Both should be fine as long they have the copy of the original documents.
Bring the copy with you to see the civil surgeon
--You might need to do Xray again. You can bring the documents for TB test
positive results with you to the civil surgeon so you don't need to do the
TB test again.
Get the vaccines in places your insurance would cover first and bring the
evidences to the civil surgeon to save money, unless your company cover all
of them. |

6000 是 implant+crown+各种前序，后续拍片， office visits 什么的加一起。
implant 前后要5个appointment， crown 要两个appointment。implant+crown 是大概
5000。可能因为看的是 specialty oral surgeon, not general dentist, 所以贵些？
普通牙医consultation每次40，oral surgeon 光consultation就120. （surgeon赚钱
不要太容易了。。。）
我保险implant/crown cover 50%，但是max benefit 很低，只有1200/year， 所以考
虑LD也在单位加上牙医保险。 假如两个保险各报50%，自己出的可能就是2500-3000？
有人用过两牙医保险吗？
这个牙拔了是因为在国内做的 root canal 不过关（当时觉得比美国便宜好多），所以
现在对国内牙医不是很信任 （没有冒犯国内牙医的意思。。可能我碰到的水平不够）

needs to pay rent, staff and insurance. His hourly rate is only 800.

到了医院，急诊室门口六辆警车一字排开。拍了下脑门，居然忘记了满月之夜？
一路飞奔到ICU。 ICU静悄悄的，全然没有交接班前的热闹场面。夜班任务分配单挂在
墙上，6个病人，3个护士，一人两个。交接班的护士告诉我，两个病人都很简单，是
Trauma，一个是车祸，一个是从自己家的楼梯上摔下来。
车祸的女孩我是知道的，三天前 我上班时进病房的，因为车祸造成了颈椎的不稳定和
C2的骨折。病人刚到病房，脑神经手术医生已经等在那里准备给她装Halo。女孩一看到
Halo就开始声嘶力竭的哭喊……另一层楼的护士给ICU打电话抱怨，你们ICU的护士真会

patient away. Hospital end up hiring MDA again for medical directorship,
but very hard to fill the position. Anesthesiology is much complicated than
people would think. CRNA with 2 yr of schooling, no residency, simply can

MRI诊断为
MCL sprain and stretch, ACL tear, meniscus damage
primary care建议手术，已经refer了一个orthopedic surgeon
还没约。
physical therapist的意见是
肌肉强度非常好，恢复很快，身体条件很可能适合手术要求
还没约Surgeon，想请教一下应该询问哪些内容
以及关于手术的知识，有什么网上资源可以了解一下
也请介绍我去阅读。

I have the same concern before my wisdom teeth are extracted. Some
experience are:
1 Before making a decision to extract wisdom teeth, consult at least 2,
preferably 3 doctors. If all of them say you should have your teeth
extracted, that is probablly true
2 Definitely have a surgeon do the operation. Usually the dentist will refer
a surgeon he knows. My feeling is, a good area and a good dentist usually

发帖数: 31 | 29 我下牙床背面靠近中间牙齿的下方张了一个包，包的位置离开牙龈边缘稍向下一点，
直径大约5毫米。洗牙时发现的，从发现到现在有7 周了，我的牙医上周又查过，说包
没变大，他怀疑是囊肿（MUCOCELE)，但不是很确定，他只是普通的保健牙医。我又看
了一个ORAL SURGEON, 他说是小唾液腺被破坏或刺激后形成的，比如被食物划到（我原
来不知道那里有唾液腺，就是下牙床背面离牙龈边缘向下比较软的地方）。我的保健牙
医和ORAL SURGEON 都很确定这个包不是肿瘤或类似的可怕的东西。ORAL SURGEON说可
以做手术去掉，但我的保健牙医认为如果包不变大就LEAVE IT ALONE, 手术的话会造成
包上方的牙龈萎缩（我的牙龈本来就有些萎缩）。
这个包直径大约5毫米，很光滑，好像中间有液体还有点白色的东西，好像也不疼。我
上周让我的保健医生检查后，上网查了口腔中的包，一下子看到好多很恐怖的 LINK,关
于那个C- WORD 的。心理都有阴影了，总觉得害怕。请问有没有朋友有类似经验？我是
否该做手术把它去掉呢？还是任由它长在那里？

place to whether operate or not. A responsible surgeon will not operate
unless absolutely necessary. Usually, the necessity is specified in
guidelines.
The more you know about your condition, the better you can discuss with your
surgeon.

"well-defined 1cm hypoechoic solid nodule. May represent fibroadenoma. Core
biopsy should be considered",BIRADS4.另外，mm无任何症状，自己也感觉不到lump
，医生检查说mm的乳房比较lumpy,即使B超无发现的那个乳房感觉也很lumpy。
妇科医生让mm去看一general surgeon。请教：
1）是否直接去找个breast cancer的surgeon看比较好？妇科医生推荐的外科医生专做
vascular surgery，是不是不太好？我们这边有个比较好的cancer center,想去找那里
的breast surgeon，而不是妇科医生推荐的外科医生，这样行不行？有何利弊？
2）biopsy是否会insert a microchip？网上看到很多有说会放chip，而且一般术前不
通知病人，有些担心。

much of the drugs.

疫相关的病史。
从四月初开始，我开始恢复运动，包括做倒立。然后发现自己双手关节在早上起来后疼
痛， 痛大概十分钟后消失， 右手的小指出现类似鸡爪子的情况，就是感觉关节需要润
滑。自己用热水泡了也不见效，也停止运动了。五月初见了家庭医生。医生建议观察一
阵看是不是会自行消失。在五月初，我也突然发现早上起来，左脚踝有点痛， 就是类
似跑步崴了脚那种痛，也是一会儿就消失了。大概一两周后，发现坐了一阵起来左脚踝
也有这种痛感，之后发展到有时候走路都有，活动起来蹦蹦的响。六月的时候，再次见
了家庭医生，也把右手小指的情况发了视频给家庭医生看，医生说这是trigger finger
，去hand surgeon那里打一针就好了。另外她觉得我的手指痛的时候比较短，不像是类

发帖数: 89 | 35 对不起大家了，我还得把这事情搬出来。我12月Rectovaginal fistula 手术，但是1个
礼拜后就发现失败了，再次出现fistula,而且connection的地方从几毫米扩大到0.5cm
。3月作了第二次，同时修补sphincter(这个医生说我的sphincter 上次手术后受伤了
，所以要修补)，而且同样是一个星期，失败！！！这次问题更严重了，下边有浮肿，
fistula/connection洞更大了（2cm)。我现在还在恢复准备一般的colon rectual
surgeon 都回避不给我治疗。。
我打算回国治疗，因为这里的surgeon 太不可信，即便我花了很大力气去找最好的有经
验的，可是回头看来都不是，他们的经验基本来自去非洲的volunteer（其实就是找免
费的人体作试验）。下次手术需要妇科和肛肠科合作起来做才行，有没有好的肛肠科/

发帖数: 789 | 37 我当时先将体检要检查的项目给ob看了，她开了检查抗体的验血order。体检的时候将
验血单给civil surgeon 就可以了。不想做皮试和X-ray的，可以验血做GOLD test。
保险cover了大部分的验血费用，我只付了很少的钱，反正比自费打疫苗的钱要少很多
（我这civil surgeon体检，疫苗和验血都得自费，ob开的order保险可以cover）。
如果体检时疫苗和胸透的结果不全也可以交485，civil surgeon会写note。然后等收到
RFE之后再补上就可以了。 |

我老婆预产期年底，今天去做第3次超声波，医生说左肾有积水，心脏壁有个小洞，希
望能痊愈，如果不能的话，宝宝出生以后要做手术。我的天啊，这么小的小孩，如果不
能痊愈的话，出生以后就要做内科手术！我这个做爹的听了真是心如刀绞！我老婆从回
家的路上就开始哭，我还得强颜欢笑哄她，总算把她哄得不哭，去上班了。。。
哎，因为毕业就遇到金融危机，也怪自己能力不行，这么多年找工作一直不顺，我这个
金融硕士一直在干销售的活，能维持生活，但存不到钱买房子，还好我老婆很懂事，从
来不提什么浪费或奢侈的要求。偶尔羡慕别人的房子，我哄哄就过去了。但私底下一直
觉得很对不起我老婆！
现在又摊上这么难受的事！还不能和家里人讲，怕两边的父母担心！实在忍不住上来发
泄一下

上周终于见到surgeon了，得到很肯定的答案说一切都好～ 不用手术了！
医生说，老二的头型在中国孩子中比较少见，前后长，有点儿像舟状头...其实4-9个月
额骨的那条缝本来就差不多会闭合的，等所有的缝闭合后，头还是能长大，就是方式变
了——不是靠骨缝，而是靠新骨头出现，旧的骨头在内部消融... 婴儿的头后期长大的
本来也会慢一些。
最近老二的额头上的棱儿最近好很多了，刚才上了张万圣节的照片，好像基本上也看不
太出来了。
感谢大家的祝福和关心。
希望他的另外的健康问题也能早日解决，不用每天吃药...

继christmas在ER待了一天之后，娃又住了三天院，吊水的过程惨不忍睹。
换了两茬人，千辛万苦在ER埋好了iv，当时我就觉得埋得太浅，但是那护士说没关系的
。果然，前天晚上准备再上抗生素的时候，发现iv掉出来了，肿了好大一个包，不知道
什么时候掉出来的，外面纱布包得很牢。
然后重新再上iv，住院部能上的护士全上了，最后连nicu的护士都叫来了，小手小脚所
有的血管都试了，插进去，在里面搅一阵，偶尔摸到血管，然后又滑掉了。就没一个成
功的。我实在忍不住，去跟医生说，能不能不吊了。
娃实在太太太可怜了，现在不光她有心理阴影，我也有了。

第四周了，昨天已经痛下决心下周回奶配合治疗，今天来发帖记录一下我目前的乳腺炎
经历，给各位新手妈妈一个参考，美帝治疗这方面确实挺麻烦的，千万不要重蹈我的覆
辙。
说来自从我家宝出生一直就一波三折，先是生下来之后一小时喂他奶他差点窒息被CPR
，又是体检查出锁骨骨折，然后月子里脐带感染在儿童医院住了5天挂了无数抗生素—
— 终于熬到第六周结束，我还以为平静的生活即将开始了，结果第七周开始就乳腺炎
发作直到现在第十周了还没痊愈，桑心啊。先来描述一下前因后果吧。
宝出生后一直是大部分泵奶，也说不上为啥，可能是被月嫂影响说这样月子做的好 （
其实是错误的）。导致基础就木有打好，因为泵奶不容易把乳管弄的很通顺，总没有宝
宝吸来的有效。
第七周：
周一开始发现左胸靠近RT的部分有一块硬的，按压还有一点疼，心里知道多半是乳管阻
塞，就想着白天让小宝多吸吸，自己多按摩按摩。其实月子里堵塞的情况也发生过好几
次，都是热毛巾按着硬推推开的，痛到掉眼泪，心想这次也差不多吧。结果白天小宝一

霉国也是surgeon的收入在所有医生中最高，其它科目的医生只有羡慕嫉妒恨的份
再说了，霉国没有回扣的原因是，surgeon的诊所都是自己开的
自己当老板，当然不需要有回扣
国内的医院都是公家的，所以本该名正言顺进医生腰包的钱变成了灰色的“回扣”，好
事变坏事 |

读完“让哥来说说国内国外的医疗对比”有一个感觉，就是笔者没有在国内医院工作
过，也感到很多对中美医疗的评价有些问题， 我不管对美国的评价如何， 但特别
关注对中国的评价， 因为我一直认为这种评价， 特别是来自这些高学历有着中美背景
的评价特别对国内医疗的改变有益。（我出来已有好几年，国内医疗最新改变

看到文章里谈到好医院的名医， 我感到有些好笑， 不是笑作者， 而是笑自己，
自己很幸运，出来前就晋升副高几年 如我不到这儿来， 现在肯定被认为是“名医”
了，还是省会数一数二医院里，数一数二的科室里的“名医”， 我自己也肯定
认为自己是“名医”。 出来前，其实就有不好的感觉，看到周围的大小“名医”
（他们中间一些的确优秀的多了，比较相比较其他医院的医生）， 觉得难道自己

发帖数: 178 | 45 3月10号递交了485,TSC， 然后就是一直焦急地等待中，不料上一周突然收到USCIS通知
，RFE给寄出的，然后更是各种担心，不知是RFE什么东西，有点担心是体检（latern
TB),或者出生证明（封面页寄出了，没有翻译），或者是其他东西。焦急地等待了五天
，今天终于等到了RFE信，内容是：
APPLICANT'S SIGNATURE MISSING (I693体检表）
然后最后一页附上了I693的第一页，在最下面一行用黄色笔给高亮了出来。我仔细看了
一下I693上这一项签名，上面的说明是“Signature- Do not sign or date Form I-
693 until instructed to do so by the civil surgeon",当时体检医生也没提任何签
名的事，所以也就没签名了。
这样的RFE类型，我在MIT上搜了下，好像也没有别人碰到过，当然这个比RFE比出生证
明什么的要好多的。具体的RFE内容如下：

行，必须在体检医生那里再做，光照X-ray也不行。而且RFE里面说“Take this letter
and your enclosed Form i-693 to the civil surgeon who conducted your
medical examination. Only the civil surgeon should open the sealed envelope.
The civil surgeon must administer the TB skin test and record the results
on the Form-I-693." 可是收到的RFE信里就只有两张纸，没看到退回来的sealed i-
693 form啊，这个表格会单独再给我寄回来吗？有收到过这个RFE的TX给confirm 一下
吗？还是他们给我忘记寄了？只有30天回复，挺急的，谢谢 |

A review of your file indicates that you submitted a medical examination (
Form I-693) completed by a designated civil surgeon. The civil surgeon
administered a chest radiograph, but a copy of the radiologist’s report was
not attached to Form I-693. Please return to the civil surgeon and obtain
a copy of the radiologist’s full and formal report, signed by the
radiologist and on official hospital or medical office letterhead. You do
not need to undergo the medical examination.

发帖数: 69 | 49 由于国内注射过BCG疫苗，觉得皮试会出现假阳性。当初没有做TB皮试，直接做了X-Ray
。结果一家三个人，一个过了，另外两个被退回。附了份信说，光X-ray不够。需要皮
试结果。但医生仍坚持BCG接种者，说这样就可以。加了说明，说是BCG接种者。
但感觉，附的信说了光X-ray不够，而且查了网站说了BCG接种的人也需要做皮试。我和
医生讲，结果他很生气。说我挑战他的决定。
想请教大家对付过这种僵局吗？自己有点怕这次寄过去再被退回，比较麻烦。

发帖数: 253 | 50 我所有的疫苗之类的东西都在我family doctor那里打好了并且我family doctor出示了
vaccine records.所以我去civil surgeon那边只做了一个体检和一个必须的血检。当
场付钱，civil surgeon那边还说我做的是最基本的，１３０刀就可以了(其中１１０是
体检的，２０是血检).还说保险不cover，都没有要我填保险。
两个星期之后受到civil surgeon那边来的另一个帐单,不单收我我没有在她那里打的
TDAP疫苗钱，移民体检价格忽然又变成２００刀了。打电话去，就推说要找billing
department，然后billing department就一直没有人接。然后bill５月４号就due，我
还要被害的有过期没付的帐单影响信用记录。 |
