３． 2019form的信息里，purpose of the form. 我有一张表上写着：Other.
reprint for travel and shipping. （学校给我印了张一样的，签上字，寄给我让我
从境外回来时用。）
给出的选项中没有Other. 可以就选replace/Amend吗？

所以应该写你为什么应该被waive, 而不是为什么需要waiver。
