information on the money order, like name, case number, country, DOB, city
of birth, SSN, see DOS instruction.
2.护照相关信息是要第一页有名字和出生日期的, if your passport extended once,
you need this extension page.
3.case number and country of last residence is written on any
documentation submitted, you need wirte in cover letter, if not printed,
you need to add.
ds3035, ds2019, supplementary information, statement of reason, you don't
need to add, it already print out clearly on 10 page PDF file |
