I was hoping that people would ask for more stool studies, but maybe people
just assume that since she's had extensive work up, all the stool data would
be available. Anyway, here are the labs from this hospitalization and from
before. If a lab is not listed, then it wasn't done. The stool studies were
mostly done on a low residue, lactose free diet.

her symptoms.
When plasma amylase is increased, but the lipase is normal, a nonpancreatic
condition is almost always the cause.

I would consider a GI workup for any other organic diseases including
diverticulitis. For a rare condition, porphyria can cause similar symptoms
in young females. |

severe hypothyroidism will show cognitive signs and rarely related to pancreatitis. her hypothyroidism might have nothing to do with her abdominal pain at all. tons of people who are admitted to hospitals have hypothyroidism because it's such a common disease. |

plays no roles in her abdominal pain.
pancreatitis. her hypothyroidism might have nothing to do with her abdominal
pain at all. tons of people who are admitted to hospitals have

in acute setting plus a normal MRCP. With somebody who has autoimmune
tendency, GI vasculitis may be considered in the differential. I agree with
A++, acute intermittent porphyria is possible and a urine porphyrins
including porphobilinogen can be easily done. I guess if all are negative,
she then might have somatization disorder. |

Case 1 speaks enough for the importance of past medical history.
Case 2. Pretty much same thing. Two comments:
a). RCC mets to pancrease is rare. But it happens. I guess it has something
to do with anatomy. They are all retroperitoneal organs.
b)." RCC pancreatic met is reported causing islet cell hyperplasia."
This is interesting. If it is the case, the treatment should focus on mets
not the islet cell hyperplasia, right? So I wonder what endocrinologists can

unresectable, the strategy is systemic chemotherapy +/- radiology with the
goal of shrink the tumor and make it resectable. If it is metastatic disease
, the main treatment is systemic chemotherapy for palliation. If he has
very good performance status, use FOLFIRINOX; with OK performance status,
use gemicitabine based chemo regimens. If he has poor performance status,

if not done already. Like I said, advanced pancreatic cancer is very poor
prognosis. Studies show people with metastatic disease in average live about
11 months with the best standard treatment (FOLFORINOX); without treatment,
the mean survival is about 6 months. Even people with early and resectable
disease, only 20% of them can be cured. This is the sad and realistic fact.

characterized by exocrine pancreatic insufficiency, bone marrow dysfunction,
skeletal abnormalities, and short stature.
既然基因检查证实是SDS，就是你说的SBDS，我是相信这个结果的，因为这个就是科学
证据， 在没有其他科学证据与它相悖的情况下就得买这个帐。
至于国内医生说有不相符的地方，他们应该了解，syndrome的意思是综合征，也就是一
系列症状的综合，不是一个特定的病， 所以符合一些症状就可以诊断了。 我从来不把
综合症当会事，但如果有了基因检测这个实实在在的证据，这个综合征就是科学产品了。
所以我建议孩子父母在要第二个孩子前做一下genetic consultation。 |

2. CEA is mainly used as a tumor marker to monitor colorectal carcinoma
treatment.
