1。 需要自己准备回寄的信封吗？（以前好像需要，但是目前使馆主页上没有找到相关
消息）
2。 需要填5474 "Authority to release info"表吗？使馆主页说：
As of 01 November, 2010, passports and documents will not be released to
couriers or agents who do not have a completed "Authority to Release
Information" form.
