标 题: 请帮忙！！再生障碍性贫血（Aplastic Anemia）

我是学生，偶然因一次腹痛，医生给order
血常规（CBC）. 发现全血细胞减少（红细胞
，白细胞，血小板均减少）最近一次检查white: 1.4k/cu mm（4.4-11.0）, red:
2.64m/cu mm(3.65-5.10), platelet: 37k/cu mm(150-400).
骨髓穿刺和活检发现只有5%-10%。
医生初步诊断为再障。建议作骨髓移植。但是费用太高而且不一定有配型。
这版上有没有人曾经见过此类病人或者听说过。他们的治疗方法？

her 2nd trimeter. Has been taking prenatal vitamin through the pregancy (
including 1mg folate acid) and extra oral VB12 3000mcg daily for 2 months.
Hb is still around 11g/dL and MCV around 102. serum VB12 level is 718pg/mL (
211~911).
Methylmalonic acid serum level is 113nmol/L (87~318), Homocysteine level is
6.6(5.5~15).

甲状腺功能减退简称甲减常伴有贫血，其发生率约占甲减患者的1/3—1/2，多数为轻、
中度正常血红蛋白或低血红蛋白小红细胞型贫血，少数为恶性贫血(大细胞性贫血)。那
么甲减性贫血的发病机制是什么呢？
(1)甲状腺激素有促进红细胞生成的功能，当甲减时甲状腺分泌减少，骨髓因缺乏
甲状腺激素而特异性受抑，呈增生不良，减少了红细胞生成素的促进物质之一，使红细
胞生成素减少而致贫血。
(2)甲减病人厌食，胃酸缺乏，约半数病例可继发有吸收减少，蛋白质与维生素B12吸收
减少，以及其它营养物质摄入减少，引起营养不良性贫血，①甲减时血容量减少，约有
32％—84％的病例血红蛋白可低至80—90g/L；以及月经过多大量失血也可导致贫血；
②某些病人因胃内因子缺乏使维生素B12吸收减少，造成大细胞性贫血，另甲减病人中
抗甲状腺抗体检出率相当高，因此认为发生大细胞性贫血可能与自身免疫对胃粘膜的损

昨天，这些人折腾了一天，白天，晚上皮啦啦，也不知道谁把谁杀了，谁把谁带走了。
反正我先生和孩子被绑架了，就不知道是不是已经遇害了。我本来是打算睡觉的，但他
们头上，地下装电磁波攻击器，让我几乎窒息。虽然我没倒下，但其实他们在我身上扎
的毒针已经蔓延，我能感觉到我的呼吸有窒息的感觉。我想我对他们已经非常仁慈了，
但他们不依不饶地攻击我，欲置我于死地，让我非常生气，特别是肆意伤害我家里的人
，绑架我先生和孩子，而且有许许多多无辜的生命毁于他们之手，有必要把他们曝光。
当然我还没分析这里边是那几个人具体攻击我，伤害我的，但他们显然都是一伙的。鉴
于他们伤害我的头部，胸部 和胃部，我就把这几部分的医生都曝光，受害者慢慢去调

The diagnosis is Vitamin B12 deficiency anemia, not Pernicious anemia.
Pernicious anemia is an autoimmune disease, seen more in younger age.
Vitamin B12 deficiency anemia includes Pernicious anemia, but also includes
other causes. In this case, old age is the clue.
In practice, if you check medicare patient routinely for Vit B12 level, you
will be surprised to see how common people have B12 deficiency.

但hemoglobin的突变还可以导致其他好多种疾病如thalassemia，而anemia就是更大的
一个疾病名称。相对于anemia而言，cancer就是所有表现为失控生长性疾病的总称，但
基因突变复杂的多，（单基因vs多基因），所以简单的在基因水平上来说就是癌症是无
数种失控生长性疾病的总称，而每一种就是一个不同的基因突变的组合，所以谈所谓的
“突破”，如果是针对cancer这个总称，我觉得没意义，应该细化到每一个单一癌症种
类，这么说才有意义。 譬如说CML leukemia就是一种相对单一的癌症，BCR-ABL基因致
病，所以CML现在由于gleevec的发明，基本是一个可以控制的癌症了。同样还有M3型
AML。
将来如果“癌症”可以被所谓的“攻克”（非专业说法），必然是各个击破 （就像现
在治疗任何一种单一的疾病如sickle cell anemia，而不太可能是突然发现一个万能的
方子，可以治愈所有癌症。

5-FU等影响DNA合成的药物，理论上都能够导致megaloblastic macrocytic anemia。但
是临床上可能很罕见。不是因为骨髓抑制不够严重，恰恰相反，是产生了严重的骨髓抑
制：The cells simply die！这些化疗药会kill blasts，所以三系细胞都会急性下降
。要产生megaloblastic macrocytic anemia，这些megaloblasts必须survive，形成
macrocytic RBC。可以想象，如果长期服用极低剂量的5-FU，就会产生megaloblastic
macrocytic anemia。
5-FU不会通过影响细胞膜或者reticulocyte而引起nonmegaloblastic macrocytic
anemia。这些化疗药的靶点就是DNA而不是细胞膜，而且reticulocyte也没有DNA。 |

will also have tachycardia and other symptoms by that point.

，难以治疗，死亡率偏高。
深圳华大的初步分析结果显示，这次肆虐欧洲的血清型0104大肠杆菌，该病菌带有
新型特异基因，与2002年从中非共和国艾滋病患者腹泻标本中分离的肠聚集性大肠杆菌

anemia。只不过叶酸和B12缺乏是最常见的原因。DNA合成障碍会导致细胞核成熟受阻，
但同时RNA、细胞浆物质的合成会继续，细胞变大，形成megaloblast。这应该包括所有
lineage的blast。Hypersegmented neutrophils by definition并不是megaloblast，
跟巨大的RBC一样只是megaloblast的产物。
Nonmegaloblastic macrocytic anemia不是DNA合成受阻引起的，没有megaloblast形成
。通常由酒精（直接毒性）、红细胞膜障碍（肝病）、reticulocytosis（hemolysis）

后开始自己买菜做饭.感觉除了上班其余时间都用来吃饭睡觉了
尤其周末花好多时间在吃上.
我第一次发现macrocytic anemia的时候就要求IM injection了,OB不同意连口服都不给
开,我只好自己去pharmacy买oral supplement. 没想到吃了2个月查了3次还是
macrocytic anemia,还是不同的lab查的.但是我半年前的一次CBC确实是WNL的.
我除了morning sickness which comes with pregnancy外没有其他GI symptoms, 没有hypothyroidism symptoms, Live function normal, no past medical history,no family history, not on any other meds except prenatal vitamin(contain 1mg folic acid per tablet) and oral V12. 想不出wh... 阅读全帖 |

Unfortunately, at this encounter, PCP did not really ask about diet like
Rubyheart did. PCP's plan included:
1) started pt with metformin 500mg, and told pt to cut down carbohydrate
intake (without asking pt first what pt was eating),
2) ordered vit B12, folic acid, blood smear tests, which all came back as
normal.
3) RTC in some months.

102.今天门诊的internist 打我电话让我去查VB 12 level and methylmalonic acid
level. 说如果低去用IM injection of VB12.
我现在很不高兴,觉得我的OB怎么没有on top of everythings 呢? 前不久竟然还让我
用了categorical C 的pain killer, while there's categorical B pain killer

（140-420是正常值，50以下是危险警戒，我现在30，而且每周往下降10个单位。呜呜
呜）
怀疑我是ITP甚至Aplastic Anemia。
今天正式开始treatment，吃激素升血小板，如果没效果就得做bone marrow确诊
Aplastic Anemia，心情down到底。

这么一折腾，我居然忘记问问医生是不是一定要去配铁剂吃了；所以这里请教大家，平
时如果多吃点牛肉啊，菠菜啥的是不是更好点？
