(5) Upper pole of kidney
Could not arise from kidney or spleen or pancreatic tail since there is
clear fat plane separating the mass from both organs.
I could not tell for sure if the mass is arising from the fundus of stomach
or adrenal gland.
(2) Contrast enhancement and internal feature of the mass.

报告上说，这个mass partially calcified，没和spleen相连，不知道是什么东西
好像CT上说contrast之后这个东西的成像没有明显变化，但是也不能完全排除是恶性的
。现在换了肿瘤科的医生，推荐做PET scan。这个医生也说不知道是什么，有可能是
previous infection留下的东西等等，只是猜测。
心中十分不安，所以上来问问。spleen内侧靠近心脏和adrena的位置应该是什么啊？我

发帖数: 6 | 7 LP身体一直很好，可昨晚右腹部突然开始疼痛，今早加剧,去了urgent care，大夫怀疑
是阑尾炎，说还是要去ER。在ER做了CT,结果发现不是阑尾炎，而是colon上长了个长条
的tumor.更奇怪的是spleen上也有个4cm*6cm的tumor.大夫说不知道两者是不是有联系
，也不确定是良性恶性的。必须动手术才能知道。现在他要明天就开刀，把脾和那段直
肠一起拿掉。现在我们很害怕，完全不知道怎么办，认为应该相信医生就决定开刀了。
可我还是不放心紧急求助一下。
1.会不会是误诊？我问他需不需要做个肠镜和ultrasound再确认下。大夫说CT结果不会
错。
2. 手术是不是最佳方案？感觉他一个人把这个就定了，也没有会诊什么的。把脾就这
么摘了还是很担心，会不会对以后有影响。
大家的任何意见我们感激不尽！！ |

Is it an accessory spleen? The hyperdensity could be bleeding instead of
calcification.
It could be symptomatic with torsion, bleeding, etc..

accessory spleen 也应在鉴别诊断中。你下一步应该做 Endoscopic Ultrasound-
Guided Fine-Needle Aspiration 简称 EUS-FNA. 大多数情况下，可以至少告诉你是良
性或恶性。但也可能没有得到enough material for diagnosis. So, find a good GI

因为记忆中自己没有受过严重的内伤，顶多是发烧咳嗽之类的
如果下一步PET scan还是不能确定，要做biopsy的话，会很危险吗？
如果不知道这个东西是什么，冒冒然去戳它感觉挺可怕的。。。

不太擅长码字，大概叙述一下情况，给大家一个借鉴：
猫猫好可怜，大概三周前发现猫猫不知为何偷吃猫砂盆里的猫砂，用的是Tidycat
Breeze，被现场捉到好几次，一开始没有引起重视，上网也查不出为何会有异食癖，只
是打算要换猫砂。过了些日子猫猫开始有些不舒服了，不是很明显，最先是呕吐了一两
次，以为是吐毛或吃了不新鲜的食物。可是，再过了几天，症状有加重的趋势，继续呕
吐，食欲减退，带到医院看了一次，查得比较简单，也没有查出问题，体温正常，无疼
痛，只是打了止吐针（没经验，这时候如果把猫保险买上就好了，不怕一万就怕万一）。
带回家后，猫猫病情有继续加重的的趋势，开始吐黄绿水，什么都不吃，只能喝些许水
，无腹泻。带到另外一家医院，拍了X光片，显示有脾肿大，同时肠子里大概有几十粒
偷吃的猫砂存留（已经有好几天没有排便了），医生一开始认为是spleen twist，要马
上动手术切除脾脏才行。在花了$60咨询了X片specialist之后，认为spleen twist的可

malformation) and primary splenic lymphoma.

此小鼠一岁时，spleen和liver出现tumor,明显比WT的大许多，见照片。
而且腿骨非常脆，一岁甚至8个月KO小鼠的腿骨就和2岁的WT的腿骨差不多，甚至更脆。
骨髓细胞中CD11b+细胞达到90%，有粒细胞白血病的征兆，正常的一岁的WT小鼠大约60-
70%。

monocyte是macrophage的precursor细胞~ 在circulation里面应该是木有什么
antipathogenic function的！
对于sepesis这种情况，我也不是特别清楚具体自身免疫系统有多强大的能力来控制
sepesis. 理论上来说，你可以理解脾脏是一个大血泡，circulation的血液都会循环经
过spleen~ spleen是人体最大的淋巴器官，因为，他可以filter血液的细菌啊什么的！
当血液不干净的时候，脾脏的pulp有一层macrophage lining，那些macrophage可以把
细菌吞了。
这些只是理论，实践中，一旦病人有sepesis，自身免疫系统木有什么能力来对付! 如
果医疗不介入，患者多半是个死！白求恩不就是得败血症死的么...
绝大多数感染是不侵犯循环系统的，只有infection progression到很严重的时候才会
出现bacteremia. |

被曝光。专家对这一研究的评价不高，认为非常全面的描述性研究，如果结果确定，可

前兩天，一位在美國的朋友打電話給我，說她的孩子發燒，已經好多天了，美國的醫生
檢查，說內臟沒有什麽問題，可就是高燒不退，美國的醫生開了退燒藥，可是只能降溫
幾個小時，然後就還是高燒，再問醫生，醫生也無奈，最後她沒有辦法，給我打了電話。
我問了問美國當地的天氣，覺得應該不是天氣的問題，於是又問：小朋友在發燒之前，
什麽東西吃多了嗎？這位朋友回想以後回答：確實幾個華人家庭出去聚餐，孩子吃了很
多東西。
於是，我分析這可能是食積導致的，於是問她之給孩子吃了什麽藥，她說在西藥無效以
後，給孩子去藥店買了中成藥柴胡桂枝顆粒什麽的。於是，我告訴她這個藥可以繼續吃
，同時，囑咐她只需要多買焦三仙各六克、炒雞內金六克，給孩子熬水喝。
結果，第二天晚上，我收到郵件，她說“中醫真神奇啊！孩子的燒已經退了。”

suggestive of sub capsular bleeds, presumably at bleeds with re-bleeds. If
the patient does not have trauma, then coagulopathy with meds or neoplasms
should be considered. I don't see the typical wedge-shaped hypo attenuation
seen with arterial infarct, But venous infarct can't be excluded. Although
post-infarct hemorrhage is not very commonly seen.
