birthdays, parents' names, birth places;
2) marriage certificate (notorized chinese/english copy), date/place.
if there's any previous divorce etc., need to have certificate for those too;
3) your birth certificate (notorized chinese/english copy), same as 1, if
you don't have it.

only did aids test and chest x-ray on me, and gave me a shot for one
vaccination that just expired.
