一定得拔掉，而且考虑到她年纪这么小，只有全麻。实在不想为一颗牙齿全麻，不知道
会对孩子有什么影响。不知道吸笑气是不是也算全麻？很不理解这里的医生就为了怕孩
子对看牙医有恐惧就不肯给她局部麻醉。

kids will not have bad memory of dental work.for one simple extraction, if
your child was cooperative, it is not necessary to use general anesthesia.
The local anesthesia and nitrous gas will be fine.The extraction will be
less than 1 minute. If your child is not cooperative, IV sedation will be
another option. It is similar to general anesthesia but much simple
procedure, the child will sleep immediately after injection, and will wake
up immediately after remove the medication, and will not remember what
dentist did to her. |

My 3-year old fell and he "probably" will lose his teeth. But his dentist
gave us two options: extraction or crowning.
To my understanding, putting a crown doesn't need extraction. My question to
you is: did the dentist suggest you for crowning? Extraciton only may cause
problem for your kid's adult teeth coming out at the right time and right
position. I would choose crowning if we found out his teeth are dead in late
summer.
My second question is: how long did the infection happen since the accident.

小朋友马上3岁。
3月底时候摔了一跤，流了很多血，过了三天去做了x-ray,看起来没有问题，说4个星期
后复查。
4月初在daycare被小朋友狠狠地撞了一下，当时那颗牙齿又流了不少血。过了俩星期4
月18日再去医院复查，结果说有感染，建议拔掉。
因为孩子没有抱怨，牙齿也没有变黑迹象，我们也一直坚持没去拔牙。终于在前两天，
也就是离上次检查一个多月以后，发现她的牙龈上长了一个小肉包。去看医生，没有再
x-ray，肉眼看了就说是abscess,也就是脓肿，考虑小孩不太配合，需要全麻。
医生说牙根的骨头都消失了，所以根管术也没用，更没提装牙冠的事情，只说拔掉是唯
一选择，还约好了外科医生。
我下周打算另外约个医生看看。的确有很多concern。虽然我们现在这个牙科儿医的声
誉很不错。 |

发帖数: 13617 | 13 不是的。是因为牙冠折断牙髓外露，进一步引起牙髓炎，根尖周炎，根尖脓肿等，单纯
服用抗生素没有用。儿童牙外伤后，有时候即使牙冠没有折裂，但是因为根尖周软组织
和血管挫伤，也可以引起牙坏死和根内吸收而致牙齿不保。
3岁的小朋友牙齿的根尖孔是非常大的，根管治疗不是很好做，但是拔牙后因为是大门
牙，形态和距离都很重要，可能需要做间隙保持器，种植牙的话大多建议青春期后再作
。。脱离这行太久了，不知道现在的观念是不是一样，请多和医生探讨。。

depending on the damage on the teeth, if there are fracture or loss of tooth
structure due to trauma, crown (SSC) is necessary to save the teeth.

是，
1.牙齿拔掉以后有什么办法保持牙距呢？是否可以装个假牙啥的？
如果空着这颗门牙会导致日后长出来不整齐么？下面对应的那颗门牙因为没有咬合也会
长不好？孩子说话是否会受影响呢？
2.我家孩子的情况估计很难配合进行局部麻醉。那如果需要孩子睡着进行处理的话，哪
种麻醉方式影响会最小呢？这种手术大概要耗时多久呢？

假牙usually not necessary for 门牙. If it is baby molar teeth extracted, the
space maintainer is needed to prevent the movement of posterior teeth.
In your case, I guess it is IV sedation. child needs oral sedation (drink
some medication first, and then use IV sedation). the IV sedation is very
fast, the child will sleep in a minute. The extraction of baby tooth usually
take a few minutes. your
dentist seems very reasonable in treatment and very experience. Just follow
your dentist.

it is hard to tell. the Crown (SSC) is used to save the decayed or cracked
teeth without infection.
2)if infection happened with crown, what will the dentist do?
extraction.
3)in the case he needs extraction, is it possible to do it under local
sedation if the kid cooperates well?
depending on dentist and your child. It is possible to use local anesthesia
with nitrous gas.
4) how to prevent infection? what should I do?
daily oral hygiene, regular dental check.
I think your dentist is conservative doctor since he gives 3 month monitor
time period.Just discuses your concerns with your doctor.
as dentists, we do understand parents' concerns. Our goal is to save young

lighter"(look from front, tongue side is still dark, but I didn't look
tongue side before). I wasn't sure about it. Last night, I asked my hubby to
check and he agreed with me.
Do you think it's a sign of getting better?
2) Also, I feel you think "conservative" in this case is bad, true or false?
That day, I think he noticed my uncertainty about what to do when he gave
me two options. And then about several seconds later, he said most partents
would choose wait and see. So he asked us come back in 3mo. |

很谨慎。 因为对牙医来说，拔牙是一种最简单的治疗方式。但对病人来说， 拔牙是一
种不可逆的， 也就是拔一颗， 少一颗， 特别是对小孩子， 可能会影响到以后的恒牙
的整齐，还有说话等等。 所以谨慎的牙医是能不拔，就要给病人保留。而且“he said
most partents would choose wait and see.”说明他非常了解家长的心理，而且想
看一下这个牙是否可以保住。 所以说他是一个很好的医生。如果你有问题，顾虑 可以
和他讨论。绝大部分的医生都是有职业道德的。
