各位看看有无更好的办法，非常感谢！！
病情： 女孩，1岁9个月时发现患有右眼先天性白内障（congenital cataract）
1岁10个月进行手术植入人工晶体。之后，右眼患有近视性的形觉剥夺性弱势（form
deprivation amblyopia）,右眼视力4.0左右，散光度数较高。
目前国内治疗方案：1）配镜
2）遮盖疗法：每天遮盖健康的左眼8小时，一周遮盖6天。
3）精细目训练：每天遮盖健康的左眼后穿针60根。
4) 红光治疗仪：治疗过一个月，但孩子视力有下降趋势，故停止
了。
5) 口服药物：一直在吃乐盯，每天一粒。
治疗时间：12个月
治疗效果：右眼视力无明显提高。左眼视力有近视趋势。目前小孩是3周岁。
恳请大家看看，先天性白内障导致的形觉剥夺性弱势在美国有无更先进的疗法。孩子一

deprivation amblyopia is considered the most severe form of amblyopia.
Usually here in the states, congenital cataract has to be taken out between
1-2 months after birth, followed by optical correction (usually contact lens
) and extensive patching. Even with all this, visual recovery prognosis is
probably around 20/40 to 20/50. If the cataract remains there after 6

病人，男，目前6month, 33 weeks 剖宫产，出生时右侧股骨短，妇幼诊断股骨缺如。
武汉 同济、协和的医生都没有明确说诊断,只说定期复查看发育情况，再找时机看能否
做截骨延长。
没有家族史，其他器官生长发育没有异常
我把Xray，Xray report和MRI的report post在下面
请问各位，除了 目前定期观察，再找时机看能否做截骨延长 或者 截肢 以外，在 美
国有什么更先进的方法吗？作为家长他们 还是希望看看国外有没有系统的治疗方案，
怕错过早期干预时机。

国几个主要医学院多中心推广研究肖氏反射弧（somatic-autonomic reflex arc）肖氏
手术。NIH已决定由哈佛大学医学院建立DATA Base，收集处理多中心肖氏反射弧手术资
料。
向NIH要求参加该计划的大学太多，在平衡协调上伤透脑筋。我当然希望多多益善，但

）另外他们有没有其它的手段来validate这个发现。我粗略查了查我们的数据库，此位
点的duplication和遗传疾病相关的有下面这么几篇，你如果有时间可以读一读。另外
，似乎所有的研究都处于experimental或early studiy in human，所以临床上的意义
不是特别大。我自己没有读这些paper，所以不知道文章里的研究是不是非常match你的

所想看看大家怎么说. hypertrophic ventricle indicates a congenital disease that affects both nervous system and heart. acute viral myocarditis is very likely and encephalitis (or encephalomyelitis) is also an interesting perspective, which is what the authors did not mention while focusing on the congenital disease. |

are benign, so no worries.
There could be a lot of reasons for infant to have this, it could be racial
melanosis, melanosis oculi, a nerve loop, congenital nevus or something else
. Note that in case of melanosis oculi and congenital nevus, yearly
monitoring is recommended. Still, the doctors won't be too concerned about
them.

先查anti neutrophil antibody .
感觉象是病毒或其他感染，不象drug induced的免疫性中性粒细胞减少。
俺估计医生也会同时做rule out cyclic or congenital neutropenia 的work up.
在大学医院看吧，一般private practice碰到这些病都会refer给大学医院。
不知道你那里大学免疫和过敏科强不强。不强的话，还是得看Peds Hem/Onc.
俺们几周前看了一个congenital neutropenia,一个Pompe disease.最后还是建议一个
去看血液科因为要做骨髓移植。一个去看medical genetics那里去follow up. |

俺就大概知道那里Peds Allergy, Peds Hem/Onc强不强了。
不过再弱的大学program,这个病肯定都见过。你要放下心来，一边继续观察临床上有没
有感染的history,一边最坏的打算是坐bone marrow biopsy.几个月前，俺们头还给了
我几篇关于congenital neutropenia 的文章，某个牛人几年前写的。感觉你家的看起
来不象cyclic或congenital的（就是你说的家族史里paternal side也有neutropenia比
较让人有那么一点点concern）。持续低又有感染，估计会上neupogen. |

章很麻烦，耽误了2个多月，材料才得以从学校寄出。个人觉得申请算是比较顺利。把
我当时准备的情况跟大家说一下也许对将要申请的人有一点帮助。
我个人从这里寄出去的材料包括：
1.国内护士执照的公正，在这里网上找的公证处，在纽约，叫最佳公正，电话
7183212810
2.指纹卡，就在我住的地方的警察局办的，free
3.handbook上的申请表，贴一张照片在右下角（handbook链接：http://www.rn.ca.gov/pdfs/applicants/exam-app.pdf）
这一页有一项是填社会安全号，我当时没有就空着的，没有关系。
4.两张支票，一张办指纹的($51)，一张申请的($75)
5.写了一封信，表示感谢，一张纸，很客气的几句话
从学校寄出的材料：
1.中英文成绩单（学校有，我来之前到学校教务处办好的）
2.学位证书和毕业证书中英文版（来之前自己办好到学校教务处办好的，盖好了章）
3.课程描述9门（内外妇儿、精神科护理、生理、解剖、免疫、微生物，好像是这些，

时许突发心脏病送医急诊，因抢救无效于当日6时许永远离开了我们。
冷丰收博士来自中国吉林省延边大学，他的夫人是一名普通的中学教师，女儿年少体弱
且患有先天听觉障碍。冷老师的突然离世有如晴天霹雳，在她们心里刻上了一道难以磨
