发帖数: 91 | 2 我父亲（64岁）2/5开始发烧,最高至103.5. 2/9有言语障碍,右踝和左肩曾经非常痛不能动.被诊
断为Streptococcal meningitis. 之后上了抗生素之后症状缓解.恢复了语言功能。之
后全身关节有时有轻微的胀痛,行动缓慢,左肺部有轻微的fluid,出院时连续两天没有发
烧，但是白细胞数高于正常值：13K左右。
另外我父亲86年曾经出过车祸，摘除了脾脏，做了肝修补。直到几周前身体一直很健康
（除了有些关节炎）。
今天是上抗生素的最后一天，自从出院（2/16）之后到现在还是每天下午有低烧(100度
左右)。医生听了肺,说很好，停药，让我们过两天再去复诊。我很担心他这个低烧的状
况。
我想请问各位医生大虾的opinion.可能是什么问题？

月，现在协和基本上觉得无能为力，所以患者家属想查找一下看看美国这边有没有在这
方面特别有经验的医院，如果有希望，哪怕是希望很小，他们考虑把患者转到美国救治。
如果你有生么好的的建议，请告知，不胜感激！
患者的一些情况见下：
患者： 女 34岁，2009年12月患红斑狼疮疾病采用激素和免疫抑制剂治疗，病情稳
定，2013年3月感染隐球菌脑膜炎，目前在中国协和医院治疗8个月，感染一直反复。最
近检查T细胞群中的CD4只有39个，正常人的值应该为500个。
2013年
3月12日 入住宣武医院，开始口服大扶康
3月24日 从宣武医院出院，每天在家口服大扶康
6月9日 脑脊液墨汁染色阳性，脑脊液抗原1：2
9月6日 脑脊液墨汁染色阳性，脑脊液抗原1：64
9月27日 入住解放军301医院，开始点滴国产两性霉素B脂质体（共使用0.71克）
10月9日 转院到协和医院，停用国产两性霉素B脂质体，使用大扶康点滴

. For example, a pt. with headache and his csf is full of WBC, is it p.
strep or N. meningitis. ? I think if the pt. has no rash, p. Strep is a
better choice,although both are very likely, and N. Meningitis will not
necessarily show rashes.
Hope you guys can understand what I mean in the real test. The above Q if
the pt. has no rash, maybe VIPoma, because it is MORE LIKELY that

以前总把CCS和CS混淆，其实这两个有一个共同点就是open choices和uncertainty。在
这总场合，最重要的可能就是要有一个严格的protocol，这样可以尽量避免遗漏。考过

bacteria而不是virus， 当然不是绝对的。 我不熟悉加拿大的计数方法， 这么多的
小数点不是诚心跟医生过不去吗？
第2，3张就正常了， 可能与抗生素使用有关。
有没有HSV PCR的检查报告？
MRI报告我不是太明白， 需要把片子贴出来我才能评论。 感觉可能是meningitis引起
的局部脑梗塞，从而引起seizure。
现在CSF已经正常了， 该处理的是后遗症， 癫痫是一部分， 大脑功能的损坏程度才是
真正重要的。 说实话， 我感觉不好， 而且除了癫痫控制， 应该没有其他更好的办法
了。

空气传播的常见传染病主要是呼吸道的TB还有细菌性MENINGITIS，如果是说有空气传播
的危险，那么实际上在美国的厕所更危险，因为美国厕所往往更封闭，不像很多中国公

当然可以，现在病毒基因图谱已经完全明了，各国CDC都可以查阅基因图谱。病毒通过
与人类ACE2受体结合致病，并无任何证据证明这个病毒具有针对性地感染只有东亚人群
易表达的细胞受体。

i agree that 21 days course of antibiotic is needed, furthermore, i think it
should be intravenous rocephin(ceftriaxone) instead of oral doxycycline,
this is my rationale, for primary infection, doxy is good enough, plus it
also covers other tick borne diseases,
while in theory the patient may have recurrent primary lyme disease due to
another tick bite, but with headache, you really cannot exclude lyme
meningitis, so you HAVE to offer a spinal tap, and if i were the patient, i
