我们这里没有neurology service，seizure的病人都是收到medicine，基本上都是药物
水平太低，load一下，monitor一下level, 调整一下剂量，最多的碰到3,4种药物一起
上才控制住的。检查最多也是EEG加MRI, 我从来没有refer过病人去做过VEEG ，
surgery或电极植入。感觉这些是那些非常顽固的病人才需要的吧。不知道疗效有多神
奇，我比较持保留态度。
另外楼主的弟弟大概20多吧，这种年龄段新发的seizure是不是secondary的可能性更大
呢？

Unless the seizure has lasted > 5 min you don't do anything except for
supportive care. FS is a very common condition in Pediatrics. Would suggest
you to search for 'febrile seizure' in PedsInReview and read the whole piece
to get a better idea.

case, Zofran usually does not cause EPS. It was a serotonin agent( which
very rare causes EPS). Reglan/compazine are D2 antagonist.
If you do not consider Seizure as a differential diagnosis, your
consultation is incomplete. D2 antagonist lowers seizure threshould and
ocular movement could be aura or non-convulsive event(not necessorily affect
MS). It is very possible when you just leave the OB/GYN after you tell

发帖数: 44 | 22 今天得知了step3的成绩，210+，心里的石头终于落下。终于轮到我给USMLE说再见了。
回顾从step1自己闭门造车考了只带一个9的分数， step2开始参加小组学习，做了所有
能做的题只为拿个99，虽胸有成竹且考试感觉很好，最终但又得到一个9的分数。虽然
这次成绩不高，依然很高兴，毕竟不用发愁如果没过的话住院医开始后没有时间复习该
咋办。过去的3年中得到太多人的帮助 (此处略去人名20个)，绝大多数甚至从未谋面。
今天发贴也是为了回报我们这个社区吧。
自我介绍：毕业>15年，220+/220+/1st/210+；
复习时间：6月，头4个月每天约2-3小时，周末由于陪孩子们玩（前两年欠的太多，你
懂得）及做点家务（再不做但心被老婆炒掉），反而复习不了多少；最后2个月每天4小
时，鼓励自己考了这个2年内再不用受这个罪。
复习资料：随大流，MTB 3, UW CCS, UW step3 Qbank, UW step2 Qbank;
模拟：考前2月，NBME 1 190+, 考前1周，NBME 2 220+;

was just discharged from hospital with a prescription of seizure meds. The
patient could not refill the meds because he has no transportation to

发帖数: 1 | 27 4/24/2017 费城考的， 考完非常焦虑， 担心过不了。Xingxing 老版主说只要好好准
备的， 都能过。后来成绩单下来， 还比及格线高很多。叶子版主让我写份考经，拖到
现在。
考点选择：Houston 应该是最IMG friendly. 一个在那考的朋友说考场里满场老印，旅
馆里也是。很多是直接从印度飞来考试的， 一个个土的掉渣。我们的英语虽然遣词造
句不如老印， 发音还是比他们好的。LA 据说 SP 非常习惯 Asian accent, 而且SP 年
轻男性多， 这也是优点。因为中年女性SP是最难对付的。费城SP 我考时是6 黑6 白 6
男 6 女。年龄分布老中青都有。一个在Atlanda 考的朋友也是这种分布。芝加哥考场
不清楚， 但没听到过恶评。
CS 考试准备：
Tutors: Ali and Marisa，早期练病例找的Ali, 后期练语言找的Marisa.
Partners: 3个， 2 Chinese, 还有一个美国长大，孟加拉读的医学院的小伙伴。练习
是很重要的，一不练嘴就生了。

实在是太厌倦考试了，整个护士读下来考试都考吐了，平时大大小小每门课至少四到五
次考试，那么多门课，加上两次HESI三小时，还有毕业前的evaluate来预测你nclex过
的概率。总之一句话，我考试已经考到吐了，所以完全没有准备考试，直接去考试了。
我75题蓝屏，几乎每三四道单选就有一个select all apply题，单选的基本都是
priority, 看哪个病人最先，没有什么概念题，或者直接问症状的。
75题覆盖了妇科，小孩，药物，med－surg 基本各个方面都涵盖了。
记得题目有，考了好几题关于读心电图，指出看哪个病人最危险，或者看了心电图后问
，这个病人问题出在哪里
药物有问副作用，两三题问药物作用是啥，比如一个是抗抑郁的药就是缓解抑郁，
seizure就是缓解seizure拉，提到的药物有lamictal， keppra
题目和护士学习考试时候基本都差不多，在这边读书的话，没啥可担心的。
重点就是知道病人有什么病，他的expected症状是啥，有没有newly developed的症状

I have seen a patient with persistent fever of unclear reasons. No infection
, antibiotics didn't help until finally sulfasalazine, the medication he was
taking for IBS was stopped.
After ICH patient may develop seizure, and maybe on seizure medications,
which may cause drug fever. I know it is rare, but just a thought.

out the possibilty he just started a new onset seizure and had "Sudden
Unexpected Death in Epilepsy".
Autopsy should help if he has died of respiratory failure or a congenital
cardiac disease. |

CVA with residual left side weakness,2nd was left CVA with transit aphasia
resolved, previous MRA negative) on ASA 81 and aggrenox bid now presented
with AMS. she was noted to be on the floor unresponsive in the morning and
she was last time seen well the night before. Pt regained conciouness in 45
min, eyes open, moving all extremities but aphasic. She is mildly lethargic,

I have seen seizures from meperidine PCA but never seen a single seizure
from a single or double dose.
75 mg of IV meperidine has an analgesic potency equal to that of 10 mg of IV
morphine or 100 microgram of IV fentanyl. Believe it or not, a lot of
patients claim only demerol is effective to their pain. I used Demerol 75
mg dose a lot but how many of you use morphine 10 mg often?

果只是focal seizure没有AMS, CT不是最好的initial imaging modality.
hypertensity in right frontal cortex没看到片片，不好下结论。hypertensive出血
一般是extending from basal ganglion to the frontal lobe with
intraventricular extension.
如果是focal right frontal cortex hyperdensity 的differential是tumor (
including ganglioglioma, PXA,or GBM), DNET, carvernoma. 或者老年人有没有
amyloid angiopathy hemorrhage. 或者是不是这人抽的时候摔到了，把脑袋给跌了，
focal cerebral contusion?

seems questionable: 1,431 patients were screened, and 939 were enrolled;
that is an unusually high enrollment rate of 66%. The study took place in 36

The patient is doing better. I would repeat an EEG. If no active seizures,
wean off phenobarbital. Keep keppra as is. In case of any recurrent
seizures, use dilantin not phenobarbital.
Don't forget to get physical therapist to see her.

to my experience, the majority of epileptic events can be well under control
by using anti epileptic drugs, no need for surgery or others, first, the
seizure type has been defined. Find a good hospital and get the best
