you need to download the form from ins web, check this url http://www.immigration.gov/graphics/formsfee/forms/index.htm
Your school admin should have copies as well.
fill it out, then go to the bank where you open account from, ask them to make
a notiry for you. It's free to make a notary from the bank if you have
checking or saving account there.
