NYHQ做OB还有一些问题，所以希望写下自己的经历，对后来人有所帮助。
首先非常感谢ACAP的帮助，让我们在NYHQ能一直有机会。我来到NYHQ医院做了一个
月的OB. 虽然知道这个机会来之不易而且还有很多XDJM在报名排队，我在来之前还是有
些犹豫。因为可以在我所在的城市里的教学医院做OB，为什么要离开家人和工作，来到
一个社区医院做呢？其实来到第一天的见闻就打消了我所有的顾虑，一个月确实挺值的
，只是没有机会再做长一些时间。
NYHQ位于法拉盛区（Flushing），医院大厅标注着英文、中文、韩文、俄文等等，
足可以看出医院的多元化（diversity）。第一天先参加orientation和infectious
control training， 然后去街对面GME办公室递交application form等paperwork，再
回到医院办理ID卡，到五楼找chief resident报道。一批observer一般有四人， 两周

It was an Interventional Radiologist who attempted to place a chest tube
into pt's left chest cavity but failed to do so and poked pt's spleen by
accident during the procedure. I heard the guy who did it was a surgeon, but
he turns out to be an IR MD. After pt started to bleed, he took him to IR
and did an embolization to the bleeding vessel.
This MD is very honest. He explained what had happened on that pt and

I wouldn't recommend aspirin 325 for long term if she is on pradaxa or
warfarin.
agree with fiorio for using CHADS2 score to determine if long term
anticoagulant is necessary. Yes, since she had a TIA already, if Afib
persists, she definitely will need a anticoagulant.
What other medication is she taking?
If she goes back to China, I think warfarin will be a better choice, at

1. There is no sufficient evidence showinh full dose aspirin is superior for
secondary stroke prevention compares with aspirin 81 mg. Your physician may be right base on your patient's risk factors and other conditions, I don't
know enough background, so I am not judging her decision.

t think so. They work differently and are used for different indications
We don't want patients to stay away from spinach or green leafy vegetable,
we just want them to be consistent.that being said, if they have been eating
green leafy vegetable everyday, we want them to keep doing it.
I actually hate when patients tell me that they are not allowed to eat green
leafy vegetables anymore. This is a myth.

2. If intra-abdominal infection is suspected and ABX is needed, vancomycin

我有几个问题：
1.我们讲的是tips是一个bridge to liver transplant。做了这个以后就没有liver
funx。而这个病人做tips已经是3年前了。是不是可以用propanolol+isosorbide （2nd
prevention of bleeding)后考虑liver transplant。
2.病人入院之前的folic acid和iron pill具体的indication是什么啊。这个case就是
要找出drug therapy problem。这是不是unnecessary medication呢·
3.因为pt已经dehydration，是不是要hold furosemide？那相应的HTX应该如何控制。
要不要增加lisinopril的dose。
4.我的理解病人之前take kcl是因为同时on furosemide。如果order lab之后，k
level在正常，是不是就可以也hold kcl呢
5.因为病人有pud 引起的bleeding，是不是也要加上在discharge的时候put pt on

Axillofemoral bypass surgery is not a procedure with minimum bleeding risk.
Actually, all cardiovascular surgeries have high bleeding risk.
Prior surgery: aspirin should be held for 5-7 days; It's appropriate to hold
warfarin and bridge it with parental short acting anticoagulant. warfarin
only needs to be held for 5 days. Bridging by enoxaparin is a good choice.

The transplant kidney I did in the early afternoon bled a little on the
first pass, I used an 18 gauge Temno needle, but path on site wanted another
specimen. I took another one and dark blood was just gushing out from the
trocar needle, and I had to put gelfoam. Luckily bleeding stopped and post
procedure images showed no bleed. Wish path could be satisfied with one 18
gauge
core specimen. :p |

到sears 买了一套叫“bolt out”的工具，正好sale,原价20，现价10。 很是好用，几
下就把exhaust manifold连header pipe处那个断了半截的螺杆给拧了出来。
至于那个锈死的tie rod end，火攻见效不是很明显。后来把那个locking nut给拧到头
，喷了点liquid wrench，等我把那半截螺杆取出后，再对付它，终于拧了出来。
安装所有的东西回去。 本来还想bleed brake的，一则天色已晚，二则后闸的bleeding
screw 已经锈的比较严重，就把brake reservior 的刹车液吸出，换上新的了事，反
正绝大多数的刹车液都换掉了。
方向盘已经不在中间了，alignment 是必须作的了。
照了几张照片，都超过1MB. 今天太累了，等有空了把照片上来看看。 |

刹车液漏光了，因为有一个管子漏了，brake fluid master cylinder reservoir中的
液体也漏光了。
请问这样的情况下，采用brake fluid bleeding的方法可以吗？谢谢！
先打开一点bleed valve，之后踩下刹车，保持住，打开bleeder valve，让刹车液和空
气排出来，再关闭valve，松刹车之后重复踩刹车，开valve的过程，松刹车，直到没有
空气出来。
这样是不是很耗时。谢谢！

reason might be air in the caliper, the instruction suggested to tap the
caliper for first use when bleed, which I did few times on the driver side
but none on passenger side. will see how people response and might bleed it
again tonight. |

吃药，打针。ANTI BAOTICS 和ANTI FLAMMTORY 又MISS 了几次，昨晚ASPIRIN 也忘了
，豆浆也只喝了大半杯，今天上班发现我放在冰箱里的豆浆都没多少了，我也没说今早
带一瓶来（想起来了却想应该还有，就懒了一下），都 好多天没吃FOLLIC ACID了。今
天护士打电话来问我有无BLEED 和BLOATED。我说没有BLEEDING，但是有BLOATED，她说
每天要喝GATORADE，我说我没有收到这个INSTRUCTION啊，所以喝了几口没用就没喝的
。她说PRE－RETRIEVAL INSTRUCTION上有。我怎么又没看见？她现在让我每天喝６４OZ
，如果到时候还是BLOATED，就不给TRANSFER。
我没有全心全意地投入到这个伟大的事业中去，到时候宝宝们嫌弃我这个妈妈，不想在
我肚子里扎根，我也没法怪他们。

A：This is called “break through bleeding”; it is a common side effect of
the birth control pills. This will not effect your treatment cycle. If you
