kind of sad, sounds like she needs a g-tube for enteral feeding for
nutritional support, she is at risk from all the complications from
immobilization such as dvt, aspiration, decubitus ulcer, don't know if the
doctors have figured out the cause of the stroke, her underlying conditions
need to be treated to prevent recurrent stroke,
remember did a nursing home rotation long time ago and this old lady was
bedridden from stroke and aphasic on tube feeding, when i entered her room
she was agitated |

told.
DVT is curable for sure, chemotherapy is not indicated, unless there is
malignancy (pancreatic cancer may be accompanied with migrating embolism). |

no role in the Tx of PE.
pulmonary embolism sounds horrible, actually it is not uncommon though,
while most of the cases shows no symptoms, until a major vessel is blocked.
If you are free of symptom, all you should do is to figure out the source of
the embolus, especially rule out DVT, the most common one, a Doppler may
help with it.
If you have SOB, tarchycardia, or chest pain, stay in the ER, heparin (

would advise against anticoagulant such warfarin. Thrombus will eventually
dissolve or scar down. It takes time and can be frustrating. If she also has
venous incompetence, compression stocking helps, 20-30 mmhg is standard.
DVT is another animal, treatment is different, make sure she doesn't have
that. |

的。大家都知道显卡好坏不只是决定于显存容量，看了NVS 3100M的指标是，首先第一
看显存位宽，才64bit的，这是起决定作用的，即使你的显存大于1G，位宽限制了性能
发挥；其次看显存类别，是DDR3的，比DDR2好，这点还不错；然后就是核心频率，显存频率，纹理渲染单元那些(可以搜wiki)；最后才看显存容量。总体来说64bit的显卡性能再强也强不到哪里。按理说64bit的笔记本独立显卡也有些年头了，不知道为什么这些厂商还在用。台式机如果出现64bit的卡都认为是骗人卡。
要想显卡好可以买娱乐本，如果嫌Lenovo的娱乐系列是idea pad系列的，可以买HP
的dvt，价格便宜，独立显存一般都是1个G，显存位宽大于等于128bit。
我现在这个HP dv5t用了1年半，nvidia 128bit 512M的9600M GT显卡，现在机子整体上还过得去，就是打3D游戏时风扇声音大了点，没办法谁叫显卡功率大，为了散热么；刚买的ThinkpadT500

就不可以了，是DVT-C, 但是从model features看不大出来。

hauppauge or pinnacle.

其实，一开始，我认为可以买一个便宜的带硬盘的dvd recorder, 然后从这个硬盘拷到

如果还是加重，应该看医生check platelet; ultrasound rule out DVT and choose
anticoagulation(这是处方药).有的医院自己的门诊也收没保险的病人（而且还给病人
申请一些补助来让他们少交或不交门诊费用），而且可以walk in的，不药那么长的预
约时间。
只能说那么多了。自己的病自己拿主意。online的建议仅共参考。 |

Match的结果终于尘埃落定，周1等电邮的时候非常紧张，因为成败就在此了，虽然一直
告诉自己，今年只是热热身，成败都可以接受，但到真的揭晓的时候，还是很紧张，毕
竟一旦卷入其中就身不由己，何况自己也还是有所付出。到周4等match program的时候
虽然也有点紧张，但程度已经大大减少，1个原因是毕竟match上才是头等大事，去哪里
尚在其次，还有1个原因是心理还是有所准备了，虽然希望留在home school，但直觉告
诉我恐怕是非常困难了，在我rank list上第二位的program应该就是我要去的地方，果
不其然。经过几天的等待，发现果然是要搬家了。

integration可惜了。对test automation来讲，low level driver,backend database
and report,test framework都比gui来得重要，因为公司内部一个产品出来~pcb设计一
改再改，或者作甲方的来bid test plan,速度和数据才是最重要的。当然这主要看公司
，你们要是有个什么垄断产品~~每年小修小补一下，那也不错 |

call,但每天都要收病人，病人主要是三类leukemia,SCD,再就是癌症了。因为我们这里
黑人比较多，所以几乎天天可以碰到pain crisis的病人，关于SCD的患者首先是要排除
acute chest syndrome,再就是PE,记得前两天有个患者，上看他的时候，突然发现他
SOB,tachycardia,我的第一感觉是PE,查访的时候attending也同意马上order CT angio
,结果发现是subsegmental PE.再就是DVT,这也是比较常见的。关于pain management,
开始都是PCA pump,如果没有过敏史 先用morphine
关于PCA的设定就要因人而异了，morphine basal一般在 1-4mg，on demand 0.5-2,
15min lockout,一般隔几个小时看看患者，如果病人经常push就需要增加basal的剂量
了。如果还不管用的话就要换dilaudid了，2个药的剂量比morphine 5mg=dilaudid 1. |

晚上一直在想前天值班收的一个病人，简单说一下，70yo female, cc:breast
swelling, PMH of breast cancer s/p mastectomy, PE/DVT on coumadin. 病人来时
血压有点低，80/50，但她没有什么不舒服的complaint, 看上去也挺好的，不像septic
。其他vital都没什么不稳定的，我也没在意，她之前有mammogram是另一侧乳房癌复发
了，就order了lab，给她1L NS bolus，就去看别的病人了。3-4个小时后护士page我说
病人心率有150，我觉得自己也要跳到150了，飞跑过去，EKG一拉 A-fib.......急了，
几个小时前还好好的sinus rhythm，怎么在我手里给fib了，死了死了。
你们觉得是什么原因，应该怎么处理？

heparin is a no brainer at those situations, esp if pt has DVT/PE in the
past, and her INR is subtherapeutic.
also very important, she should be on lovenox lifelong as long as her cancer
is not cured, instead of coumadin.
but I am not that sure her rapid onset of Afib in this case is caused by
another epside of PE because she did not present with related symptoms(breas
swelling is her chief complain) and her afib only happened |

s renal function is good.
check PT/INR q weekly in the first month if stable, change to every month.
Need very good compliance and regular follow up. The course of treatment is
