is as following:
BUY: At least $40.00 of Enfamil Premium, Enfamil Gentlease, Enfamil AR or
Nutramigen with Enflora LGG (you may mix and match)
2. SEND: Original cash register receipt(all purchases must be on one receipt
), UPC symbols from products purchased and the rebate certificate.
3. MAIL: By 6/30/2010. Limit one refund per household or address.

I am looking for the $15 Enfamil rebate form requiring"buy at least $40 of
Enfamil lipil"
They are different rebate programs. In sum, one requires purchasing 2 large
cans, the other requires purchasing at least $40 of Enfamil.
