我申请的是Davita在我家附近的一个dialysis center，面试我的是facility
administrator,他几乎是照着一张list，问的几乎全是bahavial questions,要我举例
。我是new grad, 还问我为什么选DN,为什么不选医院，问我如果得到这个职位，最大
的挑战会是什么。
我没有得到这个职位，但是真的很喜欢dialysis nurse的工作。希望楼主面试成功。 |

oligomerization state is sensitive to salt concentration, otherwise salt
concentration should not affect the purification.
In fact I know some people actually using size exclusion column to do buffer
exchange instead of dialysis when the sample volume are small.

我的FA很NICE，所以轻松水过。我当时就说了一下我的经历（刚毕业，还在家休息了一
年）。然后我跟她说我很喜欢DIALYSIS，因为我以前在这个公司的其他分部实习过，我
说我了解一些你们的机器（其实就是按RESET，其他我啥也不懂），然后我说了一些关
于透析方面的专业词，并且后来她带我参观CLINIC的时候，她每介绍一样东西，我都能
稍微说出个1.2来。其实她也没有问我什么特别的问题，就这样水过了。
我觉得你的经验比我丰富，应该是没有问题吧！！不过，像我们公司，（我在德州的
DAVITA）RN的话，有BAY NURSE 和CHARGE的分别，前者注重实践经验，后者注重管理。
所以后者的要求也比较高。当然，RN的话，关于时间控制管理和病人•TECH&#
8226;护士之间的MANAGEMENT要求是比较高的吧～～～～

发帖数: 417 | 22 以前是，现在少了。 Anemia 基本上是dialysis病人的通病。ESAs（Epogen & Aranesp
etc)以及iron的使用可以让大多数病人的hgb保持在10 - 12之间。但hgb太低的话，还
是要的。取决于医生了和病人的情况了。 |

dialysis日给500mg, dialysis 日在dialysis后给250mg? 结果伊就急了：nonono,是非
idialysis日给500，dialysis 日在dialysis后再给250mg.我一听，就乐了，显而易见
啊，dialysis日要加剂量。结果药剂，我们，谁都没发现这问题。伊的原嘱也翻给伊看
：change Keppra to 500mg IV on non-dialysis day; on dialysis day, give 250mg
after dialysis.伊看完也乐了。不过病人其实没事儿，就是防止seizure.完了，伊今
天的嘱就写成：on non-dialysis day, Keppra 500mg PO daily. On dialysis
day, give another 250mg of Keppra PO after dialysis. 其实更清楚简洁的嘱法是

发帖数: 218 | 33 我来说说吧。这边的透析护士分acute mobile dialysis and chronic dialysis.前者
要到不同的医院给病人做治疗，根据scheduler的安排，你一天可能要去2-3个医院。有
时候一个医院做一个病人，再去下一个医院做下一个病人。这个活得看公司怎么给护士
算钱。多数是paid by case，现在的行情大概是150-170刀做一个病人，少数公司按小
时算。如果按case算，透析护士必须要做的很快。我看到有些护士为了多干活，偷偷把
病人的治疗时间给掐短了，医生要求做3个小时的透析，护士会把时间掐到2个小时45分
钟。如果在同一家医院做3个case，可能只要12-13个小时就做完了。如果是公司按小时
给工资，这里面猫腻也很多，护士就可以适当磨羊工，反正我做3个case，可以拖到15
个小时。如果还要开车到不同的医院，开车的时间也是要付钱的。这样下来，3个病人
的时间可能要花到16-17个小时，如果12个小时以后是双倍工资，那这一天下来，工资
会很高。不少中国护士都做这个。比较适合单身，或者家里有另一半专职照顾小孩的护

一般的见医生，不管啥问题，你先给１５刀。然后，你的保险按照他们
承诺的百分比（比如俺保险是80%保险付，20%自己），来付他该付的百分比。
但是，保险公司有N个政策可以拒付他们的比例，比如他们这个不付，那个不付。
当年俺的N次验血，里面大概一半保险都号称不COVER。我吵了无数的架啊。：）
out of pocket，是说，你的那个百分比（比如俺是20%）累加起来，假如超过了
某个数目（1000，或者2000，或者2500，或者5000），那么之后，你就不用付那个
百分比了，保险付100%。 但是，只有那个20%参与累加，你的COPAY好像是不参与
累加的，很多其他的项目都不参与累加，比如当年俺手术的麻醉不参与计算。很大
的数目，要求自己付。保险是牛牛，俺们惹不起啊。人家号称啥不付，咱一点儿
办法都没有

的不够。可能有free biotin在 。 下次我一定试试你的dialysis的方法。我octet 信
号才3-4 nm 有的时候更低。就是很低就饱和了。确实说明binding site没了。
另外我是label 4°C 过夜。这样就成了三天时间label一次了。。。
另外我用的是maleimide-PEG2-BIOTIN 50mM HEPES buffer. 标记SH group
另外我也有NHS -LC-BIOTIN... 本来想用BSA 去label做个control 结果bsa也load不
上去
听了你说的。我感觉真的要用更多的dialysis

9个candidates一起面，4个AMG （其中2个申prelim），4个印度人（1个女的年纪比较
大，另外3个应该是fresh grad）
这次跟3个faculty member面。第一个是PD，她人很和蔼，上来便问我在extern期间学
到了啥，有啥interesting patient，我很从容地讲了一些自己的经历和体会。然后她
问我free clinic的Dr.C and Dr. P (他们都认识，且Dr.P是他们pg的previous pd)，
”they have put in a lot of good words about you”，我的天，看来平时积累人品
真的很重要，我没主动请人家帮忙，人家竟然也说了些我的好话，真是阿弥陀佛。然后
她问我有啥question。面试其间我着重强调了，自己在local，非常想留在这里，对他
们pg也是仰慕已久，感觉PD很positive，“definitely, we are going to highly
consider you”。跟PD过招，这次没给自己留太大的遗憾。

吃福利最多的是主流人种的低收入的那些，但是这都不是太大的问题，只要经济增长了
，吃福利的人就会下降。关键是经济增长问题。
把眼睛老是往某些少数民族盯，真是没出息。
但是美国政府开支中最大问题是medicare，这个是67岁以上医疗全包，很多费用是在临
终前3个月上面花费了巨大金钱，这个和什么种族根本没有关系。美国政府其他开销不
是大问题。所以Obama的Obamacare方向没有问题，就是timing有问题。
以下是一个doctor的建议，如果cut 美国medicare cost by 50%，他说的不见得全对。

1960s and used in very small number of patients. I don't think any of them
have been commercialized.

个华盛顿邮报专栏作家。多年没想到此公，刚才搜了一下，发现他已于 2007 年过世（
于是又想到了昨天猴脸的帖子，提到几位已故世的歌星。）包可华的特色是笔调幽默，
对于事物的观察非常敏锐与 opinionated（废话，要不然怎么当专栏作家？），常以讽
刺辛辣但却又幽默的笔触批评时政。让我印象深刻的几篇专栏文章包括讽刺法国人爱喝
酒、批评美国电影太黑太黄、讽刺福特总统满地找竞选搭档、讽刺拥枪派所引起的 “
武器竞争”、。。。。。等等。曾经有本书是他的专栏文章合集，可惜现在网上找不到
了。
（想到这两天版上拥枪禁枪的帖子，结果到最后一定是大吵一架。Buchwald 他反枪，
但他懂得用幽默的文笔来反枪，让不管是拥枪反枪的读者，看完后都是哈哈一笑。这就
是高手。）
他在过世前为自己的过世写了一篇专栏，贴出来分享一下，看看一代文人是如何以幽默
平静的心情面对自己的最后一页。
