(1a) if not, 是不是必须用签证本人的护照原件, or just the passport # is fine?
(2) How soon will be the gap between reservation date and actual interview
date?
It is well defined for B1/B2 or F/J/M, but no so clear for H1b. Another
website showed a recent quote on Feb. 29 and interview on Mar. 4.
