儿医说要做72小时的尿培养，这期间要注射抗生素保护宝宝的肾不被感染。大概是这个

本人45岁，39岁发现子宫肌瘤，当时5cm。因为当时刚做完剖腹产，不想再做手术，所
以保守治疗。吃了一段时间低剂量避孕药，月经正常后没有再吃。肌瘤慢慢长大。3年
前7cm。仍然不想手术。这两年长的比较快，夜尿频繁，于是决定手术。决定切除肌瘤
，但是如果有任何异常就子宫全切。今年8月3日做的手术。结果腹腔打开，医生发现肌
瘤非常大，整个子宫就露了一个小头，其他部分包括宫颈都在肌瘤下面。糟糕的是输尿
管因肌瘤压迫distorted. 妇科医生叫来了肿瘤医生，也叫了urologist， 但是
urologist在做手术，所以没来。妇科医生和肿瘤医生商量后决定一定要urologist在场
才切除子宫，所以把腹腔缝合了。妇科医生告诉我他以前从来没有遇到过这样的情况的
。医生告诉我下次手术时urologist要insert stents 进输尿管，为了避免损伤输尿管
。对了，肌瘤是良性的。
我的问题是这种情况是不是一定要urologist在场？这个妇科医生的决定是正确的吗？

篇letter to editors， 从来没有peer revie w paper 说肖有问题。
肖和peters倒是分别发文，还写书。还有cme course。
再去上网看许多美国病人的评价，有许多spina bifida patient supporting groups
支持clinical trials。去那些美国人网站去看手术后病人怎么评价方粪spam他们网站。
许多方粪拿着这篇2009的综述，断章取义或英文太差，或别有用心。把一篇比较中性的
文章说成停用肖手术。
贴下文，是为the silenced majority，没有对方粪教育的义务。

找ob——不是refer to某个specific的ob，而是作为一种condition - pregnancy。
在我孕初期前两次血检中，都发现了small trace of blood。我ob就referred我去见
urologist。 urologist ran了一堆检查。包括全面的内脏ultrasound。结论是没什么
问题。。。。。。
今天刚刚收到bill，保险公司只cover我60%的expenses，note说"Non-referral
penalty has been applied." 这样我自己就要缴将近700块。 这对于一个full-time
student实在是笔大开销。
打电话给保险公司，保险公司说urologist那边sent给他们的code看不出是pregnancy
related。所以即使是ob referred去的他们也爱莫能助。

不化验还不知道。只是影响了生育：精液不液化，影响精子质量。
顺便说一下这里的urologist。我在这里检验过一次精液，
就是找urologist看的，说我没问题。回国再查了两次，
都有问题。于是回美国后看看以前美国的检验单，发现里面
的检验项目没有国内的详细。居然没有液化的项目。
urologist这里也很多，就是不知道看谁好。
这种难治的病，我相信很多医生都治不好自己（如果他有这病的话）。
跟你说一个好笑的。我在国内看过一个牛气的很的老中医，
后来才知道他自己不久前急性前列腺炎去过医院。呵呵。 |

发帖数: 254 | 13 我们一出生就看的urologist，做了肾功能测试，和VCUG，2个月又做过一次B超，结果
是肾功能基本正常，肾积水和输尿管扩张在好转。Urologist说如果宝宝第一次的B超和
2个月做的一样的结果，他不会认为有风险而安排其他检查的。所以我们就理解应该没
有大问题，只是泌尿系统发育还不完善。
Pediatrician今天也说，做完尿培养回联系我们的urologist，看看是否需要有下一步
动作。
看起来婴儿的尿路感染还是挺严重的，可是怎么造成的呢？洗澡？便便的时候弄脏了？
本来就担心他泌尿系统的问题，还偏偏又感染。 |

发帖数: 4494 | 14 我有过保险公司拒付的情况。最后申诉成功了。
我觉得你大概可以这样做：
1）问问保险公司，你现在是不是只能申诉。如果只能申诉，就找urologist或者OB要
pregnancy related的证据。最好是让他们帮你提交申诉，因为他们更专业。我当时就
是让医院帮我申诉的，材料什么的都是他们一手准备。
2）一般保险公司都可以申诉三次。而且第一次成功率很低。所以如果你第一次被拒，
你要坚持，至少要把这次三次机会都用完。
3）如果三次都失败，你再在网上找找你们州有一些独立审核保险公司决定的那种机构
。跟他们再申诉一次。
4）如果还是失败，你就联系urologist，问他可不可以减少帐单的费用。当时我的医院
主动说如果失败，他们会re-bill我。
另外，申诉是一个很长的过程。在这个过程中，如果帐单的deadline到了，你就打电话
给urologist，让他延长付款的时间。千万不要自己先把帐单付了。基本上你自己掏钱
后，保险公司是再也不会把钱给你了。

我早晨又打了电话给保险公司。他们商量了一下，说没有maternity-related code就没有吧，只要让urologist那边提供一个medical note，指明这是maternity-related，我们就给你cover...
高兴还没到30分钟，打电话给urologist那边，前台一转昨天agreeable的口吻，说不是因为没看出什么问题吗，所以不知道是"maternity-related or not"。不能给出这样的medical note...
所以我现在的问题不是保险公司不付，而是urologist这儿别扭起来了。。。 。。。
这该怎么办呢，真是欲哭无泪阿。 |

结果没做好 皮又粘回去了
并且挺严重 几乎要把尿道口都遮住了
现在尿是没问题 但恐怕过不了多久还是会有影响
找了当地最好的urologist 说是要重做的
但是现在娃已两个月了 现在做必须要全麻
预约了三月初手术 那时候娃才3个月
医生说全麻现在是挺小 最好等到六个月心肺更成熟
现在做呢 要住院观察的
但是娃的情况估计也等不了那么久
最后说等三月如果尿没问题 就把手术延后
我们一方面怕拖久了会感染或影响penis发育
又怕全麻影响大方向的发育 全麻太未知了
即使六个月做 我也觉得全麻很危险 不知道会不会影响大脑发育
唉 不知道版上有没娃小时候做过要全麻的手术的
想听点正面的例子
郁闷死了 割包皮那么小的手术 弄成这样

Per AAP guideline "Urinalysis cannot substitute for urine culture to document the presence of UTI but needs to be used in conjunction with culture."
Gold standard is urine catherization.
New UTI guideline is that if this is this child's first episode of UTI and

proteinuria as it's more of a medical issue than surgical issue, urologist

自己吃了一颗抗
生素，第二天去看了医生，开了抗生素，症状没有了。但是若干天后医生打电话给我说
当时的尿检没有
查出细菌，让我再去查一次，发现还是有镜下血尿和少量蛋白，当时已经吃完五天的抗
生素了。后来几
个月里反复查了几次尿液，都是镜下有红血球和少量蛋白，做过肾脏和膀胱B超，结果
正常。查过血，肾
功能也正常。看了urologist，查了细胞学，没有恶性细胞，但是urinalysis 还是显示
有red
blood cell （26 hpf)已经trace amount of protien。urologist让我看
nephrologist，但是要8月底才能约到。
平时没有什么不舒服，就是最近很眼圈比较大，但是本人一直比较容易有黑眼圈，也没
有明显水肿，除
了早晨起床有时脸有一点点肿，但是以前也偶尔会这样。比较容易得UTI（现在都不知
道以前那些是不是
真的UTI了）。
因为一直有镜下血尿和蛋白，所有有点担心是肾炎，好像有些肾炎（比如igA）很难治
的样子，比较害

urologist 说他更本不认为我们家宝得了UTI(没发烧，没症状，no white or red
blood cell in urine). 他一般不相信Urine sample collected by bag, easily
contaminated by bacteria trapped in foreskin.

UTI，基本没法避免。 听听urologist的意见。还有一定要看pedatric urologist. 一
定要把医生给的antibiotics吃完to prevent pyelonephritis. |

clean soon (next day, or so), it is may just a light damage, if there is
urine retention ( urine can not pass out), it needs immediate medical
attention. In general, small damage will not be any harm for long run, for
sometimes due to scar tissue, that can cause the narrowing of urethra, you
need to consult a urologist to make sure that will not happen, and check

美国佛罗里达州的全儿童医院 (ACH) 开始了一项临床研究来对一个治疗脊柱裂和脊髓
损伤后神经源性膀胱有着很大成功希望的的外科手术进行评估。

美国佛罗里达州的全儿童医院 (ACH) 开始了一项临床研究来对一个治疗脊柱裂和脊髓
损伤后神经源性膀胱有着很大成功希望的的外科手术进行评估。
这个开始于3月份由医学伦理委员会通过历时3年的双盲试验将包括脊柱裂或脊髓损伤的
患儿。在8个儿童中有7个患先天性的脊柱裂并正在接受脊髓栓系松解术；有一半的病人
同时进行了肖氏手术并正由全儿童医院的医生进行评估。第8个病人的膀胱功能障碍是
因为在新生儿阶段手术摘除一个脊髓肿瘤造成的。这个患儿只接受了肖氏手术。
肖氏手术是由中国同济医学院的协和医院泌尿科主任肖传国医生发明的。 这个由美国

Basically, it could be anything which causes damage of small blood vessels.
By statistics, probably goes to BPH. By grave outcomes, it goes to tumor.

看这意思是说urologist的用量要上来了? 那可不得了. 一个quarter卖好了的话, 上$
30分分钟的事儿, 还不要说big pharm并购的可能性了.

urologists from US. Some of them are from decent institutions in cluding
Harvard, Bayler, U Toronto, Columbia, Stanford, Emory, U Tuebingen. A few of
them hold quite high academic ranks, including chair, director, or chief.

itself. The inquiry form was designed by whom? May be Fang. You may find out
who started that, performed that, and analyzed the so-called data. By the
way, I personally would not call it "original data" since the whole thing is
not done in a scientific way at all.
Xun Zheng also suggested the use of medical instruments rather than simply
response from patients. This notion was also mentioned by some urologists?
