但是我还有几个问题（1）dox由于其细胞活性不够，对癌细胞的杀灭不是很有效，再加
上抗药性，dox已经不常用了；（2）由于albumin的最活泼的cys只有cys34，但albumin
又是最常见的一般药物的载体（非共价键结合），所以这个Aldoxorubicin有多少比例
结合到albumin上（共价+非共价），还需要测定；（3）文章中似乎暗示药物是在tumor
tissue释放的，但据我所知，albumin 在 蛋白gp60 的帮助下，是可以进入到癌细胞
内部的，而癌细胞内的pH最低可以到3，一般是4-5，所以在癌细胞内，这个albumin-
emch-dox更容易释放。tumor tisse的酸度，我记不清了，但pH一定是高于细胞内的。
药物到底在哪里释放的，还需要进一步确证；（4）其它带有cys的蛋白（比如cys,gsh
）的影响也要检测。
我觉得最大的问题是 Aldoxorubicin 是和 dox 比较的 (phase IIb)，结果还是另人振

large volume paracentesis and as a volume expander in shock situation,
albumin get metabolized so quickly that its effect is very short-lived, plus
it's very expensive,
if the patient's albumin level is low, most of the time is from poor
nutrition and/or liver disease with poor synthetic function, infuse albumin
will not help, |

我还有几个问题，请内行人赐教／指正。
dox在已不常用，是指它被其它蒽环类药物取代了吗？还是说有被其他作用机制的药物
取代的趋势？取代品的优势是因为效能更强，还是不容易抗药，或是副作用轻？我感觉
蒽环类化合物在血液病，淋巴瘤和实体瘤上都有一些应用。
此外 和脂质体阿霉素相比，这种albumin-EMCH的优势可能体现在哪儿呢？
是的我也看到了Abraxane，它是taxol非共价结合albumin再内吞到细胞内。

expansion 列出三个选项让医生勾选：hespan 500ml x ___ for ___; albumin 5% in
500 ml ns x ____ for ____; transfuse ____ units lprbc's for hct <24% hgb < 8
.0. 我只见过他们选albumin 和prbc. 下次可以问问他们为什么从不用pentaspan/

己pack一个可以装在HPLC的柱子，用来分离血浆里面的albumin;还交代在pack之前，先

很好的留在淋巴结里，主要是macrophage的uptake，初步试验似乎是albumin和这些
lipids高度亲和，将它们带至淋巴结，但是albumin是如何转运这些lipids到
macrophage的呢？如果是nonspecific cell membrane insertion, 似乎淋巴结里其他
的细胞（B，T，Cells）因该也会分一杯羹。我的问题是：一般说来，lipids或者是
fatty acids是如何被lymph node macrophage 捕获的？有没有cell-surface receptor

acid (TFA)的溶解能力特别强，很好用。可是，高分子（白色粉末）溶解于新鲜的TFA
之后溶液由无色透明慢慢变为淡黄色透明。
是不是因为TFA是强酸有所谓的发烟现象？浓盐酸就是淡黄色，因为含有少量杂质Cl2
。另外， 含卤素或含硝基的化合物容易变颜色,是少量变质导致的吗？

点。所以H+减少，and/or Ca2+增加会导致血管收缩。
但是BRS physiology 4th edition Page 185提到：H+ 和Ca2+ 竞争albumin结合位点.
所以呼碱时，H+减少，更多的free calcium将会与albumin结合，血浆free Calcium浓
度降低，导致tingling, numbness和muscle spasm.
是不是说free Calcium对骨骼肌和平滑肌的作用是不同的？ |

ferritin is also a marker for chronic inflammation. But with chronic
inflammation, you will see other abnormalities, like anemia, low albumin, high
platelet count, high ESR, CRP, and LDH. With a normal albumin and PLT, it's

2012年11月突然得病，经组织穿刺确诊为Nephrotic syndrome - Minimal change
disease
医院治疗3次，都是出院后即肿起来，严重时眼睛睁不开，一回到医院挂albumin，
prednisone,bumex就很快好转。24小时蛋白18000（不清楚单位，是单位含量还是总量
？下同）。后经控制饮水，盐摄入，口服prednisone 60mg每天，配合利尿剂和其他平
衡药物，2013年一月底明显好转，24小时蛋白300。医生将强的松的量由60降为40。过
一个月后再次浮肿，24小时蛋白回升到12000，此后恢复就一直没有之前明显，经常乏
力，头晕，血压经常偏低，大概高压在80-100之间，低压40-60之间。
2013年4月因病毒导致腹泻又住院一次，激素效果同样明显，但是回家后恢复还是比较
慢。
最近又无明显诱因（可能是感冒）浮肿住院，血压91/46，心跳130，24小时尿液蛋白说
升到了84000，重复检测为83000，挂albumin已8天，每日静脉推注强的松，血压心跳已
正常，精神无明显好转。医生认为强的松已经不起作用，准备换药，给了两个... 阅读全帖 |

发肝出问题，加上他说的albumin低，除去肾综和长期营养不良，肝病最常见了。种种
迹象说明小孩进ED前，肝功能已经有问题了。指责医院FASTING 造成急性肝衰要打个折
，不过是压死骆驼的最后一根草。
本来不想说话了，今天好奇看了他网页上说的有严重嫌疑的外科病理检查部分，两个肝
脏活检时的内镜图显示肝脏外观变化很吓人，一个表面是密密麻麻的小水泡/脂肪泡，
一个看起来就像大小结节型的肝硬化，颜色灰白，根本不像正常肝脏那样的表面光滑细
腻，颜色暗红。难怪国内的“猪头”医生认为要么是病毒肝炎肝硬化要么是肝癌。七个
月大婴儿的肝外观成这样真不是短短几天的时间就能造成的，显微镜下应该主要有纤维
囊性变，这些都是慢性的病变过程（好像还有脾大，也是慢性的过程），在这样的器质
性改变的基础上，出现肝衰竭简直是偶然中的必然，早晚要出事。除非说这些内镜图也
是被换过的。
这个事，医院也许有疏忽漏诊，搞点赔偿就算了，其他指控很难。因为随便一点疑问，
有专业背景的人两下就能驳倒他。但是他不听劝，很多人看劝不动也就不说话了，因... 阅读全帖 |

及时就医至少能在肺炎早期及时介入治疗，至少能尽量避免急性肾衰竭，避免很多并发
症。
中国医疗有很多弊端，但是城市来说，在医疗普及性方面比美帝强太多。
我上面说得那个例子，如果及时治疗，至少能避免气管切开，就算气管插管也比切开好
多了。这个过程可不是一天两天。
美帝这边，中小医院夜班里没有主治医生值班，很多轮转的住院医水平不够。比如ICU
来说，很多病人低血压/肾衰对补液不敏感或是没有指征，明明需要及时上central
line上pressor上CRRT，但是很多住院医手潮没有信心，就拖，继续输液输ALBUMIN，拖
到下一班。
很多护士都心知肚明，但是大都没人吭声。医生水平高低群众眼睛是雪亮的。
我干了十几年护士就多过一次嘴，劝一个中国留学生转到芝加哥大学医院去。因为他的
病越治越重，走着进来，快要躺着出去了。
还是那句话，很多因为FLU而死的病人都是被耽误了，或者自己的原因，或者医院的原
因，更或者医疗体制的原因。 |

老年痴呆生活不能自理, 是可以 qualified for hospice.不一定要半年就死的，有些
病人如果护理得好，活得稍长些。
Criteria that may indicate a patient is appropriate for a hospice referral

A diagnosis of Alzheimer’s or advanced dementia
Unable to dress, bathe and walk without assistance
Has urinary or fecal incontinence (intermittent or constant)
Has no consistently meaningful verbal communication (stereotypical
