发帖数: 2682 | 8 各位PSU的扇子，以后我会多发些关于PSU RECRUITING的帖子，老是讨论SCANDAL容易吵
架，还是讨论FOOTBALL比较有趣。
今年PSU的JUNIOR DAY是2月23号，和去年相比，今年的规模会小很多。去年这个时候，
SANCTION还没出来，PSU的COACHING STAFF还是按照25个SCHOLARSHIP去筹划JUNIOR DAY
的，所以邀请了很多孩子，也来了很多。现在由于SANCTION，只能有15个SCHOLARSHIP
（因为12年实际用了12个，所以有3个可以转到13年，总数是18个。当然，还不能违反
总数65个PLAYER ON SCHOLARSHIP的规定），所以今年PSU RECRUITING的总体原则是要
SELECTIVE，故今年的JUNIOR DAY规模比较小。

内科第一年intern
从去年开始，traditional call system（每3、4、5天call一次）不符合现在的要求，
现在新规定出来后intern连续工作不能超过16or24小时（不确定）
自己所在program，有night float，从前天晚上（偶不确定是晚上还是早上。。。）六点到第二天早上七点半，每人每年轮一个月
不值night float的时候，就值白班
一、Intern之前的准备：
（一）Limited license：
根据所在州的决定，工作一、两年后，可以申请full license，正式去program上岗前，必须拿到limited license，最好在7月1日前， 有了这个之后，才能有以下的两个号：NPI（你的全国的编号）、DEA（你签名开药需要）
tele所在州board of medicine一个月开一次会，review一次你所交的资料
tele是七年制：毕业证书是五年的时候拿到，但在五年之前没有实习，是七年的后两年实习，类似这种情况，任何时候有discrepency，就要自己所在的医学院出具文件说明

这整件事情我的确觉得自己是被误导并歧视了。
我实在不能理解这里有些人的想法，首先一定要找出自己人身上的错误，然后拿着放大
镜放大再放大。我唯一做的不太符合标准的就是在经济舱广播说行李满架的情况下，大
家都在过道里堵着走不过去的情况下，先把行李放在必经的商务舱的行李架上然后再去
找乘务员商量，因为在狭窄的飞机过道里面，很难一手拉着小孩一手拉箱子。
难道这些说我的人都希望被白人如此对待吗？还是说他们希望自己的老婆孩子被白人如
此对待？
为什么他们对那个乘务长的的态度视而不见，反而说她说没有空座是委婉的说法，而我
说的确有空座就算是让别人没面子？
我来美国也有很多年了，来来回回坐飞机也很多次了，这样明目张胆的歧视还正是第一
次见到。

昨日。带教我的那个Attending现在是否康健。
祝大家新年快乐。也祝所有曾经教导过我们的师长，身体健康。
一大早，纽约下起了大雪。
早上七点钟天还是好好的，只是阴阴的。可是七点半出门时地上已是白雪皑皑
一层了。雪下的很急很快。扫雪车都还没来得及就位。刚下雪的时候路上最滑，一路上
，我竟然几次出现了车轮打滑偏出了车道的现象。下雪天开车的经验不少，可是今天我
是第一次有紧张的危机感。幸好，今天是新年夜，路上行车不多，不然是车祸都出了多
起了。慢慢地以8-10迈的速度爬行。本来去医院半小时的时程翻了一倍。到了医院发现
我的Attending还困在高速上呢。等他爬到医院时已是晚了一个半小时。他还庆幸着呢
，要是换了平常上班高峰，不堵个3，4个小时下不来。
今天是我在这个社区医院冠脉室的最后一天。这个医院的冠脉室的工作量相比
我们大学医院要轻松不少。今天的日程安排上有4个病人，一个门诊病人和3个住院病人

发帖数: 125 | 21 自从开始usmle考试，到面试，到match，一直都是在麦版上面取经。前辈们的经验一直
都是自己一路走过来的指引，收获很大。有童鞋发信问我externship的事情。因为我做
的是commercial 的通过Americlership，之前在版上也召集过人一起拿group的
discount。看到旁边有帖子强烈批判这个company的，那么我也上来说说自己的经验，
希望可以给路过的童鞋一些参考。
本人家庭主妇一枚，毕业年限>10年，没有任何connection，之前自己在本地尝试找过
externship都未果。有朋友介绍过OB，间歇做了2个月，后来因为那个clinic的政策改
变，没有办法继续。当时commercial的externship可以说是自己唯一的办法了。而且，
因为自己脱离临床很久了，也急于想要些clinical experience。当然，如果你有
connection，可以自己找到OB或者Externship，有美国的MD给你写very positive的推
荐信的话，那么我还是建议没有必要额外花这个钱。因为commercial的确实相对比较贵
一些。

Conclusion: 虽然当年麦地求助无门，却发现4～7年前各位前辈热心的面经分享，受益
无穷，给面试增添不少信心。遂鼓动小组全体成员，独乐乐不如众乐乐，为2015申请
path童鞋鼓鼓气。

道什么guideline，我们要怎么做，不要impress resident, resident的心态是，你要
把活干了。他们最喜欢的intern是，我不怕你傻，不怕你犯错，怕你说谎，只要你说一
次谎，就不会相信你了，比如sodium level 你没查，但是你说138，etc， 答应了的没
干，都是说谎，失去信任是最糟糕的事情。 不怕犯错，第一天做intern，很焦急的是

一种是在填写ERAS系统后生成的全面翔实的CV；
另一种是面试时用到的简化版CV。
以下是Jeff在这两种CV准备上的一些心得，和大家分享。
第一部分：ERAS的CV
ERAS的CV上，使program初筛时的重要参考文件，CV的内容和表述对于我们拿IV是至关
重要的。在这方面，Jeff其实是有一些教训的，而只是在几位前辈们的指点下，才意识
到自己CV的不足之处。所以，在这里希望给大家提个醒，不要犯同样的错误。以下是我
觉得需要注意的几点：
1. 避免仅凭自己的个人感觉准备CV：
在ERAS上填写CV的时候，要避免“想当然”的做法。因为我们作为一个申请者，很难做
到从PD和面试官的角度看自己的CV；同时，有时候虽然做了一些ob，但我们中的很多人
对某些专业还是不是十分了解（比如Pathology）。如果能找到相关专业的正在做
resident或者attending的前辈来把把关当然最好，他们的建议往往能让我们避免很多
漏洞和错误。一旦提交上到ERAS系统里了，就没法改了。
2. 简明扼要，不写无关或者关系不大的内容:

the limits of the institution. Abide by these limits. There is nothing
worse than me reading a note in the chart from someone who I know is an
observer. I have to inform the attending that the observer cannot see or
chart on patients. The note has to be removed and sometimes risk management
has to be informed. Don’t overstep your boundaries. Many of the interns or

对我就像新的一样。对内科来说，无论Match上了也好，需要重装上阵也好，有了实际
工作经验，才更能感觉到这些经验，字字珠玑。
能够Match上，能够生存，或者做得更好，不仅仅需要Medical Knowledge，
还要有作者那样的学习能力，效率，总结能力和情商。
“Medicine is a science of uncertainty and an art of probability.”
如果Match上了，可以思考对IM intern工作的要求，理解了吗，准备好了吗？
需要准备Match的同学，该如何提高，如何能够让PD们看到自己充分准备好了，有能力

转眼一年的intern就要结束了，对于病理从0开始，总算是有了初步的认识。写下来跟
大家分享一下。
我是个铁杆病理，当初申请只申请了这个，最主要原因是看中了不要值夜班，工作时间
相对短，事实证明我是对的也是错的。病理工作时间虽然没有内科长，体现在不用那么
早到，在surgical path我的工作时间平均是早8点到晚7点，偶尔到9点。但是需要大量
阅读，不象内科工作可以学到大部分东西，病理大部分的东西是从书本获得的，自己经
手的case实际非常有限。所以每天保证一定时间的阅读非常重要。
那么作为intern你应该做些什么呢？每个program 的schedule都会不同，但大同小异，
我拣主要的说。
Surgical path
Cover frozen之前，把OR的schedule打印出来看一下，估摸哪些手术会送冰冻，最早的
时候我甚至把可疑的case病史都查一遍copy到word里打印个大概。这样你去取冰冻的时
候就知道外科医生大概要看的是什么。后来我觉得这个有点费时，就不看病史了。因为

第一个月在VA的CT surgery，第一天是6月24日星期天，PGY-2这天起休息一周，到7月1
日才开始重新上工。整个service 5个attending (4个cardiac，1个thoracic)，一个
fellow，两个intern。病人一般是术前1-2天住院，术后一般还是intubated，直接进
Surgical ICU，几天后去PCU (step-down unit, 偶尔有病人去Ward，就是普通病房）
，然后去rehab, nursing home或者直接回家。
每天早上4-5点到，我们两个intern，每人管5个病人。我的co-intern抽签抽到patient
list的前一半，全是SICU病人，我抽到后一半，1个SICU，4个PCU病人。6点跟fellow
查房，基本就是电脑前的sitting rounds，intern汇报，presentation格式比较简单，

1。 我的attending今天终于表扬了我，一个病人来了SAH, chronic sagittal sinus
thrombosis, 他给我讲了一大堆这种thrombosis的原因，我都听着并一一记下来，临看
病人前用老刀介绍的方法，问了声这个病人有过面部感染吗？，attending马上说good
question， 然后我注意到他在录transcription,和和其他医生讨论这个病例时，都特
别提到了病人的面部感染史。真的像老刀说的要hide自己。
2。老刀类的attending：我今天才知道为什么我的attending对我这么好，ICU的一个
