expires in 90 days (it may be different with different surgeons). Also the
i693 is only valid for a year, so you can get everything ready but hold the
health exam with the civil surgeon until your PD is current. |

一年有效
基本都是不能打，有人说td可以，但是移民医生一般会注明孕妇，然后不打。
生那里。应该可以省点钱。td/tdap （十年有效，如果你还有证明）。
孕妇做TB皮试这个见仁见智，CDC认为无害，所以医生们也认为无害。不过CDC同时在09
年给civil surgeon的memo中注明blood test for TB一样有效，并指明其中三种测试方
法可以被接受。问题是TB的血检很多地方不能做，因为需要特殊的kit做血液培养。而
且处于某种原因，移民医生多会说他们不给做。你可以让你lp的ob给她order所有的抗
体测试和TB血测（如果你lp以前测过是阳性，就不用做了。如果在一年内做过是阴性，
也不用做了。），然后但是所有的结果去找civil surgeon，可以省至少150块。 |

Don't have this kind of experience but I think you can use it if it's sealed
with civil surgeon's name sticker and seal. If it was opened by USCIS, you
cannot use it. Some civil surgeons can make you another copy for a nominal
fee. |

can open the envelope and fill the forms.
So I don't know if that's exactly what lettuce137 did, but I am planning to
go to my civil surgeon to get this done.and I suggest you do the same. I was
told that it costs 25 bucks to get the shot and form done.

chest radiograph因为旧的过了一年。这个好办，去移民医生那再照一个，保险应该报
的。

以下内容大家看来可能象笑话，但就切实地发生在我身上了，人品不好吧，哎。
六月初被要求去体检。TB皮试阳性，也在预料之内。然后就开始了没完没了。
医生体检后就再也见不到了，一切都是和他诊所的前台打交道。前台给了无数错误信息
，包括该去哪里、找谁来evaluate 我的TB情况。
前台说去照X光，于是在我的Primary Care 照了。专业radiologist看了以后说没TB迹
象。于是那里的一个registered nurse 填好I693 第八页的Part 6，和x光报告一起交
上去。交了以后没多久诊所来电说，Part 6 必须在Health Department 的TB clinic
做，不能在本人Primary Care. 结果我所住的county health department 没有TB
clinic。打电话问体检诊所怎么办。前台说你就随便去任何一个TB clinic。一查，离
我最近的在另一个州，打电话过去问，人家说我们只接受本州的病人。本州的离我最近

RFE今天来了,说我体检表格里缺gonorrhea test -- 从2016年8月1日起都要做
gonorrhea test（淋病?）。
我的医生是2016年7月29号填完表并签字的，日期是7/29。我8月8号才收到表格，签字
并写了8月8号的日期。USCIS 说体检表以医生签字日期为准(USCIS considers a Form
I-693 complete on the date the civil surgeon signs it; the new gonorrhea
component applies to all I693 reports completed on or after August 1, 2016)
，可是RFE里说 我的表格是8月8号完成的。所以，这是不是USCIS看错了？也就是说我
其实不需要做gonorrhea test?
（离我最近的一个civil surgeon 在单程一个半小时以外，去年体检特别折腾，两个月
才做好，当时来版上求教过，真的不想再去了）

edition Form I-693 (which shows an expiration date of 02/28/2019 at the top
right corner of page 1). USCIS will not accept any previous editions (with
an expiration date of 03/31/2017 or earlier) that a civil surgeon signed and
dated on or after April 28, 2017. You can find the edition date at the
bottom of the page on the Form and Instructions. |

常年潜水，但是学到很多。谢谢大家
rfe还是在经验不足，写在这里警醒后人：
civil surgeon填的表因为要封口所以就没注意看他怎么填的
脑残医生漏填了我的一个section，然后把我和ld的Gonorrhea test都填错了位置。
uscis就把我们的表格又寄回来了，打开看到的一刹那真是有点崩溃（因为正好那时候
说EB3c的名额用完了）
推荐大家一定要让surgeon在封口之前把表复印一份然后自己／律师自查一下。千万不
要因为这种脑残理由被rfe。 |

无奈当时已经开始有些不舒服的症状了，就咬咬牙买了。
开始做了一次血检，看了一次surgeon，动了一次手术。
之后又作一次血检，一次CT，还有两次医院visit。
今天收到保险公司寄来的手术的理赔情况列表，
（注：偶做的只是一个灰常灰常小的切除淋巴结的手术
医院全部手术费用居然5816.50！
偶付了100块的co-pay，其它就全部cover了。
当然，这个五千多有很多虚头，
保险公司可以有很多折扣，但最终他们也付了一千多。
至于血检，偶第一次费用是四百多，保险公司只有不到一半的折扣，付了两百多；
看surgeon是两百多，保险公司有一半的折扣，付了一百多。
再加上费用还没出来的后面的血检、CT，还有pathology那边的lab费用，
偶这一年已经是让保险公司亏钱了，即使他们在账面外还有些additional折扣。
偶今天仔细想一想，如果买了外头的保险，
一年也许能省七八百，但是这5816.50估计能argue下来一半就不错
这样自己也得付几千块。 再去外头要claim？估计没太大戏，而且会拖死人。

find a good surgeon, then call the surgeon to make sure they can accept your
insurance -- Most likely they will if you have PPO.

较高（我们都没去过美国，这都是猜的）。==> It is wrong! The quality of
surgery depends on the experience of the surgeons. If you find a doctor with
lots of experience plus a good record, you need choose him regardless of
Chinese or American. Some US surgeons are really killers. My daughter
suffered from it. A tiny and unnecessary operation resulted in 17-min
cardiac arrest. Lawsuit is very hard even this obvious case. A lot of
lawyers rejected our case. |

($1600+), 快奔5000了。如果加上预备性的步骤，比如CT scan等， 6000 以上了。
这个价正常吗？感觉美国这些牙医都是 private practice, 漫天要价很狠。
谁能推荐个技术好，收费合理的 oral surgeon？ |

门训练的oral surgeon， 他们收费也高. Chinese dentist 好像都是 general
dentist， 不是surgeon...
