I am in a very similar situation -- I live in NYC, we have similar income
situations (both on individual & family level) and my son just turned a year
old.
My company is winding down, so I have two choices - 1) switch job, 2) stay
home collecting severance and unemployment for 99 weeks. However, I am
still working very hard to avoid getting into the second option.

首先，有个政策上的变化，属于医改的一部分。如果你的保险在2012年8月1日以后
renew，除非有特殊的情况（grandfather rule），那么你适用于新政策。否则适用于
旧政策。
旧政策是，如果有medical necessity，比如小孩在nicu妈妈没法喂奶，妈妈奶头塌陷
，等等情况，那么需要ob在孩子出生以后开一个prescription，上面有特殊的icd code
。然后，你把prescription拿给一个durable medical equipment provider，他们会提
供奶泵，并和保险公司结算。有些保险公司可能有金额cap，或者要求先rent再
purchase。
新政策是，奶泵是preventive care的一部分，必须cover。可以从in network durable
medical equipment provider那里买，可以从amazon或者target这样的地方买。区别
在意，前者是in-network，保险公司cover的比例高点(比如我的保险是90%和70%）。

我去年6月找她帮我办EB1A，实在很生气才在这里曝她，她在 Atlanta, GA

先讲讲她的无良“职业操守”， 头两个也许不算啥，您看完再说
1）。拉客的时候吹的天花乱坠，交了钱就不理了
2）。一般打电话，发email都不回，或1－2周才回
3）。12／15／2010 她收到补件通知，补交期限是1／26／2011。她直到1／25／2011才
通知我。还跟我说USCIS允许发
延期请求，她已经发了。
4）。然后又是找不到人，好容易通了一次电话，我也不奢望她帮我指导啥了（下面我
再说她的无能业务水平），我说我来准
备材料，你最后review一下吧，结果我把材料发给她以后从此玩消失。
5）。二月初我给USCIS打电话，他们说政策上是不允许延期请求的，我的申请快要被
abandon了。

道大家都用了那个比较有效？另外，全家申请的case，是不是主申请人申请了sp就
cover全家了？
