FPGEE.
I think I need at least 2 certificates for FPGEC: (A) 《执业药师资格证书
》; and (B) 执业药师注册证. To get (B) I need to obtain (A) first.
The following are what I found from the Cantonese Pharmacist Association
regarding pharmacist registration:

1.《执业药师首次注册申请表》一式两份；
2.《执业药师资格证书》、身份证明原件及复印件1份，受理人员对原件和复印件核对
无误后，当场将原件退还申请人；
3. 近期同一底版一寸正面免冠彩色半身证件照3张（申请表贴2张、另交1张办证）；
4. 县级（含）疾病治疗和预防控制机构出具的健康证明或医院出具的半年内有效的体
检报告；
5. 执业单位合法开业的证明（许可证）复印件1份；
6. 申请办理首次注册时间超过《执业药师资格证书》“签发日期”年度的，还需提供
《执业药师继续教育证书》或学分证明;
7. 申报材料时，申请人不是执业药师本人的，应当提交《授权委托书》（下载）1份（
须按照格式填写）；
以上材料使用A4纸打印或复印，复印件需加盖公章。
I am still not sure how to get 《执业药师资格证书》 though. Do I get it from
省人事考试局？
Earlier this week, I just submitted <<2012年度执业药师资格考试报名发证登记表

And I also do not understand item 6) from above:

《执业药师继续教育证书》或学分证明;
So if I apply for pharmacist registration 注册 as soon as I receive 《执业
药师资格证书》, do I have to take continued education / 我可不可以 避免 提
供《执业药师继续教育证书》或 学分证明?

including school transcript, translation, degree certificate, TOEFL iBT/TSE,
ECE certification, CDGDC certification, ect. I am not asking about those.
I would like to know what other professional certificate is needed from

找亲戚拿着考试人自己的县级及县级以上医院的体检表，和相关的表格（看本地sda的
要求）去申请《注册证书》，等2周或更多周让亲戚去领
有人是拿着注册证书到省药监局办理fpgee要求的－－复印，签字，盖章／／再将之复
印一份用于翻译，要办事人员将做好的那张纸装在药监局的信封中，密封，封口签字，
盖章即可
我但是不知道，资格证－－省人力资源xxx厅
注册证－－省药监局
分别办理了

timeline usually is same every year. I got mine from henan, the notice was
released from 河南职称网, different website from省人事考试局for the exam reg
and result release.

$$$ It should be said in the notice. Usually a form (attached in the notice)
along with a一份单位人事（政工）部门开具的介绍信, the person’s ID card and
your ID card (I assume you will ask your friend/relative) to get it for you.
Will 省人事考试局 give me the 《执业药师资格证书》in a few weeks?
$$$different province has different timeline. Usually in May or later. Watch
yaoq.net or ask which website in your province will release the notice.
What is required to obtain 《执业药师资格证书》and how / where do I get it?
The procedure you posted here is about how to get the registration card from
your province’s SFDA. It’s bit of complicated. Once you get your Chinese
Pharmacist license, you have to在执业药师注册服务平台（http://zyys.sfda.gov.cn/zyysweb/index.jsp）进行网上申报, but then your friend will have to take item 1-5 to the province SFDA to get the registration card for you. It can take 2 to 4 weeks.
The whole process for me took about 7-8 weeks in this summer. Luckily this
year’s FPGEE was in November, I had plenty time to apply.

I spent a good 3 hrs at yaoq.net and got some idea. Basically, we should

《执业药师资格证书》is issued between March to July, depending on your
province. For Example, Zhejiang released certificates in May 2012, while
Guangdong did that in June 2012.
The notice to obtain《执业药师资格证书》may be published by 省人事考试局, 药
监局，or 职改办 。Again it seems to depend on your province. For Guangdong,
notice and 《执业药师资格证书》were released by 省人事考试局 in June.
westeriver and bestdone, please correct me if I put it incorrectly:
For westeriver (which province? did you get notice from 省人力资源厅):
资格证－－省人力资源xxx厅
注册证－－省药监局
For bestdone (Henan province):
Notice was released from 河南职称网, did you get 资格证 from 职称网?
注册证－－省药监局?
As for continued education / 继续教育, I read that you may be able to get
registration without CE in your first year, but you need CE for the 2nd year
and afterwards. But again, it depends on your province. I plan to take CE
every year. For Guangdong, 15 credits are required for every year (1st
registration is good for 3 yrs).

through a few web sites but did not find what exact tests are required.
Would you please tell me where I can find such information?
I imagine vaccination records, tests for infectious diseases (e.g., chest x
ray to detect TB or ELISA for hepatitis) and liver function (to indicate
infection with hepatitis A, B and C) would be required?
