样写的：

以前难道没有看过PCP吗？他们refer你去high risk OB可能预约不会等那么长时间。
你以前没有看过OB,对于OB来说你是位新病人，OB可以给你预约或不给你预约sometimes
it is based on ur insurance situation. 血压很高再有点症状的话，应该马上去急
诊。
俺们科就是很严重的哮喘发作的病人头一次自己打电话来预约，基本上都是一到两个月
以后才能排的上appointment ,一般前台都会告诉病人得follow up with their PCP or
go to ER if it is an emergency. 那时候病人还没在俺们诊所看过病，俺们对这个
病人没有liability的。 |

第一，美国现在对肾癌（renal cell carcinoma）使用的是laparoscopic surgery，这
个手术在美国近十年刚刚被FDA全面approved。该手术就好像一个C section一星期就没
事了。中国还没有开始只有传统的开刀手术对病人的恢复时间会很长。也很痛苦刚开完
刀一星期内呼吸都会痛。
第二，如果你没有保险只要是合法居住的，就没问题。美国医院包括著名的癌症中心MD
Anderson有很多慈善program会对没钱的病人提出资助。在美国没钱看病是对有点小钱
的中产阶级来讲是问题。对你则没问题。
第三，开这个laparoscopic手术大概是五万到七万左右。住院一天。如果是传统手术住
院一星期。至今也不是所有的urology医生都能做的。如果你被资助75%，余下的金额可
分期付款大概50一个月。
第四，在美国开刀，最大牌的医生都不收红包，都会兢兢业业的为你服务。在中国，我
就不讲了。
第五，你是需要全部切除还是局部切除，需要的是一个非常有经验的医生给你建议，在

的底部，有6公分。中国医生说处在第一期，建议做肾切除术。我们打算回美国做，因
为第一家庭可以团聚(ld和5岁孩子在Houston）;第二美国护理条件可能较好。而且气候
好，适合病人恢复。
问题是没有美国保险，不知道典型的做这种手术,医生， 麻醉师和住院费用大概是多少
钱？在中国的费用大概是20万，包括一些红包费。如果在美国的费用比较合理，就决定
在美国做。另外，这个医生讲手术后可能不需要化疗。当然这个要看术后的病例切片决
定。如果美国后续的化疗大概是多少费用？如果是全自费，是否能有什么折扣，或医疗
救济。病人有身份，但没有绿卡。

我已经见到好几个病例，亚洲人，特别瘦，但血糖怎么都控制不好，家族糖尿病史显著
这个问题，已经引起了越来越多的关注，也许会推动pharmacogenomics的发展
公主说道你家族一型糖尿病的问题，我表示怀疑。一型糖尿病一般是autoimmune引起的
，也就是身体产生抗体破坏pancreatic beta cell.身体完全无法产生胰岛素。这样的
病人一般生下来1，2岁就诊断，终身用胰岛素，往往寿命不长。最近我见到的一个一型
糖尿病病人，40岁左右，用了一辈子的胰岛素，还是发展到了stage IV renal failure
，还能活多久完全不可预测。你说的父亲和大伯的糖尿病是二型的吧？二型的也可能会
非常的insulin dependent.
不过家族病史显著的，严格控制饮食，锻炼身体是王道。

两者有什么区别呢？
举个极端的例子。很多病人中，那种还胖乎乎的多半能吃能睡。病人中那种低BMI的，
尤其是有些癌症，或者有胃病的，都瘦的皮包骨了，还生存个鬼啊。绝对是胖子没问题
。战俘集中营里，只怕也是低BMI的死亡率高吧，一个道理。这个网站上给出的figure
，就是两个指标的直接相关性，根本说明不了什么问题。
