"BACHELOR OR MEDCINE" in the application form, would the liscense be
different than if you put "Docotor degree of medicine"?

only difference is the "MD" or "BM" title behind your name........
I have or had 5 state licenses, all I filled in "MD" when I applied for, no ANY one challenged or questioned this title.
as long as u have US formal credential compony evaluated and certified your education profile and credentials, and issued a certificate letter or document, u have the right to use "MD" or "BM", depending on what you yourself want, No one--I mean Americans--NO one cases!
surprized thing is: ONLY oversea Chinese guys CARE!!! they like to "Da3 jia3", hohoho......
