有任何地方提到pharmacist會如何被sue.
我就覺得疑惑,難道發錯葯就會那麽輕鬆過關,給病人換一下就可以了? 我上次看到
本州一些藥劑師的紀錄上都會notes, 比如有個藥劑師把葯發給了同名同姓的另一個病
人,其實這種情況真的挺怨的, 當然藥劑師應該問一下病人拿的是啥葯,但是我平時
自己去藥店取葯時,經常踫到藥劑師只是問我有沒有問題的, 我回答沒,就走了。另
外還有個Pharamcy manager gets notes on her credential because the computer
system allows the pharmacy tech to skip the warning on the dosage, therefore

you're still covered.
I don't really know how this insurance thing works and what actually
happends if you make a mistake and get sued.
Your question is a good question for pharmacists on this board who're
actively practicing to answer.

在out-patient pharmacy, 药师还应该问3 prime questions.
1. What did your doctor tell you the medicine is for?
2. How did your doctor tell you to take this medicine?
3. What did your doctor tell you expect?
如果出了错误,要向病人道歉,并解释清楚,尽量避免纠纷扩大化.只要让病人满意,不去
Board of Pharmacy complain,就没什么问题.
如果有complain,Board会依据法律采取措施. 各州法律不同,惩罚措施也不一样.
例如
TX--"事不过三"
NY--"有错必查"
想Sue Pharmacist需要同时满足两个条件
1. gross incompetence and/or gross negligence
2. patient harm

very repetitive, 'cuz some ppl might have been taking a certain medication
for many years.
Checking birthday is very important, another good idea for refill
prescriptions is opening the bottle and asking the patient whether the drug
looks familiar to the patient. Believe it or not, patients might not
remember the names of their medications but they remember how they look like
, and you can catch errors this way.

Using 2 patient identifiers (most likely name + birthday) is required in
hospital/institution practice.
It's a good idea to check with the patient. However, ISMP was strongly
against using color/shape to identify a medication unless you were comparing
to a picture in an active database.

prescriptions daily). We make errors quite offen, wrongly counting, wrong
med (strength). Fortunately, all errors didn't hurt patients seriously.

patient caught the mistake before taking it at home. The prescription was
entered wrong from the very beginning, one could only imagine how many
mistakes go undetected everyday.
