发帖数: 44 | 23 今天得知了step3的成绩，210+，心里的石头终于落下。终于轮到我给USMLE说再见了。
回顾从step1自己闭门造车考了只带一个9的分数， step2开始参加小组学习，做了所有
能做的题只为拿个99，虽胸有成竹且考试感觉很好，最终但又得到一个9的分数。虽然
这次成绩不高，依然很高兴，毕竟不用发愁如果没过的话住院医开始后没有时间复习该
咋办。过去的3年中得到太多人的帮助 (此处略去人名20个)，绝大多数甚至从未谋面。
今天发贴也是为了回报我们这个社区吧。
自我介绍：毕业>15年，220+/220+/1st/210+；
复习时间：6月，头4个月每天约2-3小时，周末由于陪孩子们玩（前两年欠的太多，你
懂得）及做点家务（再不做但心被老婆炒掉），反而复习不了多少；最后2个月每天4小
时，鼓励自己考了这个2年内再不用受这个罪。
复习资料：随大流，MTB 3, UW CCS, UW step3 Qbank, UW step2 Qbank;
模拟：考前2月，NBME 1 190+, 考前1周，NBME 2 220+;

不需要完美的英语，12个病人，10-12分钟交流，内容很熟悉，目的：得到你想要的信
息，诊断，病人想说的信息问出来；需要病人听得懂（简单的语言，语速不要太快，模
板，避免she/he, hands, tense这种错误，不用复杂的单词，地道，语调，语气：同情
等，不要像背书，脱口而出，跟情景语气等符合），病人的话听得懂（100%注意力，同
时要做笔记），避免彼此的重复，注意病人是否听着费力，不要经常用一种开头或
response，但是仍然要简单
CIS: 听懂病人情绪变化，病人暗示了，一定要接上给response, non-verbal, 打招呼
，眼神，坐得比病人低（病人心理出于劣势，仰视病人，不要站着- 感觉rush，没有足
够的关注），关心，身体前倾。情绪变化很subtle，要察觉到，安抚等。

一般的CNA，再怎么偷懒，不会不查VITALS的，除非是极品，你运气好碰上了。
我干了这么多年，多家医院，好的坏的都见过，不过不查SHIFT‘VITALS的还没见过。
如果她很忙，等一会拿也很正常。就是等一小时也没什么。病人死不了，死了也不是你
的责任。
刚开始工作，还是要夹着尾巴做人，就连王医生，第一年都还好，人家也给他续了第二
年的约。
我也经常和同事吵，不过第一年还是蛮老实的。
其实说句不该说的话，干这行，病人什么的除非是CALL CODE，其他没什么不能拖得，
还是和同事关系搞好最重要。

个box不大，可以放兜里，tele科室的gown的前面都有个兜。然后我们就可以在station
看到他们的心电图了，病人可以随便走动。但是在房间里无法看到，除非连到特殊的
测vital sign 的机器叫做dash or telemon,这些机器有wheel，跟普通的测vital sign
的机器大小差不多，但可以连到tele box上，或者tele box可以插到这样的机器里，这
样我们就可以在房间里看到心电图了。
如果是icu的话，他们不用那个box，而是直接连到房间里的monitor。即使这样，
station还是有tele屏幕，这样在station也可以看到病人的心电图。 |

TIME的工作，title是“student nurse apprentice”。
Precepter带了我3天4个shift，今天让我独立工作，给我6个病人。早上去了还要开个
会，所以搞得我手忙脚乱，8点要求完成的vital signs根本就来不及。
有个老先生是我的病人，我一进去就感觉他情绪不好，勉强配合我把vital做完，说要
起来，还要换尿片，要清洗。我第一次护理他，不熟悉该怎么样，就准备先帮他起来，
可是他马上就说：你要喊人来帮你。然后就不耐烦地摇头。问我以前有没有做过，我很
老实的告诉他我是新人。我之所以想自己弄他起来，因为前面的tech跟我report说他是
one person assist。然后我就喊人来帮忙了，一直把他弄干净了。谁知道等我离开他
房间没多一会，护士就来说他不想让我进他的房间了。于是让我跟另一个tech换了病人
，他们还安慰我说，不要往心里去，这个老头就是很怪很难缠的。
这8个小时我一直脚没停,腰上的电话不停地响,不是这个喊就是那个叫，其他的病人都

可是人家就是不应话啊。
每天量完早上的vital，就下楼买早餐了，然后有人叫帮忙，就说自己在吃饭。中午量
完vital，不打招呼就不见掉了，我在对讲机里面喊几遍，不应声的。有时侯有total
care的病人，真是没办法，最后还得别的rn来帮忙。
再有的时侯，我在对讲机里让他们去做什么，也不答话，我很招人烦的一直不停的说，
后来跑去看，他们已经做了。有时侯没有回答我就直接打charge的手机，让她在对讲机
里喊他们出来帮忙。
我自己都觉得自己很招人烦了，我已经跟charge说了这个情况，charge 也在早会上说
了这个情况，可是还是老样子。
director在度假，等他回来再反应情况。最坏的打算是申请别的科室。我自己的工作态
度是情情肯肯，我真受不了这样的工作态度。ld让我问问别的同事是不是有相同的感受
，我打算有空私下问一下同事，问一下相同的情况下，他们是怎么应对的。
这是我在美国的第一份工作，不太清楚这里的水土。真心不愿跟同事的关系搞得这么僵。

杂的，我跟病人打电话，从来不online的。门诊的时候我事先跟病人说，等到lab都来
全了，我会打电话跟你go over，有时候还需要clinic visit to discuss labs。肯定
不会online message的。
一般message都是血糖低啦，高啦，有什么药物反应了，或者有个什么问题，这些都很
快的。
我也是刚开始工作几个月，还在摸索中，group的好处就是可以看其他医生怎么做，吸取个人的经验教训，特别是怎么bill，怎么screen 病人之类的，老医生都很有一套。
Epic另一个好处就是强大的模板功能，我现在notes都几分钟就写好了，new patient几个常见病去都各有一个模板，根据模板问问题既高效又省事，而且最要紧的是不会遗漏，过去史家族史药物之类的都有自动link，vitals每次都自动更新，还可以同时看过去几次的vitals, labs。另外用模板事先根据medicare billing的要求设计好ROS, physical exam的数目，才能准确code level... 阅读全帖 |

dictations and notes ready when the visits are done.
But allscript has to have someone type in the phone messages from the
patient. Epic sounds like skipping this step and directly communicating with
physicians through the web.

看了一篇的文章
有解释为啥Shell Rotella T、Mobil Delvac这些heavy duty diesel oil的Used oil
analysis结果都很突出的原因，普通engine oil是S certification，例如SJ、SM。商
用truck engine oil是C certification， 目前是CI、CJ。C的additive package比S的
牛x很多，但有可能会不兼容乘用车上的catalytic converter。
我的G35目前就在用Shell Rotella T，摩托车下次换油也准备用这个。mobil的
synthetic motorcycle oil 1 quart 12刀，受不了。
