History: 67 y/o patient with frequent urination since 3 months ago.
CT KUB was ordered.
CT KUB Findings:
1. Left kidney 5cm upper pole mass: most concerning for primary renal cell
carcinoma.
2. Bladder wall thickening with mild bilateral hydronephrosis and
hydroureter, concerning for chronic inflammation of the bladder wall.
Neoplasm such as TCC is not excluded.

further characterize the adrenal lesion as well as hepatic lesions. Rads are
very comfortable calling TCCs and RCCs on CT. For adrenal adenomas, you
need three phase CTs or MR. Also for the liver lesion, CT liver protocol or
MR are better to tell most of the lesions.

follow ups on their own protocol if they want, especially if it is functional.
4. As I said the purpose of the MR is to further delineating the adrenal
lesion and hepatic lesion. Furthermore, it will provide us more info on RCC
invasion, to adjacent Gerota fascia or renal vein and IVC etc. so is for the
bladder TCC for staging purpose.

, renal calcs, left renal mass and bladder mass. Good tech may see the left
adrenal lesion but most won't. Then you will still order a CT or MR for
further delineation. Ultrasound doesn't have radiation, but not as accurate
. However, you can see the ureteral jets to the bladder on real time to
evaluate obstruction, which u can't see with CTs

Some adrenal tumors are benign and do not need treatment, for example
functionally inactive adrenal adenomas. Some tumors, such as
pheochrsmocytomas, need medical treatment to control blood pressure before
surgery to prevent side effects of the operation. Malignant tumors and
metastasis need different appoaches in management.

今天去见了外科医生，感觉人蛮好的，挺耐心负责的
按照他的看法，觉得这个mass应该和adrenal gland相连，但是也不确定
他建议首先验血验尿，排除phoechromocytoma，如果是，他建议一定要切掉
如果不是，有三种选择
1。直接切。听他说风险还是有的，比如说有可能碰到胰脏导致出血等等，但说可能性
很小。而且根据这个mass和adrenal gland的接近程度，有可能连整个左adrenal gland
一起切掉。感觉挺吓人的
2。先做CT guided biopsy，如果确诊这个mass没有任何潜在威胁，根据自己感觉的情
况，如果没有不适感了，可以考虑定期检查，不切。如果不能确诊是什么东西，或者确
诊是有一定潜在危险的东西，还是要切的。
3。不做biopsy，只是定期检查。
他认为我的年纪和身体状况，可以考虑稍微agressive一点的方式，尤其如果有不适感
，应该尽早切除。可是我觉得如果连带左肾上腺一起切除有点太过了，不过听医生的意
思是说，一旦开刀，肯定要把这个mass切下来，而且切多少，肾上腺切不切，只能视当

(2) Spleen/Spleen hilum.
(3) Pancreas tail
(4) Stomach
(5) Upper pole of kidney
Could not arise from kidney or spleen or pancreatic tail since there is
clear fat plane separating the mass from both organs.
I could not tell for sure if the mass is arising from the fundus of stomach
or adrenal gland.
(2) Contrast enhancement and internal feature of the mass.

I look at your CTs again and found that there is pre-contrast, portal venous
, it is best to have 15 mins delay phase.
Or your can have MR(chemical shift imaging) or PET to assist diagnosis. |

位置有关。另
外，28周、29周和31周的B超都提示："no fluid in stomach"。29周AFI= 22.6，
29周B超：right adrenal gland was visualized, right kidney couldn't be
visualized within renal fossa. pelvic kidney is not suspected. left
kidney and left adrenal gland were visualized。
请问"no fluid in stomach"这是什么问题？与肾有关吗？

1)比如，我目前复习的儿科的antidose. 对于
anticholinergic的toxic sign， antidose是 Physostigmine sulfate.我期望我能够
大脑里反映的出 anticholinergic + physostigime sulfmate的生理反应机理和药理.
2)比如看congenital adrenal hyperplasia，我就会回去看看adrenal gland 的生理机制
，还有在不同步骤各种酶。这样我就会回去看看病例生理的书。
3)UW中的解释的各个选项，我也会大概的翻翻书了解一下
我这样是否太细了？我也知道越是不熟悉的科弄得越细，熟悉的科目，自己都知道那里
是重点那里掠过。所以时间安排也合理。
关于药物治疗，是否适应UW的那样要求就可以了？
大家帮我提提建议。

发帖数: 44 | 29 今天得知了step3的成绩，210+，心里的石头终于落下。终于轮到我给USMLE说再见了。
回顾从step1自己闭门造车考了只带一个9的分数， step2开始参加小组学习，做了所有
能做的题只为拿个99，虽胸有成竹且考试感觉很好，最终但又得到一个9的分数。虽然
这次成绩不高，依然很高兴，毕竟不用发愁如果没过的话住院医开始后没有时间复习该
咋办。过去的3年中得到太多人的帮助 (此处略去人名20个)，绝大多数甚至从未谋面。
今天发贴也是为了回报我们这个社区吧。
自我介绍：毕业>15年，220+/220+/1st/210+；
复习时间：6月，头4个月每天约2-3小时，周末由于陪孩子们玩（前两年欠的太多，你
懂得）及做点家务（再不做但心被老婆炒掉），反而复习不了多少；最后2个月每天4小
时，鼓励自己考了这个2年内再不用受这个罪。
复习资料：随大流，MTB 3, UW CCS, UW step3 Qbank, UW step2 Qbank;
模拟：考前2月，NBME 1 190+, 考前1周，NBME 2 220+;

章很麻烦，耽误了2个多月，材料才得以从学校寄出。个人觉得申请算是比较顺利。把
我当时准备的情况跟大家说一下也许对将要申请的人有一点帮助。
我个人从这里寄出去的材料包括：
1.国内护士执照的公正，在这里网上找的公证处，在纽约，叫最佳公正，电话
7183212810
2.指纹卡，就在我住的地方的警察局办的，free
3.handbook上的申请表，贴一张照片在右下角（handbook链接：http://www.rn.ca.gov/pdfs/applicants/exam-app.pdf）
这一页有一项是填社会安全号，我当时没有就空着的，没有关系。
4.两张支票，一张办指纹的($51)，一张申请的($75)
5.写了一封信，表示感谢，一张纸，很客气的几句话
从学校寄出的材料：
1.中英文成绩单（学校有，我来之前到学校教务处办好的）
2.学位证书和毕业证书中英文版（来之前自己办好到学校教务处办好的，盖好了章）

TB is not the top one on my differential list. I have not seen many TB
patient, but I don't think eosinophilia is a common manifestation of TB.
The only situation I can think of is eosinophilia 2/2 adrenal insufficiency
2/2 TB adrenitis. Then the patient would be much sicker. Maybe there are
other reasons for TB and eosinophilia I don't know, but I am open to a
discussion.
That's why I recommend the patient to go to a better hospital. |

Differentials based on limited CT images are broad: GIST; prior granuloma
disease such as TB or sarcoidosis; pheo especially your symtoms fit well
with it and sounds
like you also have some family history which makes it fit even better,
however pheo usually enhance well with contrast while yours do not enhance
much with contrast, although necrotized pheo may not enhance; adrenal

几周前看了外科医生，医生说在做biopsy之前，需要验尿验血，尿是24小时的。
今天受到了结果，应该不是pheochromocytoma因为metanephrine没有超
在检验的项目中，glucose和cortisol超标，尤其是cortisol，超标了3倍。。。
外科医生很忙，我想听他的建议可是他一直没有空。朋友说这个事情应该ask for 2nd
and 3rd opinion，所以今天又约了另外一个医生。
自己也在网上做了一些阅读，感觉根据验血验尿的结果，应该要么是adrenal cortical
adenoma（肾上腺皮质腺瘤）要么是adrenal cortical carcinoma （肾上腺皮质癌）
。到底是啥也不知道了。看到网上说检验尿17酮能区分良性恶性，真的可以吗？网上还
说皮质腺瘤比较均匀，而皮质癌不均匀，我这个不均匀。。。皮质腺瘤少钙化，我这个
明显有钙化。。。感觉越来越像皮质癌了。因为之前做了PET scan这个东西没有亮，所
以一直以来觉得没啥事，可是现在又开始紧张了。。。

据香港明报报道，伊利沙伯医院化学病理顾问医生石志忠直言，儿童先天发育异常“并非不常见”，外国数据显示，每300个男婴，便有一人尿道下裂如女性。“先天性肾上腺增生”(Congenital Adrenal Hyperplasia，CAH)会导致“类固醇21-羟化酶缺乏”，更是引致女婴出现类似男性性器的最常见原因，但香港一直未有相关个案的统计数据。据美国当局统计数据显示，每1.6万名新生婴，便有一名患CAH。
身兼亚太小儿泌尿科学会会长的小儿外科专科医生杨重光，早在93、94年中大医学院已成立“综合性征模糊诊所”，集中小儿分泌学、遗传学、儿科、发育科和小儿外科医生，接收香港新生婴儿的性征异常的转介个案。多年来个案并不太多，每年只有10宗或以内，近年该中心已解散。

1995年就回国了，那个时候国内的环境有多差不用说了吧。这也快20年了。比饶益
2007
年才全职回去多了12年。比起培养的中国学生也多多了，算国内贡献肯定不会小。如果

, the only possibility of curing this cancer is early surgery. only 200
cases /year in US. this medication is for patients whose disease can not be
cured by surgery. its toxic, not very effective. |
