Presentation: Severely headache; shortly unconciousness, now awake; BP 159/
99 mmHg; respiratory 13/min; HR 66/min; right pupil dilation; mild left
hemiparesis.
CT will be
A subdual haemorrhage; B tonsil herniation; C cingulate herniation; D uncal
herniation; E SAH.
If you can correctly answer this within 1.5 min, you are almost ready in
your neurology part. |

May be, you are right, we have to consider acute SDH.
Neurological findings associated with acute SDH may include the following:
Altered level of consciousness
A dilated or nonreactive pupil ipsilateral to the hematoma (or earlier: a
pupil with a more limited range of reaction)
Hemiparesis contralateral to the hematoma.
However, considering this: 'Subarachnoid blood can be detected on CT
scanning in as many as 60% of people with traumatic brain injury', the

Let me summarize and go though the answers one by one. Please correct me if
I am wrong!
A - subdual haemorrhage
Acute subdural hemorrhage usually has a very severe primary brain contusion
and injury (MORE than epidural). If this is the case, the pt's GCS probably
is even lower and might keep in the unconsciousness, can not be awake again.

还有一个program，Barrow Neurological Institute 今天已经收到第三封信了，一开
头Happy Tuesday！上次一开头是Good morning! 以为是好消息。结果看了好几行，还
是一个要你耐心等待，还在review。看来申请那儿的人不多，这个秘书还是挺有耐心每
隔两周就发一封。 我到要看看发几封后才把我据掉。不过比那些silent rejection,
收到这种信让人心里还存一份希望。 |

The situation is really bad. I started to worry after I read this post from
USMLEforum. In neurology thread, one post says

You need some supplementary material for Neurology. FA does not do a good
job in that area. |

主要诊治脑血管疾病（脑梗塞、脑出血）、偏头痛、脑部炎症性疾病（脑炎、脑膜炎）
、脊髓炎、癫痫、痴呆、神经系统变性病、代谢病和遗传病、三叉神经痛、坐骨神经病
、周围神经病（四肢麻木、无力）及重症肌无力等，主要检查手段包括头颈部MRI，CT
，ECT，PETCT，脑电图、TCD（经颅多普勒超声）肌电图，诱发电位及血流变学检查等
。同时与心理科交叉进行神经衰弱、失眠等功能性疾患的诊治。
Neurology的申请需要锁定prelim和advanced的program，但是categorical让两步面试
化为一步成为可能。希望有任何Interview Invite或Rejection的同志上来留个言，让
其他焦急等待的人多一些信息。

It is so fake. Even if you are interviewing a well-known malignant program,
they will not ask in this way. Neurology is famous for their nerdity, but
they really want to attract good applicant. |

看到一个report 说phsical medicine 位置很少,大概只有30 non-US graduate
matched (2011??). 因为有很多位置osteo毕业生在抢.
Neuro的categorical 和advanced 有什么区别. 听说advanced 要preliminary, 不好搞
. 不知道情况怎么样.

I don't recommend neuroscience or neurology...
it's bbbbbbbbbbbbbooooooooooooooooring!
My advice to you, go straight to a medical school (no matter what or at
where)...or a podiatry school...
There are easy ways: marry a citizen or become a citizen; contribute and
earn some credit, such as social services; finally apply for the school...It
