今天受到国内亲戚的亲情求助。小女孩今年11岁。2010年查出染色体异常(Turner
Syndrome). 染色体检查结果是：淋巴细胞培养，G显带核型分析，在400-500水平，核
型：45，X.
目前医生采取的是在肚脐周围注射生长素的做法，先刺激其增长身高。尔后，等到骨龄
闭合，再服用雌激素。
但是，在治疗过程中，身高的增长不是很理想，智力的发育也有待提高等。小姑娘真的
是好可怜, 希望能提供一点信息，也许可以帮到她的治疗，改变她以后的命运。
因此，想求助这里各位医学达人: 美国目前对这种病有没有最新的治疗方法，或最新的
药物？

Also, for radiation worker (as in your example), the limit is 50 mSv. And
that number is for total body. For individual organ, limit is 500mSv.
There is no way to know how harmful it is at such low dose. There is even
theory that low dose is good for you. And even if there is, the benefit
over weight the risk |

文献上说10Gy single dose, about 100% cell death.
同样的cell line，CRO 做了 upto 50Gy/dose, 1 dose/day. After 3 days, the

tissue infections. It is more fast effective than vanco for skin MRSA.
Daptomycin is approved in 1997. It is not a new drug. You should find it in
China.
For Brain MRSA infection. High dose vanco is normal approach. High dose
vanco would penetrate BBB. But Vanco's peak and trough should be closely
monitored. It should be dosing by level. Vanco is nephrotoxic agent. IIt

抗菌谱当然要记，最基本的！
dosing也有好几个lecture，终于知道为什么P1要学pharmacokinetics and
pharmacodynamics了，全在infectious disease里体现了。前几天天天用计算器在算

if it is otitis media 80-90 mg/kd/day divided every 12 hours
400 mg bid seems ok to me for kid weight 36 lb. I thought beta-lactam
killing are time above MIC, so I would rather dose more frequently than big
dose once daily though. |

最后跟的这个医生很酷，既会讲笑话又高效，雷厉风行的，看起来不年轻了，但一直没
结婚的一个女医生。
她今天问我的两个问题算是对我的第一个rotation很好的总结了
病例一：老太太A.fib，现在肠道complete obstruction, 需要尽量把PO的药改成IV
drips. 她问我，她如果把现在病人用的口服的digoxin 和 CCB, 再加上beta blocker
全部改成IV drips for 5 doses，我作为pharmacist，该怎么办？ IV作用快，三种
rate control的药同时IV对心率作用太强，如果pharmacist不打电话给她提醒她，要求
先试2个dose,而不是惯性的一次性开5个dose,病人要是死了，pharmacist就跟她一起坐
牢去了。我没忍住笑到跟她一起坐牢肯定很好玩，后来觉得这是件十分严肃的事情，需
要好好反省！
病例二：昨天她把病人的furosemide停了，spironolactone 一直在用，lab显示钾离子

patient with heart failure. It doesn't have mortality benefit, has dangerous
side effects, and requires careful therapeutic monitoring. If the dig level
is above 0.8, it could produce worse outcome, and increase mortality. Not
that dig should bot be used in patient with heart failure. But I wouldn't
recommend it until all the standard therapy (BB/ACEI or ARB) are being

is a medication that almost has no other side effects except bleeding risk
that can be prevented with patient's compliance and physician's vigilance.
Coumadin is a very successful drug because we can tell whether our patients
are compliance or not and most of healthy care places have the military
like mentality to deal with over or under dosing. Imagining if coumadin

性不能超过 2mSv/h，也就是说这个dose rate对人绝对没有影响。放射性是随着球形的
半径的平方衰减的，假设每个堆半径是200米，那么算400mSV/h的话，在200*sqrt（200
），也就是3公里外基本就没有什么问题了。根据目前的dose rate来看，containment

你说的是Equivalent Dose对吧
然后你觉得我说的概念是Effective Dose，你觉得这两个是不一样的 OK
我前面没理解你说的话 sorry。
这两个概念中间差了一个Wt，但是没关系 icrp已经做的非常好了 Wt在描述群体的
whole body的剂量时候刚好等于1，也就是说 在群众辐射防护中 不需要考虑这个转换
系数
