不过你提的问题都是不错的问题，也确实是目前MALDI imaging存在的问题
不过还是那句话，LC/MS不是万能的，MALDI imaging确实是有力的补充
in situ的检测还是跟萃取得到不一样的信息的，比如萃取可能会破坏一些native
conformation,有时native conformation disorder才是关键。
另外有时biomaker分布才是关键，如果萃取的话，
每一块tissue都dissect出来还是比直接imaging烦很多
现在的MALDI的速度跟自动化程度，跟数据处理都提高很多了，
imaging的速度很快，关键是样品制备，也比以前快跟容易很多
还有你前面说的定量的问题，也不是不可以解决的
另外现在imaging 慢慢开始朝bioimaging以外的方向发展，这些都蛮有意思的
总之，不是一无是处，还是有research的意义所在的
imaging |

哥提到一本“Surgical Pathology Dissection-An illustrated guide",我做见习时的
几位PA也强烈推荐，不知道是不是很多program都用的，反正听说在grossing方面是简
单易学易用。 |

发帖数: 410 | 19 我既没带什么书，也没有什么dress code,不过事先和带我的医生通了电话，表示都不
需要。但是后来去的一个group大家都是dress shirt and tie,所以我也穿的正式些。
可见各个地方风格不同，最好先问清楚。
我觉得Surgical Pathology Dissection-An Illustrated Guide这本书对于grossing入
门很有帮助，过slides的时候靠的是刚刚考过board留下的印象还有以前的记忆，医生
其实对你的期望值很低，一旦你的表现能超出一些他的预期会有很好的效果，这个个人
看自己的情况不同吧。大部头的书恐怕现啃也啃不出效果，主要是要表现处好的学习态
度以及学习过程中积极的发散思维，还有和医生的personal communication.这些都是
我的个人经验，尚未得到证实，呵呵。 |

，收病人是小事，关键是还要cover20几个ICU病房病人，还有procedure.最可气有一次
收了typeB aortic dissection，cardiology拒绝收病人，第二天consult cards, 他们
attending上来就问我为啥不consult CT surgery,我心想typeB肯定不要手术，表面还
得陪笑，结果外科来了跟我预料一样。不过这个attending早已臭名远扬，我已经有了

Agree with Againstwind and Hemocell. Pt has totol thyroidectomy with neck LN
dissection with pathology confirmed advanced thyroid cancer, most likely
follicular cancer. Pt may has parathyoid injury or removal during surgery
causing low PTH, which is very common for such surgery. Post-op therapy
involves thyroid hormone supplement and I131 treatment for this stage of
cancer. Thyroid hormone reduces TSH, to some extension, inhibiting

matter. don't wait till you are 'good enough'.
and not just pronunciation. It's mostly how you communicate - First
understand what people are saying, and respond appropriately, then you can
initiate the conversation and dissection other peoples response. Can you be
the one that direct a group's activity in English and make it lively? If you
can, you pass the first measure stick. |

I attended an webinar by NCCN on adjuvant therapy for NSCLC pts and asked the case. The presenter is leaning towards using adjuvant chemo therapy. I am still collecting more openion on this. My mom will have first first follow up CT in Aug. I guess (guess only) if obvious disease progression were seen, chemo should be used. I still have the hope that if she can rest well and build up her immuine system, she will be able to keep the cancer in check.

2.成绩还是要越高越好，因为大部分人都不高，如果你高， 你就和别人不一样，而且
关乎fellowship申请
3.对成绩的expectation， match之前和match之后对成绩的要求不一样， 而且看考经
不能一味follow，各人情况不一样，step1.2 的成绩不一样，距离step2 的时间也不一样
4.如果只是一味pass，一个月也可以，但是前提是紧跟着step2，但是知识一定要学，
现在没掌握将来考Board 也是要掌握。
5.如果要仔细学，好好学的方法
• MCQ 和CCS都很重要，ccs如果fail掉3个case，整个step3就fail了
• 如果肯花时间好好学MCQ， 功力一定会大涨
• 考得是临床思维 概念， 不需要读书，考的是第一反应，而不是你知道多
少，是灵活的临床应用
• 要把握三点：阅读速度， 自信， 第一反应
• 最快最重要的是题库，要反复做

Look for left clavian artery stenosis.

尸检只是很小部分工作，很多PA不做尸检。
通常大的病理医生group需要PA， 广告上肯定是要求有PA执照。
因为很少符合条件的，没有PA执照的兄弟姐妹们可以试着去应聘：
1. 有中国病理/解剖工作经历
2. 建议看Surgical Pathology Dissection: An Illustrated Guide，网上有电子版 |
