是个主要的目的地（所以人们才质疑RNAi在肝脏以外能否work）。如果只是peptide，
renal clearance应该很厉害吧。and居然能过BBB，不知道有没有加其他料。印象中
有某peptide能过BBB但不记得是什么了，也不记得有没有paper跟进。
我个人理解medical imaging是否成功，一个是signaling motif要NB：无毒，yield高，
不受autofluorescence影响，穿透性好，etc；一个是targeting motif要NB：足够特异，
target level够高，最好affinity还能恰到好处，half life能控制好。这两方面我觉得
都不容易解决吧。fluorescence在background、穿透性等方面有局限吧？当然这方面如果
tsien组估计最有实力解决。target方面validated biomarker忒少了，
用于imaging，只有一层specificity check也有点安全隐患。

a mortar under liquid nitrogen and suspended on ice in lysis buffer (20 mM
Hepes, pH 7.7, 0.2 M NaCl, 1.5 mM MgCl2, 0.4 mM EDTA, 1% Triton X-100, 0.5
mM DTT, 100 µg/ml leupeptin, 100 µg/ml aprotinin, 10 mM
benzamidine, 2 mM phenylmethylsulphonyl fluoride, 20 mM -
glycerophosphate, and 0.1 mM sodium-orthovanadate).

acute renal attack，因为T找到抗原后几何速度增殖，肾脏处理不了那么多的死B细胞
。这个转基因方案是没有停药一说的，甚至愈演愈烈，得靠病人自己扛过去，抗体方案

首先效果并没有楼上说的好。Ipilimumab（anti－CTLA－4） objective response
rate在10%左右（各个地方PhaseIIItrial的效果不一），最好的在MSKCC，也在15%左右
。真正有survival benefit的人群在10%不到，提高的average surivival只有3个月。
考虑到toxicity，这个药目前只在PhaseIII和VI的melanoma上用，就是因为那些病人存
活率不超过5%，等于死马当活马医来治疗。希望能有那么10%不到的人能够中奖且收益
。你说的50%的response，包括了CR，PR，和SD，意义不大。在其他的恶性度比较高的
肿瘤包括NSCLC（肺癌）和prostate cancer Ipilimumab效果不好。其实CTLA－4在老鼠
上效果就只对high immunogenic的肿瘤有效。
anti－PD－1看起来效果比Ipilimumab好，但也只能说稍好，副作用稍小，适用的肿瘤

体，一个pd-l1 抗体。 在melonoma, lung cancer, renal cancer 上效果都不错，秒
杀过去几十年搞的那些药，如果这都没什么意义，癌症治疗也没啥搞头了。目前看到的
肿瘤生长曲线已经非常的exciting了，有兴趣的可以去读读这三篇文章，看看肿瘤曲线
。从ctla4 到pd-1/pd-l1，不仅仅副作用下降了，疗效也有了大幅度的提高，再等3年
，让我们来看看pd-1/pd-l1 的长期存活率，那时候会更有意义。
anti-ctla-4, 具体可以参考nejm上hodi 的Ipilimumab文章。 在50％存活率这个点比
较，你会发现这个药的疗效也一般，但是重点不在这里，大家可以比较2年半以后，也
就是30个月以后，这个存活曲线的走向。 晚期黑色素瘤,长期存活率提高15%, 10个人
里，可以救活1.5个人，现在有那个药可以做到这点？我觉得这就是重大突破。
