，那么小的小孩，手上脚上全天绑着
板子和层层胶布固定，结果还是经常滑出来。到第二周的时候，四肢都被扎满了针眼，
一半以上都是他们试针试出来的，经常找不到地方下针。每天打针都是一种煎熬，对小
娃，对我，对护士。我赶跑了两个号称他们儿科最好的护士，其中一个连扎四针都没找
到血管，第二天又被医生叫过来打针，看到我一个劲儿的说ｓｏｒｒｙ，结果还是没扎
对。当时我在月子里，激素正高，当时发飚就不让他们再扎了，当天换了肌肉注射。后
来他们从ＮＩＣＵ找了最好的护士，是个年轻男护士，一把就扎好了，总算把最后几天
熬过去了。
真希望所有的小娃都平平安安的，以后都不用受这种罪。

唉，吊个针，我和老公又吵了一架，他又怪我小题大做，不心疼娃，就不应该送医院。
每次她一发烧，我压力就忒大，说送医院就吵架，不送医院有个万一怎么办。

Cant believe so many would blame the nurses. Well, nobody takes it lightly
to place IV in a baby, it's not only small, the intense pressure from
parents as well as a non cooperating patient who is usually dehydrated make
this one of the hardest job in the hospital. Cursing don't do u or your baby
any good. I know it because I'm the one they page if the IV team can't get
it. Best thing u can do is to look the other way, or simply stay out of the
room. Trust the nurse to do the best they can.

不要找年纪大的，要找年轻人。美国以前医院都有iv team的，专门做这些，护士都不
用做；现在医院为了减少开支，iv team基本裁掉了，很多老护士几十年没有扎针了，
现在年纪大了，眼睛花了，手也抖，自己都怕得要死，根本干不了。
倒是最近几年毕业的年轻人，一毕业就自己干，越干越熟练，这活儿没别的，就是唯手
熟尔。
亚洲人确实普遍比白人黑人干活细致，手巧。

你说的这些当然是压力，做很多工作都需要承受压力的，作为ped的护士，给不配合的
小娃打针，就是她们的工作，需要面对一个会stgruggle的还不懂事的小孩子，需要面
对紧张万分的父母。做ped的nurse，当然需要well trained，当然需要更加
sophisticated的技术，这个就是我们期望的。如果她们做不到，如果她们没有办法在
压力下工作，她们可以选择不做ped的nurse。
很多工作都要面对压力的，难道因为有压力发挥不好就把压力的因素都去掉吗？
对不起，我们不可能在外面等着，让我的孩子被一群陌生人按在病床上练手。我们不能
做任何事，但是至少她哭的时候能看到爸爸妈妈，能被妈妈抱在怀里。

My duty is to the patient. There is nothing sophisticate about IV. The more
you do the better you get. On any particular day, the calmer you are the
steadier your hand will be. Putting pressure on the nurse don't do a
freaking thing. But most of them seen thing enough and don't hold grudge.
However, a smart parent shows their care ot only to their child but also to
people who take care of them.
Child more tends to cry when they see parent. Again, believe me, we all
want to take the best care of the patient with least hurt. Nobody want to 练

士2分钟就搞定了，我说这区别咋这么大啊，还问人家呢，人家不冷不热地说，这是技
术活啊，我让老公记住这俩护士的脸，不是好护士，我坚决不给宝贝们抽血了，天壤之
别啊
不是说国内不好，不过我碰到过不好的，大学那会，不是校医院，外面的大医院，半夜
去输液，一个看起来很有经验的老护士，扎了7针啊，愣是没扎好，俩手臂，俩手背都
扎青了，惨不忍睹，后来实在没辙，叫了一个看起来傻乎乎的小护士，居然一下就好了
，太惨了，俩手全青了。。。我国内屁股肌肉注射经常出血，有时候还会顺着拔出来的
枕头喷血，吓死我了，也不知道我的问题还是护士的问题，有阵子，我看到喷的血胸口
就发闷，不知道是不是晕血了
在美国，我也碰到一个傻MD，给我抽了若干针都抽不出来，一般抽血的护士除了1，2个
不行，别的都说的比较容易。。。我的糖尿病医生，专科医生，估摸着60多了，白发苍
苍，硬要给我抽血，说他的护士走了，我吓死了，告诉他，太难就别硬来啊，结果人家
一针见血，牛人就是牛人。。。

俺家领导做护士，在内窥镜室procedural room的时候，不要打IV line的。不在
procedural room的时候，过去一年大概总共打了300个IV line,病人肥瘦黑白都有，个
个都是dehydrated, 基本上每天打1-2个IV line,听她说，也就9成的成功率。胖子和黑
人的IV就是要难打一些。

还提过有发热的症状，而且好像病程反反复复很长时间了。cyclic neutropenia是有
genetic mutation的，ELA2，查出来如果可以确诊也就不用老去ER老抽血，应该是
workup的一部分吧。

First, in this system, US, people put respect and dignity in front of

drawn for some reason, rather than go ask for someone I know who is good, I'

best effort. On the oth hand, someone who is obnoxious, careless or arrogant
will be removed, even though they may be good at skill, they will never be
a top notch caregiver, and do more damage to the system than good in the end
. Again, pain from IV placement is really minimal. Is more because of the
nervousness and anxiety that greatly enlarge the perception of pain. However
feeling of belittled or be ignored hurts and sticks.
Second, anybody can whine and bitch, which is easy, especially online. It's
harder to understand and care. Be a parent, is more important to be able to
think for others, especially those taking care of your love ones. They are
not slaves just because you pay. They are humans and deserve to be treated
like one. Bitching and cursing DONT DO ANY GOOD. And you are not your child'
s boss or owner. Parents are the GUARDIANS, who should think for their
benefits. Cursing only make a situation which was stressful worse. Grow up
and control yourself. Behave like a parent.
Third, skill wise, I do agree Asians is some what more teach-able in term of
skill learning. But not a signicant degree if you standardize background
and experience. Most don't complaint for their experience in China is
because Nobody even listen to your complaint.

It's exceedingly difficult to place IV in kids. Not uncommon even for the

The stress is very high already. Even for people who is very experienced
like me. And nobody enjoy doing that or taking it lightly. As a parent, be a
little more understanding and do add more stress into it. Because at times
its better to just stay away, rather than bitching and cursing. you may

As to the notion of Chinese nurse being better, from my point of view, they
are not in general. But they are much worse in other repect, simply they

think IV issue wothing mentioning. no one there to listen to your bitching
even if you do, sometime for much more serious issues.

护士这边的我可以回答，很多新毕业生，技术需要慢慢磨练。而且学校教育不以技术为
重，侧重职业道德，医药知识，法律常识，因为这是护士考证的侧重点。考不到证不能

varies greatly among people, anxiety and stress from other disease enhance
the perception of pain. a calm and sincere caregiver alleviate the feeling
of discomfort is well known fact. if you put all the pain from 0-10, what
most commonly known as pain scale, 0 being no pain, 10 being most
excruciating pain, most people rank iv pain between 1-3. when not under
stress. so in that sense i think you are the one falls out of ordinary.

Hospitals don't have the motivation/mechanism to improve. matter of fact,
regarding China's health system, there isn't much an objective criteria to
evaluate - nothing you see is really trust worthy - just like the GDP number
you read on paper. the person you mentioned have to go through surgeries
several times - unfortunately it happens. Nobody can guarantee it won't
happen. The patient can request see another doctor, that requires you find
the doctor you want to see. He agree to take over the case, and proper
transfer of care to ensure the patient do fall through the crack during
transfer - such as schedule of antibiotics, pertinent medicines, make
available all your records and studies. and a conversation between your
current doctor and the one who is taking over. Your doctor didn't refer the
patient to another doctor most likely because he is comfortable taking care
of it. not because he want to make money from you. If say, during a C/S, the
ureter was cut, a urologist will be consult emergently, he/she will be on
the table before the patient wake up.
Your 2nd baby, that has not been fed for 4 hour (poor feeding, you said
because the mother has no milk, but honestly there could be gazillion
reasons) plus low body temperature. A first year medical student would know
the number one differential diagnosis is neonate sepsis - which start
insidiously and develops quickly, often lead to death or permanent brain
damage. It's a true emergency. I agree they should discuss with you, but a
parent's consent is not required to start antibiotics. Let's say you refuse
treatment because ignorance or some strange believe, and the doctor feel
strongly treatment is need, the hospital can ask for court order to rule
over your objection. It comes back to what i mentioned earlier, parents don'
t own their kid, but rather guardians of their kid. If you fail your duty as
a proper guardian, you could loose you kid.
And when it comes to saying "i'm sorry" or "excuse me" - there is actually
heavy legal issues behind it. if you interested, you can google physicians
saying sorry, pros and cons. nurses, including mid level practitioners are
under strict directives of the doctor by law. They are not responsible for
the medical decision. and they don't have to apologize if they had follow
the orders - for IV, no they shouldn't apologize for multiple attempts. They
would say "i'm sorry for your pain" rather than "i'm sorry i can't put the

IV真是一个很小的环节。都没有病儿家属向我们complain过。蒙古大夫有一点说的对，
IV line的pain其实根本不算什么和俺们在病人房间给病人做腰穿，骨穿，动脉血气分
析，中央静脉，这些procedure相比。俺老婆要是扎静脉被扎哭了，俺真的是要骂她太
不中用了。这也值得哭。俺当年头上手上脸上血huhu的被缝了好多针，俺没让打麻药，

我能理解，而且我是成年人，这点痛也不算什么，我绝对不会为了这个特地上网抱怨。
但是对于我才一岁的孩子，我希望她的痛苦能被降到最低，如果我能替她承受这些，不
要说多扎几针，就算在我鼻子上吊水都可以。
至于父母在场的问题，当时没有一个护士要求我们父母stay away，我以为这是一个很
小的procedure，我没觉得这是一件需要父母回避的事情。如果做手术什么的，当然，
我们肯定不会要求在场。

说老实话，美国的静脉针就象个钢笔一样，又长又直来直去，非常不好使用:-(
俺有个病人，唐氏综合症，polycythemia, 每次隔一段时间定期来放血。人好胖，血管
细的象发丝一样。连放血带的针头好粗啊，比18号还粗。过去三年她在我手上两次，每
次都要try 两次才成功。俺们每次都拿糖果先哄她。
还有黑人sickle cell crisis的，dehydrated,need IV hydration and IV pain
medication,他们疼的在那里直哆嗦，俺在那里扎脖子，好紧张啊。还好，俺都survive
过来了。
还有一次，code blue,病人没有line啊，一个住院医管气管插管，两个轮流做CPR,俺当
时第一年培训，在那里病人大腿根部做central line。CPR一做，病人身体一动一动的
，这个病人还有HIV,HCV,HBV.算我运气好，line做出来了，也没扎到自己的手。
还好这些都成了过去，再也不用做这些杂活了。

compare to the potential risk of undiagnosed sepsis. I expect you to at
least have the brain power to understand that. Educated yourself and be a
good parent is what you need to do.
