前两天刚转过去一个病人，anion gap metabolic acidosis，病人又是mental
retardation,不会complain,medical ICU fellow 来看了病人，觉得没那么重，不该转
过去，还是救了我的那个attending,坚持让他们把病人收走。交班的时候我让medical
ICU的intern一定要做腹部CT,没人理我。。。第二天一早，听说MICU急火火的让我们那
个attending会诊，病人 end up intubated,两个dilated sluggish pupil,后来发现
massive mesenteric artery thrombosis+necrotizing pancreatitis
这次medical ICU 的fellow 半梦半醒中还不忘问了我一句，你是前两天把xxx转过来的

results in low level of bile acid synthesis. Bile salts from the liver are
needed to break down and absorb fat. Any condition that results in decreased

的病人遇到的问题寻找答案。而且当天尽量搞定，否则累计下来，肯定不回再看了。

对我就像新的一样。对内科来说，无论Match上了也好，需要重装上阵也好，有了实际
工作经验，才更能感觉到这些经验，字字珠玑。
能够Match上，能够生存，或者做得更好，不仅仅需要Medical Knowledge，
还要有作者那样的学习能力，效率，总结能力和情商。

成绩出来了，涉险挤入了９９队伍。总的来说，准备的不是很充份，详见以前贴的＂我
的step ３经历＂，主要是在吃CK的老本；其他乏善可陈，成绩单里唯一的一个星号是
给了CCS，这里就说一说CCS好了。
以前总把CCS和CS混淆，其实这两个有一个共同点就是open choices和uncertainty。在
这总场合，最重要的可能就是要有一个严格的protocol，这样可以尽量避免遗漏。考过
CS的同学都知道LIQOR AAA和PAMHUGSFOSS的重要性。CCS也一样。网上比较流行的是
Neeraj的protocol，用google一搜就能找到。非常有用。我把他的protocol稍微整理了
一下，自以为是更加简洁有条理一些。给几个朋友用了用，有的朋友觉的没有时间应用
，有的朋友用了觉的还成，让我贴出来，最后的决定是如果考出来还行就贴，要不就不
出来吓人了。
象CS一样这个protocol充满了mnemonics，开始考试之前第一件事就是把所有的

最近科室里面掀起了一阵CCRN潮，科室还给大家一套AACN的复习材料给大家轮流使用。
AACN 有一盘CD 和三套模拟训练题， 我只看了CD，觉得非常有用的，每个系统把考点
总结了一下，很好的加强了理论知识。
我还用了pass CCRN CD 的题库，没有全部做完，cardio 和 pulm 实在太多了，大概各
做了70题，
其他category题目少的基本都做完了，每次无论对的错的都看解释加做笔记，虽然笔记
后来一遍都没看过 ：（
总共复习了两个星期，其中全职上班,不上班的日子每天复习8小时，上班的日子完全不
看书。
实战感觉和 pass ccrn 接近或者更简单一些，基本一分钟一题，题干也不长，磨棱两
可的做bookmark，做完全部150题 然后回头琢磨那些磨棱两可的，根本不需要三小时。
以下理论知识基本在考题中都有用到：
CVP, PAOP, PAP, CO, CI 这些正常范围一定要知道，怎么区分LVHF 和 RVHF，fluid
status,
preload, afterload,

I might miss part of it, yet my thinking is that pancrease transplant could
be a good strategy for alcohol abuse-related pancreatitis, though debate
would occur in terms of ethics, as you stated here. |
