我们按要求准备了如下：
1.visa application form
2.recent passport-style photo
3.passport
4.return-trip booking or ticket （从agent email打印的confirmation)
5.proof of available lodging in Italy (hotel email confirmation打印）
6.proof of economic means of support （bank statement)
7.documentation of socio-professional standing (我用的offer letter和paycheck)
8.health insurance card (我还另外让保险公司fax了letter证明我的保险cover国外）
DC这个领馆是周一三五的上午10点到12点半办visa，早上开车去的，路上趴车不好找，
一直开到山上路尽头
