通过朋友跟他们的牙医了解了一下，他们的观点基本上一致，都是说如果是general
dentist来做，价格会也许会比较便宜，但是periodontist本身也是specialist，所以
跟oral surgeon不会有太多区别
一个orthrodontist看了一下我的estimation，说是在这里基本上各个office的价格差
别就在2-300刀，所以这个报价也是合理的
最后就准备从了，呵呵

昨天，这些人折腾了一天，白天，晚上皮啦啦，也不知道谁把谁杀了，谁把谁带走了。
反正我先生和孩子被绑架了，就不知道是不是已经遇害了。我本来是打算睡觉的，但他
们头上，地下装电磁波攻击器，让我几乎窒息。虽然我没倒下，但其实他们在我身上扎
的毒针已经蔓延，我能感觉到我的呼吸有窒息的感觉。我想我对他们已经非常仁慈了，
但他们不依不饶地攻击我，欲置我于死地，让我非常生气，特别是肆意伤害我家里的人
，绑架我先生和孩子，而且有许许多多无辜的生命毁于他们之手，有必要把他们曝光。
当然我还没分析这里边是那几个人具体攻击我，伤害我的，但他们显然都是一伙的。鉴
于他们伤害我的头部，胸部 和胃部，我就把这几部分的医生都曝光，受害者慢慢去调
查，追查他们吧。

已经达到maximum out of pocket了。但昨天又来一帐单，是assistant surgeon的bill
，共2100刀，打折600多刀后，保险只付500刀，要我自己支付900刀。我查看plan
details，发现有这么一条：关于assistant surgeon的费用，保险最多只cover 25% of
surgeon 费用。今天打电话问保险公司，他们说surgeon费用总共2000刀，所以他们对
assistant surgeon只付500刀。
我的问题是：我能有什么办法减少这个assistant surgeon的bill吗？首先这个bill竟
然比surgeon还高，我可不可以联系这个医生要求减少一点？我还有其它办法吗？谢谢

你有两个选择，一个是去医院做。 你这种不太复杂的orthognathic surgery 99%都是
由chief resident（residency最后一年）主刀，很重要的步骤一般也是他们操刀。
attending在旁边指导，其他resident来assist。好处：便宜。收大部分保险，如果不
是因为美观原因进行手术的话保险cover大部分费用，自己可能掏一两千就够了。
Attending经验丰富且一般会严格指导，虽然每个医院手术强项和手术数量不同，但基
本上这种病例chief resident每周都会做一例 。坏处：经验上确实没有老医生高，主
要体现在一些细节和complication的处理上。但是说chief resident的水平肯定不高那
就太武断了。手术的话技术再好的医生做都不可能保证没有0.001％失败的风险。
你说你找不到收你保险的oral surgeon不甘心，想去芝加哥找，我觉得你很有可能在芝

1. 10-2003 确诊，07-2004终于手术切除，期间Jobs一直在试自己选的diet疗法。扫描发现癌变在扩大才动了手术，这段时间癌转移的多少不可知，但肯定越拖转移的越多。
2.手术后没化疗和放疗。Jobs 2005年演讲说这表明病情乐观，不知是哪个庸医给他灌输的。2004年手术是大面积切除，部分胰腺和胃和小肠，胆和胆管都切除，目的是要消除扩散，但不系统化疗和放疗怎么能最大程度减少扩散威胁？哪个外科大夫这么信自己的手艺？
3.2009年换肝，这在扩散性癌治疗是有争议的，因为换肝后压制免疫反应有可能促进别处癌的生长。也有可能当时肝已经不行了没办法。
这种癌正常治疗大多数可活10年以上，Jobs真该到东岸Hopkins或Harvard找个靠谱的大夫。

病人是symptomatic。
如果是syncope， 我从来不做carotid u/s。 因为将近90%是vasovagal 和
cardiogenic。 剩下的还要考虑orthostasis和seizure。 如果是TIA， 应该是基底动
脉的问题， carotid u/s不如MRA。
可恶的美国医疗体系是你order的test越多赚的钱就越多， 所以就有了过分的order
carotid， 一旦发现了超过70的stenosis， 就把他们refer到surgeon。 surgeon当然
求之不得， 可能是手术容易做， reimbursement高， 这些surgeon一看到我就提醒我
有没有case， 竞争不是一般的激烈。 可以想象他们的门槛会多低。
carotid stenosis并不见得一定是artery-to-artery amboli， 它的存在可能导致供血

整封信说，你的体检结果超过一年，所以请找个civil surgeon重新做体检。
体检完后把新的I-693送来，必须是密封的。有87天respond。
我的问题是，重新体检的RFE都是一样的吗，还是有要做具体的test或者疫苗。
这份信给我的感觉是要重新去civil surgeon那里做所有的事情。
如果要去ob那里开证明，ob怎么知道哪些test或者疫苗可以血检替代呢？我们
civil surgeon去年体检的时候给了一张纸，列了所有的test和疫苗，是不是
ob通过这张纸知道去补做一些test呢？
刚打电话给civil surgeon,说过去做个tb 血检就能重开证明。但开车过去好远，
能不能本体找ob做个tb 血检，给civil surgeon看了，让他开证明？

理先做了检查，重新拍了5张X光，然后大牛型男医生出来，看了X光，让做了几个动作
，认为受伤是因为disc might bulge and irritated the nerve, 现在应该是nerve在
发炎，吃点消炎药应该就没问题了。我问了有没有permanent damage, 他说应该不会，
然后说almost will never happen。
网络的力量是强大的，要不是楼上几位的建议，我是懒得去看spine surgeon的，不过
重新拍了X光（质量比在chiropactor那里拍的要清晰的多），而且还纠正了好几处
chiropactor的误诊（比如chiropactor指着那两张模模糊糊的X光说哦这里有点关节炎
的迹象， spine surgeon看了说没有任何关节炎）
得到的教训就是铲雪一定要屈膝，然后端着铲子不能剧烈扭动身体。出了问题直接找
surgeon, chiropactor真的是江湖郎中。然后spine surgeon说了25岁以后人的脊椎就

是很有保障的。但是我发现美国医生原来挺担心自己的收入。
他们说工作很忙但这是好事，至少还有工作。他们其实担心的是病人少，收益不高。看
来不论挣多少还想往多挣。
那个我认识的麻醉医跟我说了一件事儿。他说现在美国缺少一种药,可惜药名我没记住
。反正是一种老药。现在美国各地的医院都缺这种药。所以限制用。他们医院现在每个
月能用的这种药量，过去不到一个星期就用了。现在这药主要留给Emergency的情况。
总而言之没这种药surgeon就不能做手术。而现在医院的主要收入70%要靠各种手术。没
这种药，surgeon做不了手术，医院收入少就得跨了。更要命的是surgeon没什么收入还
要有很多费用如保险等要付，surgeon入不敷出，就干不下去了，很多外国人surgeon就
得离开美国去其他国家了。即便这药以后不缺了，可是过程还是不可逆的，跨了的就是
跨了，会不来了。
问起这种药为什么缺，因为药厂都是私立的，这药时间长了，没有专利起作用了，生产

我很诧异，您对数学科学几乎没有任何认识。也难怪，您从高中起，就没有接触过
数学，当然就认为数学科学不存在了。
实话实说，我当个radiologist or cardiology surgeon or neurology surgeon,
绰绰有余。但是您让100个surgeons来做数学题目， 估计一辈子都做不出来的。

一个手术室最重要的就是外科医生，好比一个诊所最重要的就是医生。没有外科医生，
其他手术室的工作人员都英雄无用武之地，诊所也一样。所以其他的辅助工作人员很容
易被替换。一个诊所可以解雇所有的员工就是不能解雇医生。同样，手术室里外科医生
就是中心，其他的都是围绕着他/她转。
不管中国还是美国，麻醉都是辅助，经常就是外科医生骂麻醉医生像骂孙子一样。
而FNP是百搭。哪里都需要。精神病院也要。监狱也要。养老院也要。邮轮也要。有

数学统计系的博士生是要求有很高的数理分析能力的，99%的老美没有这个能力。而医学不需要有很高的数学能力，mcat甚至不需要考数学。医学主要还是需要记忆能力就可以了。但是我们做数学题目，记忆能力没有什么用处的。
我很诧异，您对数学科学几乎没有任何认识。也难怪，您从高中起，就没有接触过
数学，当然就认为数学科学不存在了。
实话实说，我当个radiologist or cardiology surgeon or neurology surgeon,
绰绰有余。但是您让100个surgeons来做数学题目， 估计一辈子都做不出来的。
有些科目是需要天才去干的。有些科目只需要记忆就可以干了。

我们查了查，发现那个surgeon很年轻，2000年才毕业，00-07 resident, 07-08
fellow,2008年7月才拿到license. 是不是经验太少了我们应该换个surgeon? 怎么样才

1）是否直接去找个breast cancer的surgeon看比较好？妇科医生推荐的外科医生专做
vascular surgery，是不是不太好？我们这边有个比较好的cancer center,需不需要现
在就去找那里的breast surgeon，而不是妇科医生推荐的外科医生，这样行不行？有何
利弊？
我觉得找breast surgeon好。
2）biopsy是否会insert a microchip？网上看到很多有说会放chip，而且一般术前不
通知病人，有些担心。
没做过。但biopsy是小手术，不用担心。
3）如果不想做biopsy，有没有其他办法？还是这种情况必须做biopsy?数据显示一般
BIRADS4的病人只有15%会是癌症，那剩下的85%不就是白挨一刀？请教biopsy的利弊，
尤其是risks。
做一下放心，不做就观察。可先找别的专家再问问。
4）如果将来还要生孩子，纤维瘤应该切除还是留在里面？biopsy会否影响怀孕？还是
根本不适合再生孩子了？
一点都不会影响怀孕和生孩子。不用担心。

现在我们宝宝有27周了，我们怀这个宝宝真的很不容易。结果25周的时候做B超说宝宝

只是有可能。怎么办呢？难道我们宝宝智力和身体都不好？现在也不可能中止妊娠了，
怎么办呢？我们接下来该怎么办，真的很无助。

doctor to have the varicella shot. So he put a note "will do it at the
family doctor' on the report and asked us to mail the shot record after we
have it and the sealed reports together to our lawyer. We thought it was
fine.
Now my lawyer said it was wrong. He asked us to return to the civil surgeon
and and issue a properly completed I-693 that confirms all vaccinations
completed.
