10 lbs in 24 hours. 检查没有发现任何问题. self recovery in a week.
有哪些怀疑？检查和治疗建议？替一个朋友问。多谢！ |

pitting edema) without cardio or pulmonary conditions.
Already start lasix 40mg po daily-----no improvement
Slightly anemia (Hgb: 10.2), albumin: 3.2
Not on any medication except new start Lasix
Not a good candidate for Ted hose due to her obesity.
Any other outpatient options? I need input from you guys! Thanks |

发帖数: 65 | 3 领导两年前从楼梯滑倒，有点摔伤，但骨头无恙，做了物理治疗，没有觉得有帮助，后
来慢慢不疼了。一个月前，突然又有疼痛。找了X光， MRI， 结果说“mild
nonspecific edema surrounding the distal aspect of the pes anserine tendons
near their insertion on the anteromedial tibia. No focal fluid collection to
suggest bursitis.". 医生给开了外用的消炎药，diclofenac sodium gel. 领导用了
两周，好像不见效。 医生说如果不行，要在膝盖注射药物。我不知版上有没有医生，
看有没有好的办法。
顺便说一下，我们看医生前用了一周Thermal pad， 好像管用，但又好像治标不治本。 |

CHF?! Isn't UW (STEP 2) obviously wrong? That constriction is a compensation to the decreased
Effective volume in the circulation, and it actually can help conserve the renal filtration. What do you

Gaining 10 pounds in 24hrs and whole body edema are most likely due to heart
failure 2 to bradycardia.
It is interesting that this is transient and completely recovered within a
week. Any precipitating factors like infection? Stress? Any past medical
history? |

For history:
Abd Pain: OCDFPPLIQRAAA all needed
--She did not have abdominal pain when I saw her. She mostly compainted of
bloating and mild distension. I don't have the details of her abdominal pain
from before. All I knew was that it was generalized and not that bad.
Diarrhea: OCDFPPAAA ABCO, blood?
--1-2 times a day, watery, large volume. Unclear if it was greasy. No
visible blood. No constipation. No tenesmus.

Majority consistent to GI loss (malnutrition/malabsorption)
Edema is due to hypoalbumin and low protein. Originally lower extremity edema was thought to be due to systemic involvement. But when I saw "4+ pitting edema up to low back" on PE on day 2, I knew it could be due to low protein level. I appreciate the good PE you did. :)
Hypokalemia is most likely due to GI loss too.

edema， etc。 病因也很多， 不知道你说的到底是那种情况？
比方说papillary edema， 根据病因可以选择药物治疗和手术治疗， 所以查清病因是

死前几天都是处于不吃不喝状态。当然这个问题的前提就是iv和胃管灌食也停止了，否
则没必要解释。
这个贴主要是找理由。但是，如果你想一想，就可以勾画出病人在hospice的最后几天

sodium是有害的，因为sodium在怀孕期间流失很多，不补充的话会影响体内的微平衡。
摄入盐分过多造成的水肿其实是对婴儿无害的，除非有高血压之类的。

从今天开始，每星期我会上一个case。 周一history, 周二physical， 周三initial

genedx，是我们出院(04/13)后一个月报的(05/13)， 这个极有可能就是我们宝宝的结
果，被报呈良性。

这个变异发现了10多年了， 这个德国医生的链接的文献没有说是良性的， 你自己看看
下面德国医生附上的文献。我不是医生，我看不出来说这个变异是良性的。你如果愿意
看看我家宝宝cpt2的两个结果， 一个1.5%酶活性，一个15%酶活性，跟下面的温度相关
吻合得多好。
我家宝宝的下降更多，大概是因为他的变异更多。这个特征的变异很多年了，但是
cchmc和Baylor都没有查出来。你说说他们不知道？！

小时回家，一下车就发现膝盖剧痛，很难伸直。后在家休息了两天，一个礼拜后走路多
了膝盖会痛，一瘸一瘸，伸曲会痛，只好去看家庭医生，然后做了MRI,发现：High
signal around th media collateral ligament. Partial tear of the medial
collateral ligament is suspected.
The lateral collateral ligament is within normal limits. Quadricept tendon
is within normal limits. Questionable sprain of the patellar tendon at the

Match的结果终于尘埃落定，周1等电邮的时候非常紧张，因为成败就在此了，虽然一直
告诉自己，今年只是热热身，成败都可以接受，但到真的揭晓的时候，还是很紧张，毕
竟一旦卷入其中就身不由己，何况自己也还是有所付出。到周4等match program的时候
虽然也有点紧张，但程度已经大大减少，1个原因是毕竟match上才是头等大事，去哪里
尚在其次，还有1个原因是心理还是有所准备了，虽然希望留在home school，但直觉告
诉我恐怕是非常困难了，在我rank list上第二位的program应该就是我要去的地方，果
不其然。经过几天的等待，发现果然是要搬家了。

"A woman came to the ER with severe abd pain, had dozens of visits through
the years, multiple specialists and told she had irritable bowel,
colonoscopies and CT in past c/w bowel edema, non-specific at times.

发帖数: 44 | 41 今天得知了step3的成绩，210+，心里的石头终于落下。终于轮到我给USMLE说再见了。
回顾从step1自己闭门造车考了只带一个9的分数， step2开始参加小组学习，做了所有
能做的题只为拿个99，虽胸有成竹且考试感觉很好，最终但又得到一个9的分数。虽然
这次成绩不高，依然很高兴，毕竟不用发愁如果没过的话住院医开始后没有时间复习该
咋办。过去的3年中得到太多人的帮助 (此处略去人名20个)，绝大多数甚至从未谋面。
今天发贴也是为了回报我们这个社区吧。
自我介绍：毕业>15年，220+/220+/1st/210+；
复习时间：6月，头4个月每天约2-3小时，周末由于陪孩子们玩（前两年欠的太多，你
懂得）及做点家务（再不做但心被老婆炒掉），反而复习不了多少；最后2个月每天4小
时，鼓励自己考了这个2年内再不用受这个罪。
复习资料：随大流，MTB 3, UW CCS, UW step3 Qbank, UW step2 Qbank;
模拟：考前2月，NBME 1 190+, 考前1周，NBME 2 220+;

发帖数: 5870 | 46 还不到40岁。。。。北方人，从小就住在阴冷阴冷的背阴面，一直有关节炎，以前打球的时候保护措施也做得不太好，最近几年走路的时候两腿膝关
节都有声音。平时如果活动多了，就会膝盖不能伸直或者不能弯曲。但是通常，休息休
息就好了。前几天右膝盖忽然不能伸直了，并且到了比较严重的程度，连续几天都不好
。这才去看医生。照了X-ray 和 MRI.以下 是MRI的分析结果。
骨科大夫建议右膝盖手术，他说手术中会：
1）清理骨头表面，让它更光滑
2）清理关节腔内的各种碎片。。。
不知道是不是真的应该动手术。看病折腾了这么几天，现在不知不觉中，右腿又可以伸
直或者弯曲了,只是还不能快走和跑。现在很纠结，不知道是不是应该动这个手术。。

Patient was treated with steriod, and symptoms get better:) We were
joked at for telling them that the imaging findings are most consistent
with CNS lymphoma, and the brain and mediastinum were separate
processes:)

guess it is not very clear, it depends on each patient's clinical situation.
For CNS lymphoma, corticosteroid does have substantial anti tumor activity.

schizophrenia，stabilized with Clozapine 200mg daily 已经快半年了。最近出现
了双下肢pitting edema。 我有点担心是不是Clozapine造成了cardiomyopathy，一个
其不常见的药物副作用。我先让病人去看她的PCP。结果PCP給她Lasix 20mg daily，
EKG 未见异常。我給PCP打电话，问问他的意见，到底是何种原因造成了水肿。这个PCP
说，肯定不是心脏的原因，应该是下肢静脉thrombosis等堵塞原因引起。我问他那为何
双下肢都有，为什么你用Lasix，为什么不马上做sonogram做诊断。此PCP说他会留下
sonogram的order，让病人第二天去拿，他第二天要去休假。现在病人的双下肢pitting
edema已经明显减轻，after 服用Lasix一周多。
