ANTERIOR TILT或者说是lower back hyperextention。容易伤腰。
举重的时候pelvis永远保持neutral就对了。收紧臀肌有助于保持neutral pelvis。卧
推,overhead press的时候就需要收紧臀肌。有些人可能认为臀肌和core strength没

患者44岁,男,6年前因为肝癌做了肝移植 (符合米兰标准),之后无灾无病好几年。
去年2016年初发现轻微贫血,血色素12,因为当时在服小剂量阿司匹林,于是停服,看
看有没好转。托托拉拉到了年底,还是贫血,血色素10.5。医生说做个内镜。于是在胃
内发现一个肿瘤,内镜医生直觉是GIST。随后CT,但CT却没有异常。4月初第二次胃镜
,GIST依然还在,取样活检表明可能肝癌转移或者是非常罕见的HAC。第二次CT 报告大
小46x35mm,说比较第一次32mm变大了。可能第一次看片医生那天喝多了?肿瘤位置在
Fundus,非常接近GOJ。
AFP好久没有查了,因为肝上的情况一直很好,肝移植前AFP 100多,移植后AFP一直小
于2。上一次至少是2,3年前。也是疏忽了。现在AFP 254. 显然胃上这个要么是转移,
要么是HAC。
下周二医生还要安排一次PETCT,周三见外科医生,估计排除转移外就安排切除了。 求
这里的专家教授指点,这个情况如何是好?下面我贴上最近几个报告。

我刚翻书,说lightening是 "the baby has dropped, settling deeper into your
pelvis in preparation for labor and delivery".就是说宝宝落入盆内,就叫“
lightening”。
而说"At 0 station, the baby are said to be engaged in the pelvis, as the
largest part of the baby's head had now entered the pelvic inlet".就是说在0
的位置,才是engagement。
把我弄糊了,我听说第一胎入盆早,老问OB engaged了没有,她都说没有。敢情是问错
了?是不是应该问lightening了没有吧? |

Left Kidney: Normal;
Right Kidney: Normal;
Left Pelvis: 5mm
Right Pelvis: 4mm
医生解释是B超看到肾区有水,可能是小孩憋尿,不好的可能性就没说了,我猜想是积
水。
后来自己上网查了一下,好像中文叫做 肾盂分离,5mm是个临界点,是好和坏的边缘。
多见于男娃,不过我家的是女娃。
有些担心~~ 有类似经历的妈妈 /准妈么? |

a****e 发帖数: 353 | 23 我完全理解你的想法。顺和C都有风险。C的风险主要是伤口缝合,护理,恢复。这些很
大程度是人为因素,找个好医生,自己小心点,可以避免的。
而顺产的风险来自于不可预测性,虽然概率上很小。顺产不是任何时候都可以转C的。
小孩的头如果已经出了pelvis,到了birth canal,但身子还在pelvis的时候如果有危
险,就不能转C了。比如肩部难产,就要把小孩锁骨搞断再拽出来。
对于妈妈来说,只有顺产完了以后,才能知道tear/episiotomy到底什么程度。大部分
人二度或以下,三度就I am sorry了。四度就是大便失禁,very miserable。还有就是
产后尿失禁和子宫阴道脱垂。这些要是碰上了,OB技术再好也没有用。
还有一种民科说法是顺产后阴道会松,影响将来xxoo质量。当然,这个我没有发言权,
不知道是不是真的。
所以总的来说我更倾向剖腹产。我的OB和我说他可以在没有指症的情况下为我schedule
C,只要我不想经历labour。 |

看有没好转。托托拉拉到了年底,还是贫血,血色素10.5。医生说做个内镜。于是在胃
内发现一个肿瘤,内镜医生直觉是GIST。随后CT,但CT却没有异常。4月初第二次胃镜
,GIST依然还在,取样活检表明可能肝癌转移或者是非常罕见的HAC。第二次CT 报告大
小46x35mm,说比较第一次32mm变大了。可能第一次看片医生那天喝多了?肿瘤位置在
Fundus,非常接近GOJ。
AFP好久没有查了,因为肝上的情况一直很好,肝移植前AFP 100多,移植后AFP一直小
于2。上一次至少是2,3年前。也是疏忽了。现在AFP 254. 显然胃上这个要么是转移,
要么是HAC。
下周二医生还要安排一次PETCT,周三见外科医生,估计排除转移外就安排切除了。 求
这里的专家教授指点,这个情况如何是好?下面我贴上最近几个报告。

一路飞奔到ICU。 ICU静悄悄的,全然没有交接班前的热闹场面。夜班任务分配单挂在
墙上,6个病人,3个护士,一人两个。交接班的护士告诉我,两个病人都很简单,是
Trauma,一个是车祸,一个是从自己家的楼梯上摔下来。
车祸的女孩我是知道的,三天前 我上班时进病房的,因为车祸造成了颈椎的不稳定和
C2的骨折。病人刚到病房,脑神经手术医生已经等在那里准备给她装Halo。女孩一看到
Halo就开始声嘶力竭的哭喊……另一层楼的护士给ICU打电话抱怨,你们ICU的护士真会
折磨病人。
交接班结束后,我例行公事地查房。车祸的女孩正对着镜子往她的Halo上缠花绳,看到
我,浅笑着问,“好看吗?”我心想,“得,这位小姐可以转到其他病房了。”
另外一个病人安静地平躺在床上,除了眼睛随着我位置移动而从左边转到右边又从右边
转到左边,其他身体部位都是僵硬的。从头查到脚,只看到了一处后背瘀青。和病人聊
了聊,她一个人在家,不小心从拐角楼梯上摔下来,躺在地上有大约十个小时,直到邻
