CCS 急性胸痛的体检前的 emergent management 如何处理？
我觉得因为不做体检 和 必要的X ray，EKG，很难给出相关的 急诊处理。
比如：
1. 心梗，心绞痛：aspirin, nitroglycerin, beta blocker。体检前就该给。
2. 主动脉dissection： aspirin应该不能给吧？应该先做PE和 X ray，EKG，再给予治
疗吧？不能直接给 nitroglycerin，beta blocker吧？
3. 急性肺水肿：给了beta blocker可能会加重肺水肿和心衰吧？也应该 PE 和 X ray,
EKG 后再治疗吧？
对于这3中情况该如何给出各自的急诊management？（因为一上来都是胸痛，很难直接
给药）

Material: MTB3 and UptoDate. Step 1/Step2: 250/249 but those were 3 years
ago. Step 3: 227. Time: on/off for 3 months.
Good luck to everybody.
Not a whole lot of thoughts about this test. if you can memorize MTB3 and UW
, then there should be NO problem to pass it.

You are totally right. We were not sure about the facial weakness and were
thinking that the patient had something rather than stroke because of the
single limb weakness and significant pain in the elbow.
We did not expect the dissection. I went back after we saw the CT, he had no
pulse on right arm.

No consensus. Even TPA was proven safe but no enough data.

Breast cancer with modified right radical mastectomy (right breast) and

dissection on 20th of May, 2011.
A lump under her right armpit was self-found but it was not checked out
until 1 and a half months later.
Her histopathology report has revealed that a tumour which was noted to be
at 2.5cm from the top of the nipple, 2.5 cm from the nipple.

容气度，隐隐透出当年伊丽莎白泰勒或是奥黛丽赫本的影子，这在当今的年轻女性里面
，很难见到了。
刚跳了几步，她就对我微笑，说: “Young gentleman, please don’t jump, please
slide”。我说抱歉，立刻明白了，是我因为很久没跳，竟忘记了拉丁舞的要诀“出胯
不摇肩”，而是下意识引进了北京平四的跌宕起伏步法。照她说的改过来以后，果然觉

friend to get the pathology slides and get 2nd or even 3rd opinion from well
known hospitals in China. The reason for this is that the treatment and
prognosis are totally different whether the appendix tumor is primary or

是学医的么，为什么要给病人这种感觉？我refer病人给外科对我来说一点不影响我的
收入，我要是不refer，在我手里多“保守观察几次”，多做几次超声，我还能多赚几
次钱呢。
甲状腺肿瘤预后良好，这又不是第一天知道，恶性肿瘤总是恶性肿瘤，延误治疗引起局
部扩散甚至肺部脑部骨转移的还是有的，为什么要在明知有这个风险的情况下仍然等待。
所谓的密切注意，我们建议1cm一下的或者1.5cm超声显示良性可能大的nodule，LZ这个
两次biopsy疑似恶性，已经不在讨论范围之内。你提出密切注意，怎么密切法？每六个
月？多少年？那些超声很多如果不在同一个人手里做，根本没办法互相比较，做的人不
一样测量都不一样，难道等到淋巴结都转移了去手术？然后本来就是一个很简单的甲状
腺全切变成central and lateral neck dissection?越晚手术，涉及创面越大，可能引
起的并发症就越多。
至于你列的文章，第一篇明确说，1cm的nodule没有必要FNA，这也是内分泌guideline

is haemodynamically stable. If she is not haemodynamically stable I would
even start systemic anticoagulation therapy before CTPA (it is OK with NSTEMI
anyway). Either way I would not wait till cath directed thrombolytic is
arranged (I am not sure if it is available in our hospital in the middle of
night but I am sure it is not able to be arranged within an hour and I am
not comfortable to wait).
