warrants to purchase 19,047 shares, all of which are exercisable within 60
days after March 27, 2015. Also includes 33,653 shares acquired by Dr.
Selick under our 2004 Employee Stock Purchase Plan.
(17) Consists of 212,080 shares subject to options granted to Mr. Simon, all
of which are exercisable within 60 days as of March 27, 2015.
(18) Includes outstanding options to purchase 3,861,242 shares and warrants
to purchase 287,998 shares, all of which are exercisable within 60 days
after March 27, 2015. Also includes 84,464 shares acquired under our 2004
