Here are the steps:Go to: www.blockbuster.com/closing:

At the very bottom left of the screen click on the 2 tiny dots (Under terms
and conditions)
For the store #, enter "95576"
For the password, enter "bbonline"
Decide if you want 1, 2, or 3 out at a time from the mail
Fill out your info and submit! You will need to use a different email
address, mailing address (take off a letter from the street name, or add "
Apt A" and just change it back later), and CC if you have already set up a
free trial before.
Make sure to print out the confirmation page because it has the coupon for a
free in store movie rental!
You can cancel your online account at any time by logging in, clicking on "
My Account", "Cancel" subscription, and "No , please cancel my subscription".
Other things to know about Blockbuster Online:You can get DVD's, Blu-Rays,
and Games in the mail as part of the subscription
To request Blu-Rays whenever available, you can set the preference under the
"My Account" tab
You can take the items they send you in the mail back to a store and
exchange them for a free movie rental in store (DVD or Blu-Ray). You get 5
of these included every month.
The movies from the store that you get for free do NOT have a due date, they
just won't send your next online title until you return the store movie
back to the store
If you want to exchange the item from the mail for an in store game, it
discounts the game rental to only $5 and does NOT count against your 5
exchanges, but it will have a due date
You get 5 free in store exchanges each month; if you want to exchange more
then they discount the movie rentals to $1.99 (and these do have a due date)
They do not have a free streaming service
