么回答是，然后填谁资助。说明里面是说并且，不是或者。
If you or any accompanying family members are currently in B-2 (visitor for
pleasure) status and seeking to change status to either F-1 (academic
student), M-1 (vocational student) or J-1 (exchange visitor), select “Yes”
to indicate that you are seeking a change in status.
还有好像没看见哪里可以填以前的入境记录，说明里面：
If the beneficiary has been in the United States previously, in the
additional space provided enter the number of times he or she entered the U.
S., the location where he or she entered, and the beneficiary’s status each
